Zusammenfassung Die Dignität der Onkocytome der Schilddrüse ist umstritten. Von 1965 bis 1973 wurden an der Chirurgischen Universitätsklinik München sechs Patienten mit einem onkocytären Carcinom und zwölf Patienten mit einem benignen Onkocytom operiert. Bei den histologisch gesicherten, benignen Onkocytomen trat im Beobachtungszeitraum, von durchschnittlich 11 Jahren, kein Lokalrezidiv und keine Metastase auf. Wir empfehlen daher die primäre Hemithyreoidektomie bei benignen Onkocytomen. Die Thyreoidektomie bleibt den onkocytären Carcinomen vorbehalten.

Summary There is still a dispute about the malignant potential of Hürthle cell tumors of the thyroid. In our clinic 6 patients with Hürthle cell carcinoma (HCC) and 12 patients with benign Hürthle cell adenoma (HCA) have been treated between 1965 and 1973. None of the patients with HCA showed a recurrence over a mean period of 11 years, although only subtotal resection had been carried out. We therefore recommend total lobectomy for HCA. Total thyroidectomy should only be carried out in cases involving malignancy.
