    Notizen: Abstract The purpose of this double-blind, placebo-controlled, comparative study was to evaluate the specific clinical efficacy and tolerance of human leukocyte interferon-α (2×l06 IU/g) and podophyllotoxin 0.5% incorporated in a hydrophilic cream to cure genital warts. Preselected Asian women (n=60) aged 18–40 years (mean 22.9), with a clinical and biopsy-confirmed diagnosis of genital warts, harboring 322 lesions (mean 5.36) were randomly assigned to three parallel groups to receive one of the two test drugs or placebo. Each patient received a preceded tube (40 g) containing either human leukocyte interferon-α, podophyllotoxin, or identically appearing placebo cream for 3 days' usage. In addition to written instructions, each subject was shown how to apply a minimal amount of trial medication on their lesions thrice daily for 3 consecutive days per week (maximum nine topical applications per week). Patients were examined on a weekly basis, and a cure was considered to be a biopsy-confirmed, total elimination of a lesion. Moreover, patients cured during the study period were spared further therapy, and were requested to return in 16 weeks to monitor for the occurrence of a relapse. The remaining patients' empty tubes were collected, and similarly precoded replacement tubes were given to continue the treatment; a total of 214 tubes were used. The study was scheduled for 16 weeks with 4 weeks of active treatment. Individuals were excluded from the study due to pregnancy, breast feeding, or the receiving of any type of antiviral therapy during the 4 weeks preceding enrollment. Better results were obtained with leukocyte interferon-α cream than with podophyllotoxin both in terms of the number of cured patients (18/20 vs. 12/20, or 90% vs. 60%; P〈0.0285) and the number of eliminated lesions (105/110) vs. 87/108, or 95.5% vs. 80.5%; P〈0.001. In the placebo group 4/20 (20%) patients were cured, indicating lower results. Out of the 60 patients 51 (85%) complained of no drug-related symptomatic side effects. However, the most frequent nonobjective adverse symptoms experienced by the patients were mild tenderness 4/60 (6.6%), burning sensation 3/60 (5%), and transitory increase in body temperature (〉38°C) accompanied by headache and itching 2/60 (3.3%). None of the patients suffering from these side effects elected to discontinue the study. The patients were evaluated for 1 year after the initial visit, and among the 34/60 (56.6%) cured patients two had a relapse after 9 months. In conclusion, the clinical results of this study demonstrate that leukocyte interferon-α (2×l06 IU/g) is more efficacious than 0.5% podophyllotoxin and placebo in hydrophilic cream, and substantiates that along with mild, nonobjective, transient, localized, adverse symptoms, human leukocyte α-interferon therapy is more effective and better than podophyllotoxin in cream for curing genital warts in women

    Notizen: Abstract The purpose of this double-blind, placebo-controlled study was to determine and compare the clinical efficacy and tolerance of human leukocyte α-interferon (incorporated 2 × 106 IU/g) in hydrophilic cream to cure genital warts. Preselected Asian female patients (n=150) aged 18–40 years (mean 22.5), with the clinical and biopsy-confirmed diagnosis of genital warts (mean 2.64), predominantly flat vaginal condylomas, were randomly allocated to 3 parallel groups. Each patient was given a coded tube containing 80 g placebo/active preparation with a graduated applicator. Patients were instructed to inject 6 g of the either alloted placebo/active cream deep into the vagina thrice a day for 3 consecutive days (group A) or 4 consecutive days (group B) per week, and if not cured the same treatment was extended to 3 more weeks (maximum 4 weeks active treatment). To assess the clinical efficacy patients were examined on a week-to-week basis. A total clearance of warts (biopsy-confirmed) was evaluated as a complete cure. Patients cured during the treatment were spared further treatment and were requested to visit us after 16 weeks for relapse control. As for the remaining patients, empty tubes were collected, and similarly coded replacement tubes were given for further treatment (in total 588 tubes were used). By the end of the treatment 57.2% lesions (227/397) were eliminated in all the groups: 48% patients in group A, 90% patients in group B, and 10% patients in placebo groups taken as completely cured. Of the 150 patients 128 (85.3%) did not complain of any drug-related adverse symptoms. Transitory increase in body temperature (mean 38.4°C), accompanied by headache (14.6%) and generalized itching (6.6%) were the most frequently reported side effects; however, treatment was well tolerated by all the patients, and there were no dropouts. Our findings indicate that clinical efficacy is dose dependent, that is, the results of group B were significantly superior to that of group A (P 〈 0.05). Of the 49.3% cured patients (74/150) followed up for 6 months (monthly basis) seven had a relapse, and none had reinfection. It is concluded that clinical efficacy of leukocyte interferon-a to cure genital warts is dose dependent. These results further support the view that leukocyte interferon-a incorporated in hydrophilic cream can be considered a reliable, safe, and home-based treatment to cure vulvar and vaginal warts.

    Notizen: A direct Boundary Element formulation and its numerical implementation for inelastic transient dynamic analysis of three-dimensional solids is presented. The formulation is based on an initial stress approach and is the first ever of its kind in the field of the Boundary Element Method. This formulation employs the Navier-Cauchy equation of motion, Graffi's dynamic reciprocal theorem, Stokes' fundamental solution and the Divergence theorem, together with Kinematical and Constitutive equations to obtain the pertinent integral equations of the problem in the time domain within the context of small displacement theory of elastoplasticity. The boundary integral equations are cast in an incremental form, in which elastoplastic relations of the incremental type are used for the material description. These equations are then solved using a time-stepping algorithm in conjunction with an iterative solution scheme to satisfy the constitutive relations. Higher order shape functions are used to approximate the field quantities in space as well as in time. Finally, the applicability of this methodology is demonstrated by presenting a few example problems.

The present formulation employs the space and time dependent fundamental solution (Stokes' solution) and Graffi's dynamic reciprocal theorem to derive the boundary integral equations in the time domain. A time-stepping scheme is then used to solve the boundary initial value problem by marching forward in time. Higher order shape functions are used to approximate the field quantities in space as well as in time, and a combination of analytical (time-integration) and numerical (spatial-integration) integration is carried out to form a system of linear equations. At the end of each time step, these equations are solved to obtain the unknown field quantities at that time.Finally, the accuracy and reliability of this algorithm is demonstrated by solving a number of example problems and comparing the results against the available analytical and numerical solution.

    Notizen: In this work, the Direct and Indirect Boundary Element Methods as applied to dynamic soil structure interaction problems, are compared. Both the methods are used to find the dynamic stiffnesses of rigid surface and embedded foundations. For surface foundations, the results obtained by the two methods were very close for the frequency range considered, but for embedded foundations at higher frequencies, the Indirect Boundary Element Method produced erroneous values for the real part of vertical compliance. The possible reasons for this discrepancy are also studied in this paper.

    Notizen: The axial and lateral load-displacement response of single piles and pile groups during the secondary consolidation (creep) phase of deformation is obtained by idealizing the soil as a linear viscoelastic material. The boundary element method is used to model the soil halfspace and the piles, which are assumed to behave elastically, are idealized as beam-columns. By using the correspondence principle, the problem is solved in the Laplace transform domain and the final solution is obtained by numerical inversion of the transformed data. The salient features of the viscoelastic response are identified in several non-dimensional plots which encompass the spectrum of geometrical and material parameters commonly encountered in practice. The results of the analysis compare favourably with the limited data available in the literature.
