    Notizen: Summary A facial flush provoked by alcohol in chlorpropamide treated diabetics has been described as a genetic marker for a dominantly inherited type of non-insulin dependent diabetes. In this study a chlorpropamide alcohol flush was observed in 16.9% of control subjects (n=154), 23.3% of insulin dependent diabetics (n=437) and 16.5% of patients with non-insulin dependent diabetes (n= 145). Among the non-insulin dependent diabetics no difference in the frequency of the chlorpropamide alcohol flush was found between those with and without a family history of diabetes. Specificity was not improved by skin temperature measurement or additional placebo tests. According to these data the chlorpropamide alcohol flush does not seem to be specific for non-insulin dependent diabetes and hypotheses about the aetiology of this type of diabetes based on the chlorpropamide alcohol flush should be regarded with caution.

At most three parameters can be calculated, the other three have to be known. Refraction is taken into account exactly. The calculations have to be made with the aid of a computer. The unknown parameters can be determined approximatively by simple formulas.
    Notizen: Zusammenfassung Ein Verfahren zur Bestimmung der im Sinne der Methode der kleinsten Quadrate bestangepassten Schichtparameter an eine gemessene Laufzeitkurve wird angegeben. Es können höchstens drei Parameter berechnet werden, die drei übrigen Parameter müssen bekannt sein. Die Brechung wird exakt berücksichtigt. Das Verfahren ist für Berechnung durch Computer geeignet. Einfache Formeln gestatten eine angenäherte Bestimmung der unbekannten Parameter.

    Beschreibung / Inhaltsverzeichnis: Summary The present study was undertaken to obtain more precise information about the purine biosynthetic pathway in human blood cells. 5′-phosphoribosyl-1-pyrophosphate (PP-ribose-P) amidotransferase was found in cell-free extracts from all leukemic cells and normal lymphocytes and therefore these cells could synthesize the first intermediate of the purine-de-novo-synthesis. Normal leucocytes, erythrocytes and bone marrow cells lack this enzyme system and have an absolute requirement for externally supplied purines via salvage pathway. Leukemic blast cells show different enzyme activities independent of their cell count. Kinetic studies with the crude enzymes showed sigmoidal substrate velocity curves for PP-ribose-P, whereas glutamine shows hyperbolic kinetics. The leukemic cell enzymes from all four donor types (ALL, CLL, AML and CML) are rapidly saturated with low concentrations of PP-ribose-P and less inhibited by the physiological feedback inhibitor, adenosine 5′monophosphate. The crude enzymes of normal spleen lymphocytes and leukemic cells were further purified (10 to 15-fold) and substrate velocity curves for PP-ribose-P and glutamine show now hyperbolic kinetics and double reciprocal plots were linear with and apparent Km for PP-ribose-P of 0.14 mM and for glutamine 2.0 mM. In the presence of different concentrations of AMP, the PP-ribose-P substrate velocity plot changed from a hyperbolic to a sigmoidal curve; no difference in the degree of the inhibition between both partially purified enzymes (normal spleen lymphocytes and leukemic cells from all four donor types) could now be observed.
    Notizen: Zusammenfassung Die erste irreversible und geschwindigkeitsbestimmende Reaktion in der Purinde-novo-Synthese ist die Bildung von β-Phosphoribosylamine aus Glutamin und Phosphoribosylpyrophosphat (PRPP), katalysiert durch das Enzym PRPP-Glutamin-Amidotransferase (EC 2.4.2.14). In der vorliegenden Arbeit wird nachgewiesen, daß normale Granulozyten, Erythrozyten und Knochenmarkszellen dieses Enzym nicht besitzen und deshalb auf die externe Zufuhr von Purinbasen, bzw. auf den Reutilisierungsstoffwechsel (“salvage-pathway”) angewiesen sind. Im Vergleich zu normalen Lymphozyten aus Milz und peripheren Blut besitzen die leukämischen Zellen eine deutlich gesteigerte Enzymaktivität und damit eine erhöhte Purin-de-novo-Synthese. Bei der Untersuchung der kinetischen Eigenschaften zeigt die PRPP-Amidotransferase im Rohhomogenat für PRPP eine sigmoidale Substratsättigungskurve während es für Glutamin eine hyperbole Kinetik aufweist. Im Vergleich zum normalen Lymphozytenenzym — ist die PRPP-Amidotransferase in allen leukämischen Zelltypen bei wesentlichen niedrigeren PRPP-Konzentrationen gesättigt — und wird bei physiologischen AMP-Konzentrationen in geringerem Ausmaß gehemmt. Das partiell gereinigte Enzym (ca. 10 – 15fach) — sowohl von normalen Milzlymphozyten — als auch von allen leukämischen Zellen zeigt für beide Substrate nun eine hyperbole Substratsättigungskurve. Bei der doppelt reziproken Auftragung nach Lineweaver-Burk betragen die Km-Werte für PRPP 0,14 mM und für Glutamin 2,0 mM. Es wurden nun sowohl für das partiell gereinigte Enzym aus leukämischen Zellen als auch für das Lymphozytenenzym gleiche Km-Werte gefunden. In Anwesenheit verschiedener AMP-Konzentrationen weist die Substratsättigungskurve für PRPP wieder eine sigmoidale Kinetik auf. Das partiell gereinigte Lymphozytenenzym wird nun durch AMP im gleichen Ausmaß gehemmt, wie das Enzym aus leukämischen Zellen. Nach den hier vorgelegten Befunden ist die Steigerung der Aktivität des Schlüsselenzyms der Purinsynthese (PRPP-Amidotransferase) ein metabolischer Parameter der maligne transformierten Leukämiezellen. Zugleich wird verdeutlicht, daß hier ein Punkt besonderer Angreifbarkeit durch die Chemotherapie liegt.

