    Notizen: Abstract. Our previous study revealed that anaphylac tic shock can be produced in WBB6F1-W/Wv (abbreviated as W/Wv) mice. Because they are congenitally deficient in mast cells, we are certain that some other cell types are involved as mediator sources. In the present study, with the aim of examining the role of circulatory granulocytes, the effect of monoclonal an tibody to a mouse granulocyte antigen, Gr-1, upon active and passive anaphylactic shock was tested in W/Wv mice using several mast cell-bearing strains as references. An intravenous injection of 40 μg of anti-granulocyte antibody one day before the antigen chal lenge produced a marked decrease in neutrophils and nearly complete abolition of lethal shock in W/Wv mice regardless of the sensitizing method. Both pre vention of shock and reduction of neutrophils lasted for three days after the treatment with the antibody but not for six days. From these results, a reasonable conclusion would be that circulating Gr-1+ cells (predominantly composed of neutrophils) are the major mediator source. Reference experiments using mast cell-bearing strains revealed that the suppressive effect of anti-granulocyte antibody was also observed against active anaphylactic shock in C3H/HeN mice but not against active and passive anaphylactic shock in the other mice.

Background: T1- and T2-weighted magnetic resonance (MR) images frequently show fan-shaped areas of hypo- or hyperintensity in the hepatic parenchyma adjacent to a treated hepatocellular carcinoma after percutaneous ethanol injection (PEI) therapy. These areas correspond to abnormal contrast enhancement on serial dynamic MR images. The purpose of the present study was to describe the location, appearance, and frequency of these abnormalities because it is important to understand these entities for the correct assessment of therapeutic efficacy. Methods: MR imaging including a multisection dynamic study was performed in 20 consecutive patients with hepatocellular carcinoma treated with PEI therapy. We retrospectively evaluated the presence of fan-shaped hypointensities adjacent to treated tumors in the liver parenchyma on T1-weighted images and hyperintensities on T2-weighted images and corresponding fan-shaped contrast enhancement on both arterial-dominant and delayed-phase dynamic MR images. We review the location, appearance, and frequency of these findings, and we discuss the possible causes on the basis of pathologic examinations. Results: Seven (35%) of the 20 patients showed fan-shaped hyperintense areas adjacent to the treated tumors on T2-weighted images. These areas showed isointensity in five patients and hypointensity in two patients on T1-weighted images. Of these seven patients, one (14%) underwent the MR imaging within 1 month after the completion of PEI therapy, and six (86%) had it 2–9 months after the completion of PEI therapy (mean = 6 months). In all seven patients, fan-shaped hyperperfusion abnormalities corresponding to these areas of hyperintensity on T2-weighted images were seen on both arterial-dominant and delayed-phase dynamic MR images. Pathologically, the coagulative necrosis of the hepatocytes with sinusoidal dilatation and the restoration by the development of fibrous tissue were seen in these fan-shaped areas. Conclusion: The fan-shaped areas of abnormal intensity on T1- and T2-weighted images and contrast enhancement on dynamic MR images seem to be attributable to pathologic changes in the normal liver parenchyma induced by the toxic reaction of ethanol. Awareness of the occurrence of such abnormalities in the peripheral liver parenchyma adjacent to the treated tumor is important for the correct assessment of therapeutic efficacy. Background: The purpose of the present study was to describe the computed tomography (CT) appearances of transient hepatic attenuation differences (THADs) in patients with acute pancreatitis and to discuss the mechanism of THAD. Methods: Two-phase dynamic CT images of 28 patients with acute pancreatitis were reviewed. Among them, THAD was seen in nine patients. All patients underwent ultrasonography, and four patients with THAD underwent surgery. Results: Three types of THAD (THAD adjacent to the gallbladder in five of 28 patients, THAD with left lobar distribution in three of 28, wedge-shaped THAD in one of 28) were seen on the two-phase dynamic CT scans of patients with acute pancreatitis. In five patients, THAD disappeared when acute pancreatitis had subsided. Conclusions: THAD in acute pancreatitis is probably caused by increased arterial blood flow attributable to the inflamed lobe of the liver or the inflamed gallbladder. THAD in acute pancreatitis should not be confused with primary liver abnormalities.

