    Notizen: A high-resolution x-ray polarimeter has been installed at the torus experiment for technology oriented research (TEXTOR-94). The instrument consists of two spectrometers in Johann geometry, utilizing the dependence of Bragg reflection on the polarization. The dispersion plane of the so-called horizontal spectrometer coincides with the midplane of the tokamak, whereas the dispersion plane of the so-called vertical spectrometer lies perpendicular to the midplane of the tokamak. Both spectrometers operate with cylindrically bent quartz crystals of dimensions 153 mm×38 mm×0.7 mm with a 2d spacing of 4.9130 Å. The radii of curvature of the horizontal and vertical spectrometer crystals are 3820 mm and 4630 mm, respectively. The detectors are multiwire proportional counters having a large entrance window of 180 mm×90 mm, with a high count rate capability of up to 2.5×105 photons/s, and a spatial resolution of 0.4–0.65 mm, depending on the count rate. The unique data acquisition system makes it possible to record up to 8192 spectra per TEXTOR-94 discharge to study fast events and impurity transport. The instruments are sensitive to spurious amounts of argon in hydrogen or deuterium discharges and aim for x-ray polarization measurements and other core plasma diagnostics like electron temperature, ion temperature, plasma rotation, impurity transport, and relative abundance of Ar15+/Ar16+. The first measurements of toroidal rotation speed, ion and electron temperatures show a very good agreement with charge exchange recombination spectroscopy (XRS) and electron cyclotron emission (ECE) diagnostics available at TEXTOR-94. The measured values of the ratio Ar15+/Ar16+ are higher than that predicted from coronal equilibrium. The ratio depends strongly upon the electron density both for ohmic and neutral beam injection cases. This suggests that the charge state distribution is also dependent on diffusion and charge exchange processes in the plasmas. © 2001 American Institute of Physics.

    Notizen: Background : Peristomal infection can sometimes complicate percutaneous endoscopic gastrostomy (PEG) placement. Antibiotic prophylaxis has, in some studies, been shown to reduce the incidence. However, the use of prophylaxis varies widely, possibly because the design and findings of the studies have differed, making their relevance to clinical practice difficult to interpret.Aim : To determine the efficacy of antibiotics, either prophylaxis or concurrent antibiotics at the time of the procedure, in reducing peristomal infection after PEG insertion in the context of a study designed to reflect current practice.Methods : One hundred and forty-one patients undergoing PEG placement were randomised to group one to receive either a single dose of 750 mg of intravenous cefuroxime (n = 50) or placebo (n = 51) 30 min before PEG insertion. Forty patients who, for various reasons, were already receiving antibiotics were allocated to group two. The peristomal site was evaluated on day 3, 5 and 7 following insertion. Erythema and exudate were scored on a scale from 0 to 4; induration was scored on a scale of 0–3. A maximum combined score of 8 or higher or the presence of pus was criteria for infection. The primary outcome measure was the occurrence of a peristomal wound infection at any time within one week of PEG insertion.Results : Peristomal wound infection was significantly reduced in patients who received antibiotics either as a single dose of cefuroxime [one of 33 (3%)], or in those on antibiotics for prior indications [one of 36 (3%)], compared with placebo [six of 33 (18%)], P = 0.04 and 0.03, respectively.Conclusion : Antibiotics, either prophylaxis or concurrent, reduce the incidence of peristomal wound infection after PEG placement.

    Notizen: Ten patients with dry non-inflammatory tinea capitis were evaluated in a pilot study which ran from September 1989 to February 1990. Each patient was given oral terbinafine for 6 weeks; each was followed up 2 weeks later. Eight (80%) were completely cured, one (10%) was mycologically cured and showed minimal signs and symptoms, and another (10%) showed improvement (negative mycology, but persistent clinical signs and symptoms). No topical or systemic side-effects were noted. Terbinafine appears to be an effective and safe antifungal agent in the treatment of non-inflammatory tinea capitis.
