    Description / Table of Contents: Summary Synovitis (inflammatory arthritis), acne (pustulosa), pustulosis (psoriasis, palmoplantar pustulosis), hyperostosis (acquired), and ostitis (bland osteomyelitis) are symptoms forming the acronym SAPHO, which is a syndrome of nosologic heterogeneity. All entities forming the SAPHO syndrome are connected by a non-obligate dermato-skeletal association with an aseptic pustulous character. 86 cases were analyzed clinically, radiologically and by histology/histopathology. 31 adult patients showed the typical triad of pustulosis palmo-plantaris (psoriatica, PPP), sterno-costo-clavicular hyperostosis (SCCH), and “productive” spondylopathy, which we define as entity I: spondarthritis hyperostotica pustulo-psoriatica (Spond.hyp.pp). Twelve adolescent and 13 adult patients showed entity no. II: chronic recurrent multifocal osteomyelitis (CRMO), being characterized by non-purulent osteomyelitis of plasma-cell sclerotic type, potentially being a reactive inflammatory process. 50% of the adult patients with CRMO showed PPP. Differentiation between these two entities is possible by detection of ossifying enthesiopathy in cases of Spond. hyp.pp and primarily chronic osteomyelitis in cases of CRMO. Two more entities or abortive forms of group I and II are III: the inflammatory syndrome of the anterior chest-wall (ACW syndrome) and IV: the more productive form of isolated sterno-costoclavicular hyperostosis (SCCH). Both are connected quite frequently to HLA-B-27-independent forms of spondarthritis and to pustulous dermatosis. More rarely we find osteo-articular symptoms in cases of acne pustulosa, which form group V: acne-associated spondarthritis and CRMO in the case of acne. Adult forms of CRMO with different forms of appearance (lumo-sacro-iliac hyperostosis with retroperitobeal fibrosis, pelvic type with affection of the hip-joint) are described. The immunologic theory of a “reactive osteomyelitis” potentially triggered by saprophytes is described. The inverse acne triad is brought in a context of skin symptoms. A case of intercurrent post-partum symptoms together with ulcerative colitis is described. Three cases of patients with Crohn‘s disease are described. Clinical features, radiological findings, and histopathological elements are brought together to determine the connections between the different entities and the possibilities of differentiation. With these elements together with bone-scan, it is often not necessary to obtain a bone specimen. Therapeutical possibilities, especially concerning CRMO, are discussed. “SAPHO syndrome” is more a sign-post on the way to a more subtle diagnosis when it comes to hyperostotic, skin-associated diseases, and it needs interdisciplinary work to clear the situation.
    Notes: Zusammenfassung Synovitis (Arthritis), Akne (pustulosa), Pustulose (Psoriasis, Ppp), Hyperostose (akquirierte) und Osteitis (blande Osteomyelitis) sind als Symptome zum Akronym eines Syndroms 1.Ordnung vereint, das in sich inhomogen und nosologisch heterogen ist. Es sind zwei Entitäten und drei Syndrome 2. Ordnung, die durch eine nicht obligate dermato-skelettale Assoziation aseptisch pustulösen Charakters miteinander verbunden sind. Unser eigenes Kollektiv umfaßt 86 SAPHO-Fälle, die wir klinisch, radiologisch und histopathologisch analysiert haben. Von diesen bieten 31 erwachsene Patienten die Trias aus Pustulosis palmoplantaris (psoriatica), sternokostoklavikulärer Hyperostose und überwiegend produktiver Spondylopathie, potentiell mit peripherer Arthritis, diagnostisch also die (I.) Spondarthritis hyperostotica pustulo-psoriatica (Spond.hyp.pp). Sie entspricht der Pustulotic arthroosteitis, die aber symptomatologisch eine ausgeweitet unscharfe Grenzziehung aufweist. 12 jugendliche und 13 erwachsene Patienten bieten die (III.) Chronisch rekurrierende multifokale Osteomyelitis (CRMO), die gekennzeichnet ist durch nichteitrige Osteomyelitiden mit dem wahrscheinlich reaktiven Entzündungsprozeß vom plasmazell-sklerotischen Typ. Die adulte Form der CRMO wird in der Hälfte der Fälle von der Pustulosis palmo-plantaris (Ppp) begleitet. Pathogenetische Unterscheidungsmerkmale der beiden Entitäten sind die ossifizierende Enthesiopathie der Spond.hyp.pp einerseits und die primär chronische Osteomyelitis der CRMO andererseits. Zwei weitere zugehörige, aber abortive Krankheitsbilder werden definiert im Zusammenhang einerseits mit (II.) der sternokostoklavikulären Hyperostose (SCCH) und andererseits im Zusammenhang mit (IV.) dem entzündlichen Syndrom der vorderen Thoraxwand (ACW-Syndrom). Beide Syndrome kommen selten isoliert vor, meistens sind sie von Symptomen B-27-unabhängiger Spondarthritiden und fakultativ von pustulösen Dermatosen begleitet, ohne zunächst – oder noch „undifferenziert“– einer der Entitäten I oder III zuzugehören. Die selteneren, der Acne pustulosa (conglobata und fulminans) assoziierten osteoartikulären Krankheitsfälle schließlich trennen wir als (V.) Akne-Spondarthritis und Akne-CRMO von den pustulo-psoriatisch geprägten Krankheitsbildern I bis IV ab. Als Neubeschreibungen werden einbezogen die adulte Form der CRMO und deren lumbosakroiliakale Hyperostose, zweimal mit retroperitonealer Fibrose, sowie der Beckentyp der adoleszenten, überwiegend weiblichen CRMO und deren azetabulär lokalisierte chronische Osteomyelitis mit sympathischer Coxitis. Die immunologische Theorie der „reaktiven Osteomyelitis“ wird vorgestellt; und kommensalen Keimen, Propionibacterium acnes und anderen wird die hypothetische Rolle der antigen-potenten Ätiopathogenese zugeschrieben. Die inverse Akne-Triade (inkl. Hidradenitis suppurativa) wird in die Hautsymptomatik einbezogen. Über eine interkurrent postpartale Exazerbation mit Colitis ulcerosa wird berichtet, und 3 Fälle mit Morbus Crohn werden einer enteropathischen Trias zugeordnet. Methodisch interdisziplinär erarbeitet wird die Morphologie auf die Klinik projiziert. Verwandte Krankheitsbilder, wie auch das akquirierte Hyperostose-Syndrom, werden gewürdigt und eingeordnet. Die radiologisch relevante Morphologie ist charakteristisch bis typisch, bedarf aber der Beachtung des überwiegend szintigraphisch darzustellenden Befallmusters und, aber nicht immer, der histopathologischen Differenzierung, jedenfalls der Differentialdiagnose wegen. Therapeutische Erfahrungen werden mitgeteilt und vorläufige Empfehlungen erarbeitet. Die damit erfolgte Differenzierung des SAPHO-Syndroms und die jeweils entsprechende nosologische Zuordnung des Einzelfalles sind aus klinischen und therapeutischen Gründen erforderlich. Der Syndrombezeichnung „SAPHO“ kann nicht ein diagnostischer Rang, aber eine nützliche, wiederum nur vorläufige Hinweisfunktion zukommen, der die rheumatologisch-interdisziplinäre Diagnosefinding folgen muß.
