Zusammenfassung In der Chirurgischen Universitätsklinik Düsseldorf wurde von 1959–1974 bei 18 Patienten eine familiäre Polyposis diagnostiziert. Bei 3 Patienten handelte es sich um Geschwister, wovon 2 die Operation 1964 ablehnten. Sie starben im Gegensatz zu ihrer operierten Schwester 7 bzw. 8 Jahre nach Diagnosestellung. Die typischen Symptome, die durchzuführenden diagnostischen Maßnahmen und die Therapiemöglichkeiten werden aufgezeigt. Aufgrund eigener Erfahrungen sollte die Ileorectostomie zugunsten der Proktokolektomie (evtl. heute mit Kockschem After) verlassen werden. Die Prognose bei frühzeitiger Operation (auch bei carcinomatöser Umwandlung) ist gut. Aus unserem Patientengut starben nur diejenigen, die die Operation ablehnten und jene, die zu spät zur Operation kamen, so daß nur noch als Palliativmaßnahme ein Anus praeter angelegt werden konnte.

Summary Between 1959 and 1974 we observed 18 patients with familial intestinal polyposis. 4 patients were from one family. Two brothers refused the operation and died 7 respectively 8 years after diagnosis had been made. The typical symptoms, the diagnostic procedure and the therapy are demonstrated. We prefer the proctocolectomy (eventually with the “Kock-Ileostoma”), because we observed a lot of recidivs after ileorectostomy. The prognosis of the familial intestinal polyposis is good, if the operation is performed early (also in cases of carcinomatosis). Only those patients died, who rejected the operation or were operated too late.
