    Notizen: Summary Arabinosyl-5-azacytosine (AAC), a new nucleoside antimetabolite, is broadly active in preclinical tumor screening evaluations. To assess the potential for intrathecal use of this drug, we studied the toxicity and pharmacokinetics of intrathecal and intraventricular administration in nonhuman primates. Four adult male rhesus monkeys were given single 10 mg intrathecal (n=1) or intraventricular (n=3) doses of AAC to determine its acute toxicity and pharmacokinetic parameters. An additional 3 animals were given four weekly 10 mg intrathecal doses to assess the systemic and neurologic toxicity associated with chronic administration. Disappearance from the cerebrospinal fluid (CSF) was biexponential, and CSF clearance was 0.2 ml/min, which exceeds the rate of CSF bulk flow by 5-fold. The peak CSF concentration and area under the concentrationx time curve achieved with the intraventricular administration of 10 mg were one hundred, and fifty fold greater, respectively, than those achieved after an intravenous dose of 200 mg/kg (1500–2400 mg) in prior experiments. No clinically evident neurotoxicity was observed in either the single or the weekly × 4 dose groups. A slight, transient CSF pleocytosis and increased CSF protein was observed. Systemic toxicity was limited to one animal in the weekly × 4 dose group who demonstrated a mild and transient decrease in his peripheral leukocyte count unassociated with a change in his hematocrit or platelet count. These studies in nonhuman primates demonstrate a clear pharmacokinetic advantage for intrathecal vs systemic administration of AAC. This is demonstrated by a 50-fold greater CSF drug exposure with an intrathecal or intraventricular dose 1/200th of that which can be given systemically. Intrathecal AAC was found to be safe on a weekly dosing schedule. On the basis of these results, human trials evaluating intrathecal AAC are planned.

    Notizen: Summary Piroxantrone is an anthrapyrazole derivative with broad anti-tumor activityin vitro and less cardiac toxicity than the anthracyclines. The metabolic pathways and central nervous system penetration of piroxantrone have not been determined. In this study we examined the pharmacokinetic behavior of piroxantrone in plasma and cerebrospinal fluid in a non-human primate model. In addition, a urinary metabolite of piroxantrone was isolated and its cytotoxicity evaluatedin vitro. The disappearance of piroxantrone from plasma after an intravenous dose of 150 mg/m2 given over 60 minutes was biexponential with mean t1/2 alpha of 1.0 minutes and a mean t1/2 beta of 180 minutes. The mean area under the curve was 220 ΜM·min and the clearance was 1420 ml/min/m2. Piroxantrone was not detectable in the cerebrospinal fluid. Piroxantrone and three other compounds not present in pre-treatment samples were detected in urine. The major urinary metabolite was isolated. Its cytotoxicity against MOLT-4 cellsin vitro was at least one log less than that of piroxantrone. In addition, one of the other compounds detected in urine was determined to be a glucuronide conjugation product of the major metabolite. The results of this study may be useful in the interpretation of the activity and toxicity of piroxantrone in clinical trials.

    Notizen: Abstract Extracorporeal immunoadsorption is a new technique for removal of circulating radiolabeled antibody from the peripheral blood (1) to reduce background activity for improved tumor imaging, and (2) to reduce whole-body and marrow toxicity when high doses of radiolabeled antibodies are used for antitumor therapy. A pharmacokinetic model was developed to describe plasma disappearance of111In -KC-4G3 prior to, during, and after immunoadsorption in humans. The model is developed based on a two-compartment open model, and during immunoadsorption a third compartment is added for removed radioactivity by the immunoadsorption column. Goodness-of-fit statistics indicate a good fit of the model to the data. The resulting pharmacokinetic parameters for a selected patient are V1=2.64L, Vss=3.64 L, t1/2α=3.77hr, and t1/2β hr. The immunoadsorption clearance (CL1A=19.3 ml/min) was 21-fold greater than the patient's plasma clearance (CL10=0.899 ml/min), indicating a very effective immunoadsorption process. The model predicts an increase in plasma radioactivity upon termination of immunoadsorption, probably due to redistribution of radioactivity from the extravascular compartment to the plasma in response to the rapid decline in plasma radioactivity during immunoadsorption. Two series of simulations were performed to examine the influence of onset time and duration of immunoadsorption. In series one the onset time was varied and in series two immunoadsorption duration was varied. In series one, the predicted radioactivity amounts adsorbed by the immunoadsorption column ranged from 75% of the injected dose (4-hr onset) to 52% of the injected dose (24-hr onset). In series two, immunoadsorbed radioactivity ranged from 32% (2-hr duration) to 64% of the injected dose (12-hr duration). When instituted as early as 4 hr, the predictions suggest that earlier immmoadsorption onset improves the effectiveness of radioactivity removal, relating to higher early circulation concentrations, and longer immunoadsorption periods remove more radioactivity, but also result in larger predicted radioactivity redistribution from tissue to plasma. To employ the immunoadsorption procedure for tumor imaging and therapy optimally, the data and our predictions indicate that a compromise must be made that will balance immunoadsorption onset and duration against tumor radioactivity uptake and subsequent radioactivity redistribution from tissues back to plasma. Together with biologic considerations providing sufficient antigenantibody interaction and dependent on the utilized radioisotope, these data support the utility of extracorporeal immunoadsorption during the radioimmunodetection of cancer and for future therapeutic applications.

