    Notizen: An unusual giant-cell granulomatous inflammatory oral lesion is characterised by the presence of hyaline rings and by a lack of agreement on their nature, opinions ranging from their being hyaline degenerative blood-vessels to remains of leguminous cells. Ten such lesions and a variety of vegetables have been examined in the present investigation. Light-microscopical examination of the lesions revealed the hyaline rings surrounded by inflamed granulation tissue with associated multinuclear giant cells. Electron-microscopical examination showed them to consist of a material similar to that of vegetable cell walls, sometimes with an associated superficial layer of collagen. The observations indicate that the lesions consist of vegetable remains with an associated foreign-body type of inflammatory response.

    Notizen: Opportunistic fungal infections account for a significant amount of morbidity associated with HIV disease. We report here a case of localised oral histoplasmosis without evidence of disseminated disease in a patient who lacked stigmata of HIV disease at the time of initial presentation. The diagnosis is based on histology with special stains, complement fixing antibodies in serum, and culture of the organism from fresh tissues. Activation of subclinical disease following an infection in Uganda may explain the development of these exophytic oral lesions in this British resident.

    Notizen: Ductal ligation of the sublingual gland induced mucocele formation in a high percentage of cats. Extravasated mucus was associated with all sublingual glands up to 20 days after ligation, and extravasation mucoceles were associated with 12 out of 27 sublingual glands between 7 and 365 days after ligation. Enzyme-histochemic examination revealed that the extravasated mucus induced a migration of macrophages and a fibro-blastic reaction. It is thought that if these reactions are sufficiently intense in the early days after ligation, then continued spread of extravasated mucus and mucocele formation does not occur; however, if the extravasation is too great to be contained, a mucocele forms in which a balance develops between extravasation of mucus and its removal. Possibly the numerous macrophages in the mucoceles are involved in degrading mucus and facilitating its removal.

    Notizen: Abstract: A variable response following ductal ligation of feline salivary glands corresponds to the human condition but contrasts with a predictable atrophy in obstructed salivary glands of rodents popularly used as a model for human salivary problems. The present investigation is concerned with a possible reason for the variable response, namely the preservation of the innervation. Ducts of feline submandibular and sublingual salivary glands were ligated with or without the inclusion of the chorda tympani. Inclusion led to a delayed initial response followed by progressive atrophy until the parenchyma was extremely atrophic, whereas avoidance of the chorda led to the variable response in which variable numbers of acini of a similar form to normal persisted. The results establish the atrophic effect of inclusion of the chorda tympani in ductal ligation and indicate the caution that should be exercised in the extrapolation of the rodent model to the human condition.

    Notizen: As the genesis of salivary calculi in man has not been established, the fortuitous observation of microcalculi in sublingual glands of cats prompted this study. Microcalculi were seen occasionally within acinar cells and lumina and macrophages, and rarely interstitially. Microcalculi were stained with the periodic-acid/Schiff technique, and acid-phosphatase activity was associated with intracellular microcalculi. Intracellular microcalculi were seen ultrastructurally in acinar cells in membrane-bound vacuoles that also contained debris and sometimes what appeared to be secretory material. X-ray microanalysis showed the microcalculi to consist of crystals containing calcium and phosphorus. The observations suggest that the microcalculi form within autophagosomes in acinar cells by a precipitation of calcium and phosphorus present in degenerate material, and that the microcalculi eventually enter lumina, where they may be discharged in the saliva or retained and enlarge.

    Notizen: Although microliths occur in normal human salivary glands and may be an aetiological factor of sialadenitis, little is known of their natural history. In an attempt to remedy this, we investigated a large archival collection of normal and experimental feline parotid, submandibular and sublingual salivary glands. In submandibular and sublingual glands, microliths were detected ultrastructurally in: all types of acinar secretory cells; myoepithelial cells; ductal cells; lumina; intercellular spaces; basement membrane; stroma; macrophages; multinuclear giant cells; and neutrophils. Microliths were not detected ultrastructurally in parotid glands. Microliths appear to form in acinar cells during autophagy and in stagnant secretory material in lumina. Microliths appear to be removed by secretion in the saliva, discharge from cells laterally and basally, and engulfment by macrophages. There appears to be a turnover of microliths, which possibly is upset by secretory inactivity with a resulting accumulation that leads to localized obstruction and sialadenitis.

