Zusammenfassung Beim Makrocarcinom sind die Behandlungsergebnisse sei über 10 Jahren weitgehend konstant. Die absoluten 5-Jahres-Heilungsergebnisse bewegen sich zwischen 10 und 15 %. Anders die Ergebnisse beim Frühcarcinom. Hier gibt es 5-Jahres-Heilungsquoten bis 90 % und mehr. Daraus resultiert die systematische Suche nach dem Frühcarcinom. Praktisch geschieht dies durch regelmäßige Kontrolle der gefährdeten Gruppen. Das Behandlungsverfahren der Wahl ist für Makro- und Frühcarcinom die subtotale Resektion. Die Kardiaresektion sollte durch die Gastrektomie ersetzt werden und die Gastrektomie definierten Befunden vorbehalten bleiben.

Summary For over a decade, therapeutic results in cases of macrocarcinoma have remained almost constant. The absolute 5-year survival rate is 10-15 %. However, in cases of early detection, the 5-year survival rate is over 90 %. Systematic screening to detect the cancers early has resulted. The practical results is the systematic observation of high-risk groups. The therapy of choice for macrocarcinoma and cancer that is detected early is subtotal resection. Resection of the cardia should be replaced by gastrectomy, which should also be performed only when defined diagnostic criteria are met.