    Notizen: Summary Three cases with renal angiomyolipoma in Bourneville's disease are reported. In two patients advanced renal insufficiency developed, one being treated by chronic intermittent hemodialysis. In the third patient rupture of angiomyolipoma with severe retroperitoneal bleeding occurred. In one patient, tumorous enlargement initially was diagnosed only in the right kidney and preceded angiomyolipoma of the left kidney for several years. In this patient nephrectomy was performed because malignancy was assumed before diagnosis of Bourneville's disease was established. Occasionally, histology may suggest sarcomatous changes. However, the absence of distant metastasis and the overall long survival of patients with renal angiomyolipoma in Bourneville's disease underline the benign character of the tumor. Treatment should be conservative because surgical intervention with loss of kidney parenchyma may enhance progression to end-stage renal failure.

    Notizen: Zusammenfassung Das Aktivitätsmaximum der alkalischen Phosphatase im Cyclus um die Zeit der Ovulation ist ein Progesteroneffekt. Er tritt vor den uns bisher bekannten Sekretionszeichen in der Schleimhaut auf. Die Ausbildung der Spiralarterien, denen wir im Hinblick auf die hämochoriale Placentation des Menschen eine entscheidende Bedeutung beimessen, läßt sich bei der mit Oestradiol vorbehandelten Kastratin durch 200 mg Progesteron intramuskulär, verteilt auf 10 Injektionen in 10 Tagen, erzielen. Man kann bei der Kastratin mit der gleichen Progesteronmenge die Ausscheidungswerte für Pregnandiol und die biphasische Temperaturkurve des normalen Cyclus gerade erreichen.

    Notizen: Summary A prospective, randomized, doubleblind, placebo-controlled trial was conducted to evaluate the efficacy of Acetylsalicylic Acid (ASS) (100 mg/d, starting 24 h after operation) on vein graft patency. Sixty of 88 patients having undergone surgery entered the study; in 24 of 31 patients in the placebo group and 22 of 29 patients in the ASS-group angiography was performed 4 months postoperatively. There were no significant differences between the groups with respect to age, number of diseased vessels or previous myocardial infarctions. Mean number of grafts per patient was 2,2 (placebo) and 1,8 (ASS) for proximal anastomoses (p〈0.10) and 3.4 (placebo) and 2.6 (ASS) for distal anastomoses (p〈0.05). Graft occlusion rate for proximal anastomoses was less in the ASS-group, 10% (4/40), as compared with placebo 32% (17/53) (p〈0.05). Graft occlusion rate for distal anastomoses was also less in the ASS group, 19% (11/57) as compared to 35% (28/81) in the placebo group (p〈0.10). All grafts were patent in 16/22 patients in the ASS group but only in 9/24 in the placebo group (p〈0.05). On designation of patients without postoperative angiograms but cardiovascular events as well as those with at least one graft occluded as “failures”, the incidence of the latter was 9/29 in the ASS group and 20/31 in the placebo group (p〈0.05). Early postoperative bleeding was similar in both groups, no side effects of ASS were observed. In this trial with initiation of low — dose ASS therapy 24 h after operation, antiplatlet therapy reduced the graft occlusion rate significantly.

