    Notes: The term ‘jingry back syndrome’ (ABS) was coined by Mitchell in 197S. It was.stated that a strong positive patch test reaclion could create an ‘angry back’ which becomes hyperretictiye to other patch test challenges. The present study investignled whether the ABS is a generalized state of hyper-reactivity of skin, whether it is a localized hyper-reactivity of skin. i.e. only in lhe close proximity to a strong patcli test reaction, whelhcr it is an individual specilic phenomenon and if ABS is a reproducible phenomenon. The studies failed lo demonstrate any generalized or localized change in the reactivity of the skin. This lell the possibility that ABS might be a rare, and individual-specilic phenomenon. However, even in the subjects who had previously been diagnosed as having ABS we failed to reproduce the angry back.

    Notes: Summary Diagnosis of allergic contact dermatitis depends upon the use of epicutaneous patch tests with suspected antigens. It has been claimed that the method is unreliable because of variability. Therefore, we have tested this by comparing responses to various forms of replicate, standardized challenge in nickel-sensitive subjects. Nickel-containing coins applied at 2-4 different sites on the back elicited similar responses in up to 95% of tests. Serial dilutions of nickel sulphate (NiS04) applied, in duplicate, gave highly reproducible responses on the back. However, the forearm skin was clearly less responsive. Examination of the effect of the quantity of nickel-containing vehicle showed that, above a certain minimal level, this had no effect on the reproducibility of the responses. In conclusion, the immune system can give highly reproducible responses when presented with standard eliciting stimuli.

    Notes: Summary Data on the use of oral cyclosporin A (CyA; Sandimmun®) therapy for severe adult atopic dermatitis have accumulated since 1987. Details of over 200 adult patients who have received short-term CyA were presented at an international workshop in April 1993. Eighty-six of these patients had participated in three randomized, double-blind, placebo-controlled studies in which CyA was given for 6–8 weeks. The efficacy and safety of short-term CyA treatment in atopic dermatitis is established, provided that appropriate guidelines are observed. Evidence to date suggests that atopic dermatitis patients are no more prone to toxicity than patients with psoriasis, in whom more detailed and longer-term data are available. Early data also suggest that long-term CyA may be effective in atopic dermatitis, but there is concern regarding long-term safety, as experience is still limited. Careful monitoring of all CyA-treated patients is therefore mandatory. CyA should only be used under the direct and regular supervision of a hospital-based dermatologist who is knowledgeable in the use of cyclosporin A, and experienced in the management of severe skin disease and in the use of potentially toxic drugs.

    Notes: We studied sensitization to dinitrochlorobenzene (DNCB) in psoriatic patients before, during and 2,4, and 6 weeks after a course of photochemotherapy (PUVA). The inhibition of sensitivity observed during PUVA was completely reversed after 6 weeks. Thus the PUVA-mediated inhibition of cutaneous immune responsiveness is of short duration. However, whether there are long-term sequelae remains to be established.

    Notes: We studied 67 patients with multiple contact allergies to determine whether there was an association of this state with any particular HLA antigen. HLA-A, -B and DR antigens were typed by standard serological methods. There was no significant HLA association, although there was an increased frequency of DR4 in those patients who included nickel as one of their sensitivities (64% compared with 33% in controls), and an increase in DR6 in those patients who included sensitivity to a rubber accelerator (45% compared with 16% in controls). However, when the probabilities were corrected for the number of HLA antigens tested and the number of substances in the patch test battery, these associations were no longer statistically significant. We also examined the morphology and numbers of Langerhans cells in epidermal sheets from six subjects with multiple allergies. There were no differences in appearance or numbers of Langerhans cells stained for ATPase, compared with 20 non-allergic controls.

    Notes: Five patients with autosomal dominant ichthyosis vulgaris (ADI) were studied to see whether the abnormal keratinization was associated with disturbances of the appearance or the distribution of epidermal Langerhans cells (LCs). The LCs were identified by ATPase staining and electron microscopy. They were present in normal numbers, were of normal morphology and were in their usual mid-epidermal position. These observations do not support the hypothesis that LCs are involved directly in the process of keratinization.

    Notes: The inhibitory effect of PUVA on delayed cutaneous hypersensitivity to 2,4-dinitrochloroben-zene (DNCB) has been analysed by sensitizing and challenging patients with psoriasis on shielded and treated areas of skin, and comparing the results with those for untreated patients. PUVA suppresses both induction and expression of delayed cutaneous hypersensitivity by systemic as well as local actions, but the major effect is local inhibition of induction. The possible contributions of Langerhans cell depletion, impaired lymphocyte reactivity, and reduced cutaneous inflammatory responses are discussed.

    Notes: Cell mediated immune reactivity was studied in eighty-five psoriasis patients and twenty-five healthy controls by an improved (quantitative) method for measuring contact hypersensitivity to 2,4 dinitrochlorobenzene (DNCB). Patients were sensitized with 500 μg DNCB, the lowest dose found to sensitize all healthy subjects. Responses to epicutaneous challenge with a series of concentrations of DNCB were measured as volumes calculated from diameter and thickness and the various groups were compared by the differences in the log dose-response curves. Patients with untreated psoriasis were less responsive than healthy controls; responses were less still in patients treated with dithranol/UV-B/tar and they were least of all in patients treated with photochemotherapy with 8-methoxypsoralen and UV-A (PUVA), particularly in those who pigmented least. Sensitization and challenge at different stages of treatment showed that both induction and elicitation of sensitization were impaired by PUVA. The possible relationship of these changes to DNCB metabolism, Langerhans cell damage and a decrease in circulating T cells is discussed. Although the clinical significance of the findings is unknown, treatment with dithranol/UV-B/tar has proved safe over many years of use.