    Notizen: Abstract. We present our clinical experience with dynamic contrast-enhanced MR angiography (MRA) with subtraction for assessing intracranial vascular abnormalities. Ten patients with various cerebrovascular disorders underwent dynamic contrast-enhanced MRA on a 1.0-T system. Thirty sections (2 mm) were acquired in 29–30 s. Maximum intensity projection images and subtracted source images were compared with those obtained by conventional angiography. In all cases, the presence or absence of abnormalities in the targeted vessels, as well as the morphology of the sagittal sinuses, was clearly visualized as in conventional angiography, without any obstructions such as hyperintense hematomas or thrombi, or intraluminal turbulence. Although the temporal and spatial resolutions with current hardware are insufficient, these preliminary results suggest that dynamic contrast-enhanced MRA with subtraction may be useful for assessing vascular lesions with hemorrhage or thrombus, and the dural sinuses.

    Notizen: Summary Degeneration of afferent and efferent fibres supplying leg muscles was elicited by applying dry ice to the sciatic nerve in the mid femoral region. Two months after the denervation the cats showed practically normal walking activity. After 3 months the twitch tension of the reinnervated muscle (triceps surae) was almost similar to the tension developed by the non-operated contralateral control muscle. Afferent discharges with a pattern typical for muscle spindles could rarely be found before the end of the 4th month. The muscle spindles which could be recorded before the end of the 6th month showed mainly phasic responses to ramp-and-hold stretches and only a few of them exhibited a pause during muscle contraction. None of them showed any sign of gamma innervation, as tested by several independent gamma-activating or gamma-blocking procedures. The conduction velocities of the regenerated afferent and efferent fibres were lower than those of the normal fibres; they gradually recovered with time after the operation but did not reach normal values by the end of the 6th month. Fibre histograms revealed that the afferent fibres of groups I, II and III had regenerated, while of the efferent fibres only those corresponding to the alpha group had regenerated. Since during the period of observation the reinnervated muscles lack the support of a functioning gamma innervation, it is proposed to call them “alpha muscles”.

    Notizen: Summary Various doses of tetanus toxin were injected into three hind leg and two fore leg muscles of the rat. The neuromuscular transmission was tested by recording the mass action potential of the muscles elicited by a single electrical stimulus to the motor nerve after strong symptoms of local tetanus had developed. The muscle responses were depressed and blocked at lower toxin doses in the fast tibialis anterior than in the mixed gastrocnemius latemlis, while blocking of the slow soleus required the highest dose. The extensor carpi radialis and the flexor carpi ulnaris muscles showed medium sensitivity. In all five muscles the contraction time was measured and correlated with its individual minimal blocking dose. The more phasic (i.e., the faster) the muscle, the more sensitive its neuromuscular transmission was to tetanus toxin. The proportional distribution of red, white, and intermediate fibres, which are associated with specific end-plate types, was evaluated for the five muscles. The percentage of white fibres in the muscles displayed a very good negative correlation with the blocking dose. The relation between structures of end-plates and effects of tetanus toxin were analysed and it is suggested that the differences in sensitivity to tetanus toxin in the neuromuscular transmission in the five muscles is determined by a differential distribution of endplates with varying sensitivities to this toxin due to structural properties.

    Notizen: Summary In anaesthetized cats the activities of Renshaw cells (RCs) and Ia-inhibitory interneurones (IaINs) were recorded during the accumulation of tetanus toxin in the spinal cord following injection into the gastrocnemius muscle. The early response of the RCs increased during the period of development of local tetanus. With some cells there was a subsequent decrease in the early response in later periods of the observation time (16–44 hrs after intramuscular injection). The effects on the spontaneous activity of the RCs were in good correspondence to those on the early response. The hyperactivity of the RCs is proposed to be mediated mainly via disinhibited cholinergic gamma-motoneurones using muscarinic postsynaptic receptors. The “pause” which follows the early response and the recurrent inhibition of IaINs was not reduced during the development of local tetanus. These results indicate that the central action of tetanus toxin in local tetanus does not consist of a general loss of postsynaptic inhibition. It is suggested that tetanus toxin acts mainly on synaptic elements of the alpha- and gamma-motoneurones or on presynaptic nerve terminals in their vicinity. In later periods a disturbing influence on the cholinergic transmission at Renshaw cells seems to occur.
