    Notes: We have previously demonstrated that the peri-operative measurement of increased serum concentrations of the cardiac markers troponins I and T and creatine kinase-MB can be predictors of major cardiovascular outcomes (including cardiac death) at 3 months after surgery. In the present study, we have followed the postoperative course of 157 patients undergoing major vascular surgery or major joint arthroplasty to 1 year using a patient questionnaire, general practitioner follow-up and case-notes review. Increased postoperative marker concentrations were defined as values greater than the upper reference limit. Increases in troponin I and troponin T concentrations, as well as a single elevated creatine kinase-MB and two successively elevated creatine kinase-MB concentrations were measured in 12, 13, 33 and 15 patients respectively. Thirty-nine major adverse cardiac outcomes were recorded (cardiac death, myocardial ischaemia, congestive cardiac failure, unstable angina, cerebrovascular accident and major arrhythmias needing active treatment). There was no association between increases in any of these cardiac markers and cardiac death to 1 year. However, increases in troponin I and both a single elevated creatine kinase-MB and two successively elevated creatine kinase-MB concentrations were associated with an increased incidence of major cardiac outcomes, including cardiac death, to 1 year (odds ratio [95% confidence intervals] = 4.19 [1.16–14.87], 3.97 [1.65–9.44] and 5.19 [1.60–16.22], respectively).

    Notes: The effects of repeated point contact loading of a toughened ceria-stabilised zirconia ceramic, CeTZP, by softer metallic cones, have been investigated. All tests were conducted on a purpose-designed and built computer-controlled apparatus. The results clearly indicate that zirconia is susceptible to cyclic loading. The evolution of the cyclic deformation and cracking processes have been analysed in terms of contact pressures, applied loading conditions and number of cycles. The ground tips of both quenched, and quenched and tempered, silver steel cones are plastically deformed during the initial loading cycle to produce a conforming planar surface approximately 50-100 μm in diameter. In all cases, plastic deformation in the CeTZP was observed within, and adjacent to, the contact zones. The degree of plastic deformation increased as the number of cycles increased. The expansion associated with the tetragonal-to-martensitic phase transformation in the zirconia caused granular lifting from the surface, at the edge of the contact zone. This resulted in localised spalling at the peripheries of the contact zones, where the tensile stresses are the highest, followed by a radial expansion at higher cycle numbers. This investigation has identified that the rate of evolution of the fatigue damage, in decreasing order of significance, was controlled by: (a) cone hardness, (b) cyclic load amplitude and (c) mean load.

    Notes: Aims to measure the appropriateness of the prescribing of benzodiazepines and the effect of intervention strategies designed to improve the appropriate use of benzodiazepines. Cross-sectional data of appropriate prescribing of benzodiazepines were assessed before and after one of three intervention strategies. Appropriateness of prescribing was assessed using an algorithm based on published evidence. Hospitals were randomly allocated to receive verbal (oral communication), bulletin (paper communication) or control feedback (no communication). The change in rate of appropriate prescribing of benzodiazepines was compared between intervention groups. Gives results and concludes by highlighting areas which can be addressed to increase the impact of intervention in future studies.

    Notes: Abstract This study investigated anomalies in two species of benthic sculpins (Myoxocephalus octodecemspinosus and M. scorpius) inhabiting a marine inlet in Newfoundland, Canada, where effluent from a pulp and paper mill is discharged and from two reference sites. Condition factor of both species of sculpins from the polluted and a reference site was similar. Some differences in parasite fauna were observed. Tissues from reference fish, obtained from two uncontaminated sites for comparison, were normal. Impaired vision, fin and tail necrosis, hyperplasia of the gill epithelium, depletion of energy reserves in the liver, and multifocal hemosiderosis in the spleen were observed primarily in M. octodecemspinosus captured near the paper mill. These stress-associated lesions, probably caused by undetermined xenobiotics, appear to be associated with the sediment.
