    Description / Table of Contents: Zusammenfassung Im Sputum von Patienten mit chronisch obstruktiven Atemwegserkrankungen werden unter anderem Immunglobuline, vor allem IGA sowie proteolytische Fermente und deren Inhibitoren gefunden. Bei 8 Patienten wurde im Vergleich mit dem klinischen Verlauf untersucht, ob bei täglicher Analyse dieser Parameter Rückschlüsse auf das Krankheitsbild gezogen werden können. Unsere Untersuchungen ergaben eine Zunahme des IGA-Spiegels im Sputum unter der Therapie mit Glucocorticoiden und Antibiotica bei großen täglichen Schwankungen. Die proteolytische Fermentaktivität veränderte sich nur wenig unter der Behandlung. Alpha1-Antitrypsin, als wichtigster Proteasehemmer des Organismus, spielt in der Lunge eine untergeordnete Rolle, er imponiert eher als Ausdruck der Entzündung. Sputumeigene Fermentinhibitoren scheinen bei der Inaktivierung der Proteasen im Bronchialraum in unterschiedlichen Konzentrationen zur Verfügung zu stehen. Bei allen Patienten schwankten die Meßwerte erheblich bei den täglichen Untersuchungen.
    Notes: Abstract The sputa of patients affected with chronic obstructive airway diseases contains among other things, immunoglobulins, mainly IGA as well as proteolytic ferments and inhibitors of them. We analyzed these parameters daily in 8 patients and compared the values with the clinical course of the disease to see whether the values measured allowed any conclusions on the clinical picture. Our investigations showed a rise in the IGA-level in the sputa during the administration of glucocorticoids and antibiotics but with large daily variations. Proteolytic ferment activity showed only minor alterations during this treatment. Alpha1-antitrypsin, the most important protease inhibitor in the organism, is of less importance for the lungs and is more important as a sign of inflammation. Ferment inhibitors intrinsic to the sputum seem to be available in varying concentrations in the bronchial space during inactivation of the proteases. The values measured at the daily examinations varied considerably in all patients.

    Notes: Abstract Spirometry, full-bodyplethysmography, and inhalation challenge tests are the most frequently used methods for lung function testing. In all, 677 examinations were performed among exposed employees at the Degussa carbon black plant in Kalscheuren/Germany using the above mentioned methods, in order to detect whether there is a measurable impact of carbon black fine dust on pulmonary function, or a higher prevalence of obstructive air-way diseases among the study candidates, and whether fine dust exposure is related to the prevalence of bronchial hyperresposiveness. Within the smokers' group carbon black dust exposure reveals a (minimal) impact upon the lung function of the study subjects. However, the impact of fine dust exposure within that group is less significant than the influence of smoking on pulmonary function. Nevertheless, smokers are displaying significant more frequently signs of obstructive airway diseases compared with nonsmokers. In the smokers' group we found 7.3% of study subjects with signs of obstructive airway diseases compared with 3.9% in the group of nonsmokers. No significant impact of fine dust exposure on lung function could be detected within former-and nonsmokers. The overall percentage of 5.1 % is not higher than that of subjects with bronchial hyperresponsiveness in other comparable studies.

    Description / Table of Contents: Zusammenfassung 9α-Fluor-16α, 17α-isopropyliden-dioxyprednisolon (FIDP) hemmt nach Staubreiz die Emigration von Makrophagen in den Alveolarraum. Die Aktivität der zur Phagocytose im Alveolarraum zur Verfügung stehenden Zellen ist nicht herabgesetzt. Auch bei Versuchen mit isolierten Zellen war keine durch FIDP bewirkte Phagocytosehemmung zu messen. Kulturzellen veränderten sich morphologisch unter dem Einfluß von mikrokristallinem FIDP stark. Außer einer ausgeprägten Epithelisierung der spindligen Fibroblasten bildeten sich um die Kerne große Höfe gekörnten Plasmas. Anschließend war eine starke Vacuolisierung des Plasmas zu beobachten. Die morphologischen Veränderungen waren reversibel. Die Milchsäureentstehung in diesen Kulturen war zeitweise total gehemmt. Der O2-Verbrauch entsprach dem nur geringfügigen Wachstum dieser Kulturen. Die Stoffwechseldepression war ebenfalls reversibel. Da die durch gelöstes Glucocorticoid hervorgerufene Stoffwechsel- und Wachstumsminderung nach Auswaschen rückläufig war, während sie bei gleichen Versuchsbedingungen unter FIDP bis zu 4 Wochen anhielt, nehmen wir eine Phagocytose der Glucocorticoidkristalle durch die Zellen an, die ihre Wirkung in den Phagocyten über 4 Wochen aufrechterhalten. Am Beispiel dieser Wirkung ist zu diskutieren, ob der Einfluß der Glucocorticoide auf die entzündungsaktiven Zellen auf den gleichen Stoffwechselveränderungen beruhen, wie wir sie in unseren Kulturen messen konnten.
    Notes: Summary 9α-fluor-16α,17α-isopropyliden-dioxyprednisolon (FIDP) inhibits after dust irritation the emigration of macrophages into the alveolar space. The activity of the cells available to phagocytosis in the alveolar space has not been reduced. Also in experiments with isolated cells no inhibition of phagocytosis, induced by the action of FIDP, could be measured. Culture cells altered strongly morphologically under the influence of micro-crystalline FIDP. Besides a marked epithelisation of spindle fibroblasts, large halos of granular plasma formed around the nuclei. Following, a strong vacuolisation of plasma could be observed. The morphological changes were reversible. Lactic acid production in these cultures has been completely inhibited for some time. O2-consumption corresponded to the only feeble growth of these cultures. Metabolic depression was also reversible. As the metabolic and growth reduction produced by the solved glucocorticoid was retrograde after ablution while it continued under FIDP under the same test conditions up to 4 weeks, we assume a phagocytosis of the glucocorticoid crystals by the cells which maintain their activity in the phagocytes over a period of 4 weeks. Tating this effect as example, there is to discuss whether the influence of glucocorticoids on the active inflammatory cells are based on the same metabolic changes as those we were able to measure in our cultures.

