    Description / Table of Contents: Zusammenfassung 12 Probanden (11 Frauen und 1 Mann) mit Cushing-Syndrom, die 1960/61 eingehend psychiatrisch untersucht worden sind, wurden im Jahre 1976 bezüglich psychischer Befunde nach der Behandlung des Cushing-Syndroms nachuntersucht. 10 Probanden wurden mit Adrenalektomie behandelt, 1 Probandin nur mit Radiotherapie der Hypophyse und 1 Probandin hat jede Behandlung verweigert. Die Beobachtungsdauer vom Zeitpunkt der Behandlung bis zur Nachuntersuchung oder bis zum Tod (5 Probanden) beträgt im Mittel 15 2/3 Jahre. Bei 7 Probanden gelang es, die überschießende Hormonproduktion der Nebennierenrinde zu beseitigen. Entsprechend war der weitere Verlauf günstig mit völliger Wiederherstellung der Arbeitsfähigkeit. Die Erscheinungen des endokrinen Psychosyndroms bildeten sich weitgehend zurück. Eine dauernde Veränderung der Persönlichkeit wurde nicht beobachtet und die vitale Abhängigkeit von der hormonalen Substitution wurde gut ertragen. Bei 4 von 5 Probanden mit ungünstigem Verlauf war es nicht gelungen, die Nebennierenrindenhormonproduktion zu normalisieren. Im Laufe weniger Jahre kam es zur Invalidität und Pflegebedürftigkeit. Es bildete sich ein mehr oder weniger deutliches organisches Psychosyndrom aus. Im Gegensatz zum Langzeitverlauf der Hypophysen-insuffizienz, wo unspezifische Persönlichkeitsfaktoren einen wichtigen Einfluß haben, hängt beim Cushing-Syndrom die Spätprognose ganz über-wiegend von der erfolgreichen Beseitigung der pathologischen Produktion von NNR-Hormonen ab. Die künstliche Substitution bedeutet in bezug auf das seelische Leben nicht notwendigerweise einen Nachteil.
    Notes: Summary Twelve patients with Cushing's syndrome (11 females, 1 male) who were examined for psychopathologic alterations in the course of the endocrine disease during the years 1960/61, were reexamined in 1976 regarding psychiatric sequelae after treatment. Ten patients had been treated by adrenalectomy, one patient by radiotherapy of the hypophysis, and one patient had refused any treatment. The average lapse of time from operation to reexamination or death (five patients) was 15 2/3 years. In seven patients it had been possible to stop completely the excessive production of adrenal hormones. Correspondingly the long-term result was favorable, with complete recovery of the ability to work. The previous symptoms of the endocrine psychosydrome disappeared almost completely. There was no permanent alteration of personality and the vital dependence on hormonal substitution was well tolerated. In four of the five patients with an unfavorable course of the disease it had not been possible to stop the excessive production of adrenal hormones. These patients became increasingly invalid and had to be cared for. Symptoms of chronic organic brain syndrome developed. In contrast to the course of panhypopituitarism where nonspecific personality factors are of considerable importance, in Cushing's syndrome the long-term prognosis depends entirely on the successful stoppage of the overproduction of adrenal hormones. The artificial substitution is not necessarily a disadvantage for the psychic functioning of the patient.