    Notizen: Summary Alpha-1-microglobulin (alpha-1-m) is a low molecular weight glycoprotein (mw 25–33 KD) that is filtered through the glomeruli and reabsorbed in the proximal parts of the renal tubules where it is catabolized. Normal ranges were established for alpha-1-m (100 healthy controls) in serum (20–42 mg/l) and urine (3.5–8 mg/l). Alpha-1-m was then measured in 341 urine samples whose protein pattern had been classified as “pathologic” and “normal” according to microelectrophoresis. Increased alpha-1-m concentrations were found in 266 out of 280 pathologic urines (5% false negative) and in 3 out of 61 normal urines (4% false positive). Beta-2-microglobulin (beta-2-m), total protein or protein test strips showed a poorer correlation to the electrophoretic results. Measurement of alpha-1-m is, therefore, the most sensitive of these methods for the detection of proteinuria. In 90 patients with low molecular weight proteinuria and either with or without renal insufficiency alpha-1-m concentrations were determined in both urine and serum. While all patients had elevated urinary alpha-1-m concentrations, increased serum values were only found in renal insufficiency (Ccrea〈100 ml/min). Independently of these results, we were also able to establish that increased alpha-1-m levels are found at decreased glomerular filtration rates (Ccrea 〈70 ml/min). Pathologic alpha-1-m concentrations therefore only allow the conclusion of isolated tubular impairment when the GFR is greater than 70 ml/min. Data from 350 patients with various renal and hypertensive diseases showed that serum alpha-1-m is a more sensitive indicator of renal insufficiency, even in the so-called “creatinine blind” range (60–100 ml/min) of the GFR than either creatinine or beta-2-m.

    Notizen: Abstract Objectives: (a) To investigate the relationship between gut ischemia parameters (gastric intramucosal pH [pHi], mucosal–arterial carbon dioxide difference [PCO2-gap]), and endotoxin or cytokine release during hemorrhagic shock; (b) to compare the predictive value of pHi, PCO2-gap and arterial lactate concentrations. Design: Prospective study. Setting: Surgical intensive care unit of a university hospital. Patients: 20 multiple trauma patients with severe hemorrhagic shock. Interventions: Intramucosal measurements and blood samples were obtained on admission to the emergency room and repeatedly over 48 h. Measurements and results: Endotoxin was measured using a chromogenic limulus amoebocyte assay. Cytokine [tumor necrosis factor-α (TNFα) and interleukin-6 (IL-6)] values were evaluated by immunoradiometric assays. Only 3 patients had positive blood cultures but endotoxins were detected at least once in all patients. Endotoxin levels were similar in survivors and nonsurvivors over the study period and were not related to pHi or PCO2-gap. Initially, high levels of IL-6 were observed in both nonsurvivors and survivors [median 1778 pg/ml (range 435–44 540) vs 2068 pg/ml (range 996–92 300)]. IL-6 levels progressively decreased in the survivors but not significantly. On admission, TNFα concentrations were similar in nonsurvivors and survivors (42 ± 35 vs 46 ± 27 pg/ml). From the 24th h, TNFα values were higher in the nonsurvivors than in the survivors (24 h: 72 ± 38 vs 34 ± 17 pg/ml, p 〈 0.05). The greatest IL-6 levels were found for a pHi 〈 7.20 (28.5 ± 36.5 vs 1.8 ± 1.3 ng/ml, p 〈 0.05) or a PCO2-gap 〉 7.5 mmHg (1 kPa) (32.5 ± 37.5 vs 1.7 ± 1.3 ng/ml, p 〈 0.01). With the same pHi threshold, no difference was found in endotoxin levels. The lactate concentrations were predictive for outcome from the 12th h (9.5 ± 5.9 vs 3.6 ± 2.3 mmol/l, p 〈 0.05). Conclusions: During severe hemorrhagic shock, endotoxin translocation from the gut was a common phenomenon that seemed independent of both pHi values and outcome. It could not explain IL-6 and TNFα release. In severe hemorrhagic shock, neither pHi nor PCO2-gap provides additional information to the lactate measurements.
