    Notes: Abstract Streptococcus faecium strain 3 produced a bacteriocin (enterococcin Sf3) and contained a homogeneous species of plasmid DNA (pJK3) with an Mr of 3.5 · 106. Plasmid pJK3 was transferable in a filter mating procedure to S. faecium M16. The non-bacteriocinogenic strain M16 was susceptible to enterococcin Sf3 and harboured a non-selftransferable 19.1 MDa plasmid, which was responsible for erythromycin resistance. Transcipient cells of S. faecium M16 contained the 19.1 MDa and the pJK3 plasmid, produced the enterococcin Sf3 and were resistant against the inhibitory action of this bacteriocin.

    Description / Table of Contents: Zusammenfassung Antibiotikaspiegel im Knochen, die ohne Berücksichtigung der Blutmenge im untersuchten Material ermittelt werden, sind nicht unbedingt relevant. Unter „Knochengängigkeit“ eines Antibiotikums muß das Penetrationsvermögen in den extravasalen Raum des Skelettsystems verstanden werden. Es wird eine Methode beschrieben, bei der jede Knochenprobe vor der Homogenisierung und Elution des Wirkstoffes in die anatomischen Bestandteile Kompakta, fetthaltiges Mark und bluthaltiges Mark zerlegt wird. Der Blutanteil der Blutspongiosa wird durch Bestimmung der Hämoglobinkonzentration in ihrem Eluat berechnet. Es zeigt sich, daß die in der homogenisierten Blutspongiosa gemessene Gentamicinaktivität dem anteiligen Blutspiegel weitgehend entspricht. Dieses Ergebnis und negative Befunde an der Kortikalis belegen — im Gegensatz zu anderen veröffentlichten Ergebnissen —, daß ein meßbarer Übertritt von Gentamicin aus der Blutbahn in gesundes Knochengewebe bei kurzfristiger Behandlung nicht erfolgt. Prinzipiell unterstreichen die Ergebnisse die Forderung, bei der Untersuchung von Antibiotika-Konzentrationen im stärker durchbluteten Gewebe den jeweiligen Blutanteil quantitativ zu berücksichtigen. Die Bestimmung der Hämoglobinkonzentration im Gewebeeluat kann hierzu empfohlen werden.
    Notes: Summary Antibiotic levels in bone which are estimated without taking into account the retained blood are not necessarily relevant. Only the antibiotic found in the extravascular spaces of the skeletal system is a measure for its degree of penetration into bone. With the described method each bone sample was separated into its anatomical components: compact tissue, yellow, and red marrow, before they were homogenised, and the drug was extracted. The amount of blood in the red marrow was determined from the hemoglobin concentration in the extraction fluid. It was shown that the gentamicin concentration in the red marrow really represented its concentration in the retained blood. In contrast to other observations, this result and the negative findings in the compact tissue indicate that during short-term therapy there occurs no measurable penetration of gentamicin from the blood into healthy bone. In investigations of the antibiotic concentration in highly vascular tissue, the retained blood should be taken into consideration quantitatively by determining the hemoglobin content in the extraction fluid.

    Description / Table of Contents: Zusammenfassung An 609 Patienten mit Verdacht auf koronare Herzerkrankung (KHK) wurden die Verteilung und der Schweregrad einer begleitenden renovaskulären Erkrankung (RE) untersucht. 152 dieser Patienten (25%) waren von einer RE betroffen. Unter den 399 Patienten mit signifikanten Koronarstenosen (〉50%) fanden sich 143 mit renovaskulärer Beteiligung (35,8%). Geringgradige Veränderungen (20–40% Stenose) stellten sich hierbei in 12,5%, mittelgradige (40–60% Stenose) in 10,5% und höhergradige (〉60% Stenose) in 12,8% der Patienten dar. In allen drei Gruppen zeigte sich signifikant häufiger eine 3-Gefäßerkrankung (GE) als eine 1- oder 2-GE (p〈0,01). Der Effekt einer perkutanen Revaskularisation auf Blutdruck und exkretorischen Nierenfunktion wird für höhergradige Stenosen in der Literatur uneinheitlich bewertet. Bei den 51 Patienten mit Stenose 〉60% führte eine Stentimplantation zu einer signifikanten Reduktion der Blutdruckwerte von 163±30 auf 145±17 mmHg systolisch und von 93±18 auf 83±10 mmHg diastolisch (p=0,008), sowie zur Abnahme der antihypertensiven Medikation (3,2±0,9 auf 2,8±1,0; p=0,005). Die Serumkreatininkonzentration verringerte sich nicht signifikant von 1,46±0,70 auf 1,39±0,58 mg/dl. Die Verteilung der Schweregrade einer RE in einem großen kardiologischen Patientengut liegt jeweils bei ca. 1/3 für gering-, mittel- und höhergradige Nierenarterienstenosen und geht in jeder dieser Gruppen mit einer progredienten KHK einher. Dabei können höhergradige Nierenarterienstenosen durch eine elektive Stentimplantation sicher und blutdruckeffektiv revaskularisiert werden.
    Notes: Summary To explore the relationship between coronary artery disease and renal vascular disease, we performed renal arterial angiography in 609 patients undergoing coronary angiography for suspected coronary artery disease. We defined renal artery stenosis as nonsignificant (〈40%), borderline (40–60%) and significant (〉60%). One-hundred fifty-two patients had renal artery stenosis, while 457 did not. Two-hundred and ten patients had no coronary disease; of these, only 9 had renal artery stenosis. On the other hand, the 143 patients with renal artery stenosis, when subdivided, had similar degrees of coronary disease; three vessel disease was significantly more common than one or two vessel disease in all groups. Renal artery stenosis of all severity degrees was associated with common atherosclerotic risk factors. However, hypertension was not a clue to the presence of renal artery stenosis. To evaluate the effect of percutaneous revascularization on hypertension and renal function all 51 patients with significant renal artery stenosis were treated by primary stent implantation and were follwed up for 6 months. Stent implantation showed a marked decrease in systolic and diastolic blood pressure (163±30 to 145±17 and 93±18 to 83±10 mmHg; p=0.008) with a decrease in the amount of antihypertensive medication but without beneficial effect on serum creatinine during follow-up (1.46±0.70 mg/dl to 1.39±0.58 mg/dl, p=ns). We conclude that renal artery stenosis of any severity is strongly suggestive of three vessel coronary artery disease. The fact that renal stenting lowers blood pressure decreases antihypertensive drugs and increases medication flexibility in patients with coronary artery disease would support the notion of revascularization in patients with significant stenoses.

