Isolated avulsion fracture of the lesser tuberosity of the humerus is a very rare injury. Diagnosis is often not found right away. In comparison with the literature, the presented case shows the importance of exact anamnesis and physical examination in finding the right diagnosis. Late surgical treatment with endoscopic evaluation of possible attendant injuries can also be useful, as shown in the following case.

Der isolierte knöcherne Ausriß der Subskapularissehne ist eine seltene Verletzung. Die Diagnose erfolgt oft nicht auf Anhieb. Anhand des vorgestellten Falls werden im Vergleich mit der Literatur die diagnostische Bedeutung einer exakten Anamnese und körperlichen Untersuchung dargelegt. Auch die verspätet eingeleitete operative Fragmentrefixation mit arthroskopischer Abklärung möglicher Begleitverletzungen erweist sich in dem vorliegenden Fall als sinnvoll.