    Notizen: Fractional dietary Ca absorption, ‘a’, is measured by determining the ratio of two stable isotopic tracers, one of them orally (44Ca @ 0.2-0.5 mg/kg) and the other intravenously (42Ca @ 0.02-0.1 mg/kg). Thermal ionization mass spectrometry (TIMS) is used to measure the perturbation of natural abundance isotope ratios (delta % excess). Typical sensitivity of the TIMS permits detection of a 2.5 delta % excess change from the natural Ca isotope ratio with relative standard deviations of about 0.5%. At sufficiently long times absorption becomes constant so that ‘a’ is determined by a product of constants and a measured ratio.

    Notizen: Abstract Bacteria are an important cause of acute otitis media and successful treatment depends on achieving inhibitory or bacteriacidal antimicrobial drug concentrations in the middle ear. To evaluate further otitis media treatment success and failure, we developed a chinchilla model to study antimicrobial drug penetration through the middle ear mucosa. Using quantitative histomorphometry, we measured the middle ear space in 10 chinchillas and found a mean ±SD volume of 2.09 ± 0.08 ml and a mean SD surface area of 14.41 ± 1.48 cm2. To measure the apparent rate constant (K e) of antibiotic elimination from the middle ear, through the middle ear mucosa, an antibiotic solution was inoculated into the middle ear cavity, and samples were aspirated between 1 and 8 hr later. In normal ears, the mean K e ±SD for amoxicillin was 0.118 ± 0.013 hr−1, that for a trimethoprim 0.461 ± 0.090 hr−1, and that for sulfamethoxazole 0.265 ± 0.062 hr−1. In ears inoculated with type 7F Streptococcus pneumoniae to induce acute otitis media, the K e ±SD increased for all three drugs (P 〈 0.05): amoxicillin to 0.286 ± 0.089 hr−1, trimethoprim to 0.662 ± 0.118 hr−1, and sulfamethoxazole to 0.411 ± 0.056 hr−1. These values demonstrate that amoxicillin had the lowest apparent penetration rate constant of the three antibiotics but the greatest increase from normal to infected mucosa (142%). Trimethoprim had the highest apparent penetration rate constant of the three antibiotics but the smallest increase from normal to infected mucosa (44%), while the sulfamethoxazone apparent penetration rate constant increased from normal to infected mucosa by 55%. The K e for amoxicillin was the same for inoculation volumes of 0.8 and 1.6 ml (P = 0.557) and the same for sampling intervals of 4 and 8 hr (P = 0.054). All three antimicrobial drug concentration–time curves were log-linear, as predicted by Fick's first law of diffusion. In conclusion, this model overcomes the technical limitations of previous models and permits investigation of the many factors that can influence antibiotic penetration into the middle ear and reduce otitis media treatment efficacy.

    Notizen: A 1-D model, which neglects radial variations, describes the hydrodynamics of cell-free ultrafiltration hollow-fiber bioreactors (HFBRs) and the transport of highmolecular-weight proteins trapped in the extracapillary space (ECS). The profiles of radially-averaged protein concentrations predicted by this model are identical to those obtained using a model with radial variations. The model predictions agree well with axial profiles of bovine serum albumin (BSA) and human transferrin concentrations measured in transient and steady-state experiments. The validated model explores the influence of cell culture operating conditions on HFBR protein transport. Increasing protein loading decreases BSA and transferrin polarization in HFBRs operated with unidirectional lumen flow. A relationship developed predicts the protein loading needed to ensure a nonzero steady-state protein concentration throughout the ECS. This critical protein loading depends only on the lumen pressure drop and the ECS protein osmotic pressure. Periodic reversal of the lumen flow direction also decreases protein polarization. The influence of the flow-direction switching time and membrane permeability on the ECS protein distribution is investigated.

    Notizen: A mathematical model of mass transport through dispersed-phase networks to be used for the sustained release of drugs or other solutes at steady rates is presented. The drug is assumed to be encapsulated within the dispersed microdomains and transported to the bulk by diffusion across the interface. A drug is released to the surroundings by diffusion through the bulk. The results show that the desired steady flux of a drug to the surroundings may be obtained given appropriate values of structural properties of the network. These properties may be manipulated easily in the fabrication of dispersed-phase networks reported previously.

    Notizen: Turbulent flow friction factors were determined for flocculated suspensions of thoria, kaolin, and titania in tubes 1/8- to 1-in. diameter. The non-Newtonian laminar flow data were arbitrarily fitted with the Bingham plastic model. With this model the range of yield stress values was 0.018 to 1.39 lb.f/sq. ft., with a maximum ratio of coefficient of rigidity to viscosity of suspending medium of 11.1. The volume fraction solids were varied from 0.042 to 0.23.Two types of behavior were observed depending on the value of the yield stress. For yield values less than 0.5 lb.f/sq. ft. the turbulent friction factors were always less than those for Newtonian fluids but tended to approach the Newtonian values as the Reynolds number was increased. For yield values greater than 0.5 lb.f/sq. ft. the friction factors were again less than those for New-tonian fluids but tended to diverge from the Newtonian values as the Reynolds number was increased. Both sets of data were correlated with the Blasius relation with the coefficient and exponent given in terms of the laminar flow properties und the volume fraction solids.