    Notizen: A search for microcalculi was made in 14 cases of chronic submandibular sialadenitis. Microcalculi were found in all cases. They were within serous acinar cells and ductal cells, within lumina, and interstitially. They were stained variably by periodic-acid/Schiff and Alcian Blue at pH 2.5. Ultrastructural analytical examination showed them to consist of crystals containing calcium and phosphorus. The observations support the possibility that microcalculi form in autophagosomes, enter lumina and occasionally become impacted to produce sialolithiasis and sialadenitis.

    Notizen: The pattern of molar and canine contacts and the contractile activity of the masseter and anterior temporal muscle were studied, through integrated electromyography, during right-sided mastication of banana and apple in ten healthy male subjects. Tooth contacts occurred on both the chewing and the non-chewing side. This study does not support the clinical concept that non-chewing side contacts are necessarily detrimental to the jaw muscles and the temporomandibular joints. The activity of the elevator jaw muscles and the tooth contact patterns suggest that the mandible tilted around a sagittal axis, and rotated around a vertical axis, during the phase of elevation of a masticatory cycle.

    Beschreibung / Inhaltsverzeichnis: Résumé Malgré une diminution lente de l'incidence du cancer gastrique des 90 dernières années, 100000 patients vont mourir de cette maladie chaque décennie en Angleterre et au pays de Galles. La survie à 5 ans ne s'est pas beaucoup amélioré au cours de ce siècle, essentiellement parce que le stade au moment du diagnostic n'est pas modifié. II existe bon nombre de facteurs pronostiques cliniquement significatifs. L'âge est un facteur important car les lésions sont fréquemment plus avancées chez le patient de moins de 40 ans si bien qu'il faut avoir une forte suspicion devant un patient jeune chez lequel les symptômes persistent. Il n'est pas clairement établi d'après la littérature que la durée des symptômes soit corrélée au stade tumoral, mais il est évident que le raccouricissement de la période des symptômes par une mise en ocuvre des investigations peut amener à faire un diagnostic rapide et à traiter une plus grande proportion de patients à un stade précoce de la maladie. Le site des lésions est également important: actuellement, l'augmentation de la proportion des cancers du cardia est responsable d'une réduction de la survie globale. La taille de la lésion ne devrait pas intervenir dans la décision de résection car la plupart des études ne montrent pas de rapport entre la taille de la lésion et le stade. Le stade de la tumeur, par contre, est le facteur pronostique le plus important car il est démontré que les efforts pour augmenter la proportion des patients avec une maladie au stade I, stade chirurgicalement curable, modifient l'histoire naturelle de ces tumeurs.
    Kurzfassung: Resumen El cáncer gástrico es responsable de alrededor de 10,000 muertes por año en Inglaterra y Gales y la rata global de sobrevida a 5 años se ha mantenido estática alrededor del 5% en el curso de los últimos 50 años. A pesar de la pobre tasa global de sobrevida, existe un número de factores que tiene influencia en el resultado a largo plazo luego de tratamiento en los pacientes con cáncer del estómago. Esta bien establecido que el estadío del tumor es el más importante factor de pronóstico y que la razón para la no mejoría en la tasa de sobrevida es que la mayoría de los pacientes se presenta en los estados III o IV de la enfermedad; precisamente ésta es un área donde los avances en el diagnóstico precoz pueden resultar en una mejor sobrevida global. En esta revisión se discuten los diversos determinantes principales de pronóstico que tienen relevancia clínica en el cáncer gástrico.
    Notizen: Abstract Despite a slow decline in the incidence of gastric cancer over the last 90 years, we can still expect to see over 100,000 patients die of this disease each decade in England and Wales. The 5-year survival rate has not improved during this century, which is largely due to the stage at diagnosis being unchanged. There are a number of prognostic determinants in gastric cancer which have clinical relevance. Age is an important determinant; patients under 40 years commonly have more advanced diffuse lesions than older patients so that a higher index of suspicion needs to be maintained in younger patients with persistent symptoms. Conflicting reports make it unclear whether the duration of symptoms bears any relationship to tumor stage, but there is some evidence that actively shortening the symptom duration by early investigation can have a beneficial effect in the proportion of patients diagnosed with early cancers. The site of the tumor is important; unfortunately, the proportion of patients with cardia lesions is increasing and this has had the effect of reducing the overall survival. Tumor size should not play a part in the decision to resect a lesion as most studies show no clear relationship between tumor size and stage. Tumor stage is the most important prognostic determinant and efforts to increase the proportion of stage I cancers presenting for surgery can be shown to alter the natural history of the disease, by diagnosing it when it is still surgically curable.
