Zusammenfassung Die Übergangsfraktur der distalen Tibia stellt eine besondere Verletzungsform aus Epiphysenlösung und Epiphysenfraktur dar. Diese Sonderform tritt beim Abschluß des Skeletwachstums im Übergang vom Kindes- zum Erwachsenenalter auf. Scherkräfte treffen die Epiphysenfuge, deren Fugenschluß ungleichmäßig von dorsomedial nach ventro-lateral fortschreitet. Dabei entsteht eine Zwei- bzw. Dreiebenenfraktur mit räumlich schwierig angeordneten Frakturlinien. Die Charakteristika der Übergangsfraktur sind: Die Altersphase zwischen 12 1/2 und 14 Jahren, einwirkende Scherkräfte, der Reifegrad der Epiphysenfuge und die günstige Prognose.

Summary The transitional fracture of the distal tibia can be regarded as a combination of epiphysiolysis and epiphyseal fracture. This fracture mainly occurs between the ages of 12 and 14 years, when the epiphyses begin to fuse. A contributory factor is the vulnerability of this region to shearing stresses. The cause of this type of fracture is the unequal closure of the distal tibia epiphysis, which develops from the postero-medial side to the antero-lateral side. The lesion consists of lysis of the chondral part and fracture of the calcified part of the epiphysis.
