Zusammenfassung Die durch neuere Entwicklungen eingeführten, ausgeweiteten und eingeschränkten Indikationen chirurgischer Eingriffe werden gegliedert in mehr diagnostische und mehr therapeutische, die letzteren in direkte (Resektionen und/oder prothetischer Ersatz, Transplantationen) und in indirekte (auf pathophysiologischen Erwägungen beruhende) Eingriffe. Die Gesichtspunkte des Internisten werden zu den präoperativen Untersuchungen, zur Differentialdiagnose, zur Indikationsstellung und zur Nachbehandlung eingebracht. Eine Ausdehnung chirurgischer Eingriffe auf sorgfältig ausgewählte metastasierte Tumoren im Rahmen einer multidisziplinären onkologischen Therapie wird empfohlen.

Summary Recent developments have both extended and restricted the indications for surgical intervention. In this review, surgical operations are divided into direct (e. g. resections, prosthetic substitution, organ transplantations) and indirect ones performed on the basis of pathophysiological factors. The author considers the internist's points of view on preoperative diagnostics, differential diagnosis, indications and postoperative treatment. He recommends the extension of surgery also to carefully selected cases of metastasizing tumours within a combined treatment.
