    Notizen: Probabilistic knowledge is an important input to the analysis of many decisions, and may be required for long-range forecasting, classical decision analysis, influence diagrams, fault tree analysis, and expert systems. If objective probabilities cannot be calculated, it is vital that decision makers use the best available subjective measures of probability. Standard mathematical theory allows the analyst several choices in framing subjective probability assessments. When assessing cause-effect probabilities, the three choices are causal, diagnostic, and joint probabilities. Choosing among these is difficult in light of conflicting reports that, due to cognitive heuristics, probability judgment may be biased in various poorly understood ways. For example, it has been reported that, when judging causal probabilities, people are subject to the causal information bias, and thus upwardly revise their prior probabilities more than when judging diagnostic probabilities. On the other hand, it has also been reported that in some cases, people do not take proper account of new evidence, which results in under-revision of prior probabilities. Furthermore it has been reported that, when assessing joint probabilities, people are subject to the conjunction fallacy, and thus often judge a joint probability to be higher than one of the two corresponding marginal probabilities. Our research compares the relative effects of these biases in a laboratory setting: we present new empirical results comparing the test-retest reliabilities and under(over) revision rates of causal, diagnostic, and joint probabilities. Our results suggest that the tendency to both under-revise and over-revise prior probabilities is greatest when judging diagnostic probabilities, as opposed to either causal or joint probabilities. Furthermore our results suggest that joint probability judgment is more reliable than either causal or diagnostic probability judgment.

    Notizen: The purpose of this audit was to study reasons for starting antibiotic therapy, duration of antibiotic treatment, reasons for changing antibiotics and the agreement between clinical suspicion and microbiological results in intensive care practice. We conducted a multicentre observational audit of 316 patients. Data on demographic details, site, treatment and nature of infection were collected. The median duration of antibiotic therapy was 7 days. Infections were community-acquired in 160 patients (55%). Antibiotics were started on clinical suspicion of infection in 237 patients (75%). Pulmonary infections were the most common, representing 52% of all proven infections. Gram-negative organisms were the most common cause of proven infections (n = 90 (50%)). The antibiotic spectrum was narrowed in light of microbiology results in 78 patients (43%) and changed due to antibiotic resistance in 38 patients (21%). We conclude that the mean duration of treatment contrasts with existing published guidelines, highlighting the need for further studies on duration and efficacy of treatment in intensive care.

    Notizen: Reports and guidelines concerning intensive care practice have been issued recently. However, the introduction of such centrally issued recommendations may be difficult because of marked heterogeneity between intensive care units. This study examined the facilities (number of beds, consultant sessions, nursing establishment), annual workload (number and types of patients admitted) and outcome (intensive care unit mortality) in the (old) Anglia Region. There were significant differences in the distribution of patients' ages, severities of illness, diagnoses, durations of admission and outcomes. Such heterogeneity may make multicentre trials more difficult to conduct and create problems when uniform measures designed to improve intensive care services are being planned.

    Notizen: Summary Outcome measurement in critical care is difficult because of the wide variety of patients treated and the diverse therapeutic options and pathways available. Individual outcome measures for critical care are available but are naturally limited to only a single aspect of performance. Most importantly, better performance in one aspect of care may compromise the standard of care in another. A global measure of performance would be helpful. For the year 1999–2000, the five hospitals in the East Anglian Critical Care Network provided data on capacity, workload and performance. The data was transformed and displayed graphically on a radar chart so that the area of the polygon within the radar chart was proportional to each unit's overall performance. The results from the five hospitals suggest that there is little overall difference in the units' global performance but the graphical representation highlighted some individual deficiencies. Graphical analysis of complex processes such as critical care delivery may facilitate performance assessment, providing that the measures chosen, weightings assigned and scales used are standardised with care.

    Notizen: Objective  To determine the effect of ONO-8815Ly on uterine contractions.Design  A randomised, double-blind, placebo-controlled, dose-ascending, cross-over study.Setting  Department of Obstetrics and Gynaecology, University Hospital of Lund, Sweden.Population  Seventeen, healthy, parous and permanently sterilised women.Methods  Intrauterine pressure was recorded on days 1–3 of bleeding of two menstruations. Subjects were intravenously treated with 4 or 8 μg/minute of ONO-8815Ly or placebo for 130 minutes. Intravenous bolus injections of oxytocin, 50 pmol/kg body weight, were given 10 minutes before, during infusion after 60 and 120 minutes and 60 minutes after completion of infusion. The plasma concentrations of ONO-8815Ly were measured in samples obtained immediately before each oxytocin injection.Main outcome measure  Area under pressure recording curve (AUC) 10 minutes before and after each oxytocin injection.Results  Twelve women, six in each dose group, completed both recordings. Of these, two women of each group were not included in efficacy analysis due to non-responsiveness to oxytocin or missing baseline value. The AUC over 10 minutes before oxytocin injection after 60 minutes of infusion of ONO-8815Ly at 4 and 8 μg/minute was reduced to 21% and 37% of that before infusion, respectively. The AUC after oxytocin at that time amounted to 21% and 19%, respectively, of that before infusion. The activity and responsiveness remained low after 120 minutes but started to return to baseline 60 minutes after stopping infusion. Placebo had no effect.Conclusions  ONO-8815Ly is a potent inhibitor of spontaneous uterine contractility in non-pregnant women and it reduces the uterine response to oxytocin injections.

    Notizen: Trans-plasma membrane electron transport is critical for maintaining cellular redox balance and viability, yet few, if any, investigations have studied it in intact primary neurons. In this investigation, extracellular reduction of 2,6-dichloroindophenol (DCIP) and ferricyanide (FeCN) were measured as indicators of trans-plasma membrane electron transport by chick forebrain neurons. Neurons readily reduced DCIP, but not FeCN unless CoQ1, an exogenous ubiquinone analog, was added to the assays. CoQ1 stimulated FeCN reduction in a dose-dependent manner but had no effect on DCIP reduction. Reduction of both substrates was totally inhibited by ε-maleimidocaproic acid (MCA), a membrane-impermeant thiol reagent, and slightly inhibited by superoxide dismutase. Diphenylene iodonium, a flavoenzyme inhibitor, completely inhibited FeCN reduction but had no affect on DCIP reduction, suggesting that these substrates are reduced by distinct redox pathways. The relationship between plasma membrane electron transport and neuronal viability was tested using the inhibitors MCA and capsaicin. MCA caused a dose-dependent decline in neuronal viability that closely paralleled its inhibition of both reductase activities. Similarly capsaicin, a NADH oxidase inhibitor, induced a rapid decline in neuronal viability. These results suggest that trans-plasma membrane electron transport helps maintain a stable redox environment required for neuronal viability.
