    Notizen: Abstract The authors retrospectively reviewed ten pediatric brain tumor patients with intractable seizures who underwent lesionectomy without intentional identification and resection of the epileptogenic region to assess the clinical features and seizure outcome after lesionectomy in such patients. Seizures were complex partial in seven cases and simple partial, absence, and generalized tonic-clonic in one case each. Tumors mors were located at the medial temporal lobe in four cases, at the frontal lobe in four cases, at the parietooccipital and the suprasellar areas in one case each. The most common pathology was benign oligodendroglioma (five cases) followed by ganglioglioma (two cases). Others were pleomorphic xanthoastrocytoma, hamartoma, and primitive neuroectodermal tumor (one case each). In four cases, complete removal of the tumor was feasible. Postoperatively nine of the ten patients showed favorable seizure control (Engel's classification 1 and 2) and of these, six were seizure-free during the follow-up period (mean duration: 40 months). Therefore, lesionectomy can be an appropriate initial treatment for patients with brain tumor and medically intractable seizures.

    Notizen: Abstract In an investigation of the benefits and risks of direct surgical approaches to pediatric pineal tumors, the need for such approaches, the feasibility of surgical removal and operative complications were evaluated in 25 histologically proven cases. Five tumors were germinomas (GEs) or GE-predominant mixed germ cell tumors (GCTs), in which stereotactic biopsy can be adequate. Twenty of the 25 pineal tumors (80%) were teratomas (TEs), TE-predominant mixed GCTs or other tumors that need direct surgical approaches. Among the 21 tumors resected by the occipital transtentorial (OTT) approach, 13 were removed radically (〉95%) and 6 others, subtotally (〉75%). Complications of using the OTT approach included homonymous hemianopsia (7), Parinaud's syndrome (6), other eyeball movement limitation (7), seizure (5), and new hydrocephalus (3). The majority of these were transient, resolving within a few days or months, or easily controlled. The results showed that 80% of pediatric pineal tumors needed direct surgical approaches and that the majority were successfully removed by OTT surgery with an acceptable level of risk.

    Notizen: Abstract Congenital brain tumors are very rare, and 2–9% of them are accounted for by glioblastomas. We encountered a case of congenital glioblastoma detected at the 39th week of gestation by fetal sonography, which revealed a large echogenic mass in the left temporo-parietal area of the fetal brain with significant midline shift and dilatation of the contralateral lateral ventricle. A detailed sonogram obtained 7 h later showed that the mass had increased in size, and this suggested an expanding hematoma. An emergency cesarean section was performed. Postnatal MRI demonstrated an enhancing mass with a large hematoma. Biopsy revealed a malignant brain tumor. Further management was refused and the boy died 6 days after birth. The postmortem pathological diagnosis was glioblastoma. When fetal sonography demonstrates an echogenic mass, a congenital brain tumor should be considered. The mode of delivery should be determined by the nature of the mass and the condition of the fetus.

    Notizen: Abstract The authors report two cases with a rare complication of ventriculoperitoneal shunt. The peritoneal catheters migrated upward to the scalp and to the clavicular area. In one case, the whole length of the peritoneal catheter was pulled out from the peritoneal cavity and was coiled subcutaneously. Both of the patients had the habit of head rotation and one of the patients had a history of subcutaneous fluid collection at the site of coiling. At surgery, whitish inflammatory granulation tissues were noted around the catheter in both cases, which might act as an achoring point for the “windlass effect”.

    Notizen: Abstract Infarction of the territory supplied by the contralateral superior cerebellar artery in cases of descending transtentorial herniation has not been reported. The authors experienced a primitive neuroectodermal tumor in a patient whose consciousness deteriorated transiently due to descending transtentorial herniation. On the magnetic resonance imaging taken 8 days after the herniation, lesions thought to be infarction of territories supplied by the ipsilateral posterior cerebral artery and the contralateral superior cerebellar artery were noted. The possible mechanism of this peculiar pattern of infarction is discussed.

    Notizen: Abstract Primary spinal cord primitive neuroectodermal tumor (PNET) is a rare entity. In all, 13 cases have been reported in the literature, including 3 with intracranial seeding. A 3-month-old girl with involvement of the spinal cord below the mid-thoracic level is described. The brain MRI revealed findings indicative of seeding along the intracranial subarachnoid space. Biopsy, duraplasty and removal of laminotomy flap were done. In spite of a good response to the first cycle of postoperative ‘8-drugs-in-a-day’ chemotherapy, further treatment was refused. She died 21 days after the onset of leg weakness, which reveals the rapid progression of untreated cases. To our knowledge, this is the first case of spinal cord PNET with parenchymal involvement that has been described in an infant.

    Notizen: Abstract To evaluate the efficiency of our management protocol, 33 pediatric cases of bacteria-infected cerebrospinal fluid shunt were reviewed. The causative organism was staphylococcus in 23 patients. In 23 patients, shunt infection was managed according to the protocol. The complexity of the shunt system did not prolong hospitalization. Unchanged but externalized tubings showed persistent colonization despite adequate antibiotics in 10 of 21 patients. Staphylococcal infection was oxacillin-resistant in 7 of 19. The efficiency of an `off-antibiotics' trial was minimal. Further modification of the protocol is expected to enhance efficiency of the management.
