    Notizen: We report two cases of Schnitzler's syndrome in which anti-interleukin-1α autoantibodies and functional autoantibodies against the high affinity IgE receptor (FcεRIα) or against IgE were absent. One patient responded well to TL-01 phototherapy, a treatment which may be considered in patients with Schnitzler's syndrome if, as is usually the case, they are unresponsive to antihistamine therapy.

    Notizen: The major histocompatibility complex (MHC) acts as a marker for self during T-cell ontogeny and is associated with the pathogenesis of many autoimmune diseases. Recent investigations have shown about 30% of patients with chronic idiopathic urticaria (CIU) have IgG autoantibodies against the high-affinity IgE receptor, FcεRI, or IgE. A link between MHC class II alleles and CIU has not been reported previously. DNA was extracted from blood of 100 Caucasian patients with CIU, and the MHC class II type determined using the polymerase chain reaction with sequence-specific primers, testing for DRB and DQB1 alleles. The frequency of alleles in CIU patients was compared with that found in 603 controls. Further human leucocyte antigen (HLA) typing on patient subsets, classified by the patients' responses to intradermal injection of autologous serum and their serum-induced histamine release from basophil leucocytes of healthy donors, was undertaken. HLA DRB1*04 (DR4) and its associated allele, DQB1*0302 (DQ8), are raised in CIU patients compared with a control population (P = 2 × 10−5 and P = 2 × 10−4, respectively). HLA DRB1*15 (DR15) and its associated allele, DQB1*06 (DQ6), are significantly less frequently associated with CIU. The HLA DRB1*04 association is particularly strong (corrected P = 3.6 × 10−6) for patients whose serum has in vivo and in vitro histamine-releasing activity. HLA class II typing is consistent with the concept that CIU is a heterogeneous disease, and supports an autoimmune pathogenesis in a subset of patients.

    Notizen: SUMMARY. .Three bed-wetting patients are described in whom the use of enuresis blankets with electric buzzer alarms led to skin ulceration and scarring. In each case the cause was faulty design of the apparatus, resulting in failure of the alarm to ring despite flow of current due to bed-wetting. In two the buzzer alarm could be switched off although the circuit remained live. In the third a run-down battery failed to activate the alarm. Certain design modifications are suggested which would greatly reduce the risk of electrical injury with this apparatus.

    Notizen: SUMMARY.— Two cases of the Morquio syndrome, with associated mucopolysacchariduria, are described. Striking cutaneous abnormalities were found in each, the skin being loose, thickened, tough and inelastic particularly at the extremities. Generalized telangiectasia was present especially on the face and limbs. Similar changes have frequently been reported in the closely related Hurler syndrome. The presence of common features in the skin in these two conditions indicates that the cutaneous abnormalities of the Hurler syndrome are not unique.

    Notizen: : Aquagenic pruritus is a disease in which itchy prickling skin discomfort is evoked by contact with water at any temperature without observable cutaneous lesions. Little is known about its etiology and pathogenesis. Previous reports show that increased levels of blood histamine and cutaneous mast cell degranulation are present before water exposure and that they increase stilt further with water challenge. This paper shows that fibrinolytic activity is markedly increased both before and after water exposure, while circulating fibrinolytic activity is normal before water exposure in three cases of aquagenic pruritus. A patient who was asymptomatic at the time of the study had no observed increase in fibrinolytic activity either before or after water challenge, suggesting that the remission of symptoms of aquagenic pruritus and normalization of cutaneous fibrinolytic activity are interdependent factors.

    Notizen: SUMMARY.— The responses to topical application of tetrahydrofurfuryl nicotinate (Trafuril) cream were studied using a continuous skin perfusion method in clinically uninvolved skin of patients with chronic urticaria and in control patients. Trafuril causes inflammation but also renders the skin susceptible to dermographism.In non-urticarial subjects little pharmacological activity was detected during Trafuril inflammation or after stroking the skin to induce dermographism. This suggests that Trafuril causes inflammation by a direct effect on the cutaneous vasculature. By contrast in urticarial patients histamine was recovered more frequently than in non-urticarial patients during Trafuril inflammation, both before and after the skin was stroked. Perfusates from urticarial subjects also contained unidentified smooth muscle contracting activity which was not due to histamine, kinins, serotonin or acetyl choline. These results suggest that the skin of patients with chronic urticaria releases histamine and some other pharmacological agents more readily than that of normal subjects in response to inflammation producing stimuli.

    Notizen: Mast cells are considered to be the primary effector cells in urticaria but it is possible that lymphocytes contribute to the formation of weals by secreting histamine releasing factors. The aim of this study was to examine the population of mast cells and to quantify the T cell subsets and their activation status in delayed pressure urticaria (DPU), chronic idiopathic urticaria and normal controls. Three biopsies were obtained from each of four patients with chronic idiopathic urticaria but not DPU. Three biopsies were taken from each of 13 patients with DPU, from a combination of unchallenged skin and at 0, 2, 6, 24, 48 and 120 h after weighted steel rods (diameter 1.5 cm) had been applied to the thighs. Three biopsies were similarly obtained from each of four normal controls before an identical pressure challenge and at 6, 24 and 48 h afterwards. The chloracetate esterase stain was used to demonstrate mast cells and an alkaline phosphatase anti-alkaline phosphatase immunohistochemical technique to assess the phenotypic and activation characteristics of the T cell infiltrate. The mast cell count did not differ significantly between unchallenged skin from DPU patients and normal controls. Following a pressure challenge to the DPU patients, the number of stainable mast cells decreased significantly to a level comparable with that in spontaneous weals of chronic idiopathic urticaria. Investigation of T cell subsets showed a preponderance of CD4+ cells over CD8+ cells. There was no evidence of T cell activation in chronic idiopathic urticaria or DPU when compared with normal controls. These data support the view that mast cell degranulation occurs in chronic idiopathic urticaria and suggest that it may also play a role in the pathogenesis of DPU. There was no evidence that lymphocyte activation occurs in either condition.