    Description / Table of Contents: Zusammenfassung Das Wachstum von Fibroblastenkulturen wird durch Glucocorticoidbehandlung verzögert, jedoch nicht durch die gleiche Behandlung mit Mineralcorticoid. An Epithelzellkulturen wird die Wachstumshemmung durch Glucocorticoide nur bei extrem hoher Dosierung gefunden, Mineralcorticoid wirkte eher wachstums-fördernd. Die als „bimodale Glucocorticoidwirkung“ beschriebene Erhöhung der Mitoseraten in den Kulturen über die Norm nach Absetzen hoher Konzentrationen oder bei ausschleichender Behandlung ist nicht spezifisch für mesenchymale Zellen, sie konnte bei gleicher Behandlung auch an Epithelzellen ausgelöst werden. Die reversible Stoffwechseldepression durch Corticosteroideinwirkung war dagegen spezifisch für Fibrocyten. Sie wurde an Epithelzellen nicht gemessen. Extrem hohe Mineralcorticoidkonzentrationen bewirkten ebenfalls eine reversible Stoffwechseldepression an Fibrocyten. Strukturelle Veränderungen durch Corticosteroidbehandlung wurden ebenfalls nur an Fibroblasten beobachtet. Epithelzellen blieben unverändert. Nach Mineral-corticoidbehandlung in extrem hoher Dosierung zeigten sich weit geringere Struktur-veränderungen an den Fibrocyten als nach Glucocorticoideinwirkung. Diese verschwanden bei allen Corticosteroiden mit der Normalisierung des Stoffwechsels. Aus den Versuchen kann geschlossen werden, daß die im Bereich klinischer Dosierungen spezifische Stoffwechsel- und Wachstumsdepression mesenchymaler Zellen durch Glucocorticoideinwirkung den Entzündungsmechanismus maßgeblich beeinflußt. Die geringe Wirkung der Mineralcorticoide reicht wahrscheinlich nicht für eine Beeinflussung der Proliferation von Entzündungszellen aus. Die geringe Wirkung der Corticosteroide auf kultivierte Epithelzellen spricht ebenfalls für eine spezifische Wirkung der Nebennierenrindenhormone auf mesenchymale Zellen.
    Notes: Summary Growth of fibroblasts cultures is retarded by application of glucocorticoids, but not when mineral corticoids are applied in the same way. The growth inhibition of epithelial cell cultures caused by glucocorticoids is found only when using high dosages; mineral corticoids show rather a growth encouraging effect. The increase in the rates of mitosis in cultures above normal after stopping the high concentrations or with lingering out the application, described as “bimodal action of glucocorticoids”, is not specific for mesenchymal cells, it could also be released in epithelial cells treated in the same way. The reversible metabolic depression by the action of corticosteroids was, contrarily, specific for fibrocytes. It has not been measured in epithelial cells. Extremely high concentrations of mineral corticoids also resulted in a reversible metabolic depression of fibrocytes. Structural changes by corticosteroid application have also been observed in fibroblasts only. Epithelial cells remained unchanged. After application of mineral corticoids at extremely high dosages, the fibrocytes showed far less structural changes than after glucocorticoid application. These structural changes disappeared with all corticosteroids used when metabolism became normal again. It may be concluded from these experiments that the specific metabolic and growth depression of mesenchymal cells within the range of clinical dosages, induced by the action of glucocorticoids, is considerably affecting the inflammation mechanism. The minor action of mineral corticoids is hardly sufficient to affect the proliferation of inflammatory cells. The little effect of corticosteroids in case of cultivated epithelial cells also pleads for a specific action of adrenal cortical hormones on mesenchymal cells.