    Notes: Abstract An activated renin-angiotensin system is a major risk factor for cardiovascular events. Angiotensin II acts on AT1 and AT2 receptors. Stimulation of AT1 receptors is associated with ¶• endothelial dysfunction, mainly as the consequence of an increased vascular production of superoxide radicals, ¶• vasoconstriction, ¶• platelet activation, ¶• enhanced release of plasminogen activator inhibitor-1, ¶• activation of immediate early genes c-fos and c-jun, ¶• myocyte hypertrophy, ¶• connective tissue formation, ¶• endothelin-1 synthesis, and ¶• activation of growth factors like PDGF and TGF-β1. ¶Stimulation of AT2 receptors can mitigate or abolish the growth promoting effects of AT1 receptor stimulation. The contribution of these effects – single or in combination – on the progression of atherosclerotic lesions, the phenomenon of restenosis and the process of remodeling in heart failure is being progressively elucidated. With increasing knowledge about these relationships the inhibition of AT1 receptors appears as a main target in preventive and reparative strategies in cardiovascular diseases.

    Description / Table of Contents: Summary Various methods of lumbosacral fusions for the treatment of degenerative spinal diseases are used clinically. Results vary greatly depending on indication, type of fusion, implants, and method of evaluation. In a retrospective clinical and radiological examination after an average follow-up time of 3.9 years this study reports on the outcome of lumbosacral distraction spondylodesis (LSDS) in a consecutive series of 147 patients being fused for the treatment of spondylolisthesis, failed-back syndrome, or lumbar instability. LSDS consists of a posterolateral fusion together with an autologous corticocancellous H-graft wedged under distraction between the spinous processes of L4 and S1. With 81.0% good and excellent results this noninstrumented fusion technique showed the best outcome in patients with spondylolisthesis, while in cases with a failed-back syndrome or lumbar instability only 62.3% excellent to satisfying outcomes were noted. The rate of pseudarthrosis was 13.6% in the whole patient group; no major complications such as nerve root damage, postoperative neurological deficits, or spinal stenosis were found.
    Notes: Résumé Diverses méthodes de fusion lombo-sacrée sont utilisées dans le traitement des affections dégénératives rachidiennes. Les résultats sont très variables et dépendent de l'indicatio, du type de fusion, des implants et de la méthode d'évaluation. Par une analyse rétrospective clinique et radiologique après un délai postopératoire moyen de 3,9 mois, cette étude rend compte du résultat de la “spondylodèse lombo-sacrée en distraction” (LSDS) dans une série consécutive de 147 patients traités par arthrodèse pour spondylolisthésis, échec de la chirurgie lombaire ou instabilité lombaire. La LSDS consite en une greffe postéro-latérale associée à un greffon autologue cortico-spongieux en H enfoncé sous distraction entre les épineuses de L4 et de S1. Avec 81,0% de bons et excellents résultats, cette technique d'arthrodèse sans ostéosynthèse a montré ses meilleurs résultats dans le traitement des spondylolisthésis, tandis que dans les cas d'échec de la chirurgie lombaire ou d'instabilité lombaire il n'a donné que 62,3% des résultats excellents ou satisfaisants. Le taux de pseudarthroses était de 13,6% pour tout le groupe et l'on n'a pas trouvé de complications majeures comme une atteinte radiculaire, un déficit neurologique post-opératoire ou une sténose lombaire.
