Notizen: A review is presented of the effects of loss of teeth, with impaired occlusal stabilization, on the temporomandibular joint in experimental animals. Loss of teeth, with subsequent abnormal loading of the temporomandibular joint, leads to histomorphological, pathological and pathophysiological changes in the articular cartilages, the articular disc, the synovium, and the bony articular components. The severity of the changes increases with increasing age, and an existing arthritis of the joint is aggravated by impaired occlusal stabilization. The experimentally induced changes resemble those in human subjects with extensive loss or extensive attrition of teeth.

Notizen: An overview is presented of the physiology and pathophysiology of dynamic skeletal muscle contractions in the intact living organism. Dynamic muscle activities are divided into concentric contractions with shortening of muscle fibres and the production of positive work, and eccentric contractions with lengthening of muscle fibres and the production of negative work. In positive work, muscle tension overcomes external forces. In negative work, external forces overcome muscle tension. The latter phenomenon, with relatively few active motor units, explains the injuries induced by eccentric contractions. Both the contractile and non-contractile elements are involved in the muscle injuries and, clinically, they are referred to as myofibrositis.

Notizen: An overview is presented of the physiology and so-called pathophysiology of static muscle contractions in the intact living organism. Static muscle activity produces no external mechanophysical work, but is used for fixation. Contingent upon the levels of the generated forces, and the duration of the isometric contractions, static activities give rise to fatigue and pains. The discomforts are viewed as somatic responses that are normally within physiological limits. That is, upon cessation of isometric contractions there is usually rapid recovery from the discomforts, and they do not normally induce chronic lesions.

Notizen: To study concurrence of specific electromyographic chewing patterns and the side of mastication, twelve subjects chewed gum for 15, 20 and 25 s. Gum bolus placement on the right or left side of the dentition was estimated by three series of visual inspections. Contraction patterns of the chewing and non-chewing side masseter muscles were appraised by integrated and cumulative surface electromy-ography. Consistent and predominant gum bolus placement on the right side of the dentition could be predicted fairly reliably from comparative electromyographic observations. This was also the case in instances of consistent left-sided gum bolus placement, but not in predominant left-sided bolus placement. The latter phenomenon was explained by deviant observations made in two subjects only. Agreement between a presumed chewing side and a predictive contractility pattern was established in 78% of observations (P〉0-001).

Notizen: Fifty dentate maxillary casts, obtained from thirty-four males and sixteen females, were mounted in the three-dimensional co-ordinate system of a contour meter. A transverse line of reference (x-axis) was drawn through the distal contact points of the maxillary canines, at a right angle to the y-axis which passed through the contact points of the maxillary central incisors. Relative to the canine-to-canine baseline, the locations of the lateral borders of the right and left first primary rugae were determined. The rugae were distributed on both the anterior and posterior sides of the baseline, and the anterior and posterior distances from the baseline were on the average about 1 mm. The age of the subjects, ranging from 12 to 52 years, appeared to be unrelated to the frequencies of anteriorly and posteriorly positioned rugae. Right and left first primary rugae, located on the anterior side of the baseline, showed a minute asymmetry in their topography. By contrast, there was topographical symmetry when the rugae were located on the posterior side of the baseline.

Notizen: Ten subjects performed deliberate right and left-sided chewing of gum, each exercise with a duration of 60 s, followed by maximum voluntary teeth clenching (MVC) until onset of pains in the masseter muscles (pain threshold). Contractile activities of the right and left masseter muscles were assessed by integrated and cumulative surface electromyography (EMG). When the right and left muscles functioned as chewing side muscles only, they generated practically identical levels of chewing force; the same applied when they functioned as non-chewing side muscles only. During identical functions, EMGs suggested identical motor unit recruitment and firing patterns in the two paired muscles; i.e., units producing relatively high tensions for chewing side activity, and other units producing relatively low tensions for non-chewing side activity. During isometric MVC activity, until onset of pains, the right and left muscles generated practically identical levels of bite force, but right and left motor unit activity patterns seemed to differ. Right and left motor units might have fired at different rates or begun to drop out at different times. In the latter case, rapidly before slowly fatigued units. There was a tendency for a low pain threshold with high chewing side activity in both masseter muscles. Motor units producing high levels of tension, but easily fatigued, might have caused early onset of muscle pains. No relationships could be established between preferred chewing side and side of initial muscle pains, nor between MVC activity and side of pain onset and preferred chewing side.

Notizen: Three human subjects performed teeth clenching at maximum voluntary contraction strength (MVC) of the mandibular elevator muscles. At intervals of 10 s, ten successive bouts of MVC clenching were exercised until total exhaustion of the contracting muscles (isometric endurance). Isometric endurance time decreased exponentially from 59 to 12 s. The curve of the decrease showed a monotone power function (y=x−1). Electromyographic recordings from masseter muscles suggested that fatigable motor units, possibly fast glycolytic units, dropped out during the initial stages of maintained MVC isometric activity. The silent period of the monosynaptic jaw jerk reflex was increased by about 35% following exhaustion of the masseter muscle.

Notizen: An overview is presented of the mandibular reference positions of centric occlusion, centric relation and centric relation occlusion, and the position of rest. Clinical applications of the different positions are discussed on the basis of the physiology of the mandibular locomotor system.

Notizen: The pH of the gut lumen was measured in 39 healthy persons using a pH-sensitive, radiotransmitting capsule. Thirteen persons were studied twice. The location of the capsule was determined by X-ray. The pH rose from 6.4 in the duodenum to 7.3 in the distal part of the small intestine. In 17 persons the pH dropped by 0.1–0.8 pH units during the last hours of the small intestinal transit. The pH was 5.7 in the caecum, but rose to 6.6 in the rectum. Gastric residence time was 1.1 h, small intestinal transit was 8 h, and colonic transit time was 17.5 h (median values). The results provide a firmer basis for prediction of the level, and the rate of release of active substance from pH-dependent sustained-release oral preparations.
