    Notizen: To determine maternal and fetal mortality associated with Eisenmenger's syndrome in the UK, a postal questionnaire was sent to 225 NHS obstetric units with neonatal intensive care units, requesting information about maternal and fetal outcome in cases of Eisenmenger's Syndrome between 1991 and 1995. Fifteen cases were identified. The maternal mortality was 40% and fetal loss 8%. Only 15% of infants were born at term. Maternal mortality associated with Eisenmenger's syndrome remains as high as it has been for the past 50 years. Pooling of national data on rare medical conditions in pregnancy is required to aid management of individual cases.

    Notizen: A postal survey of 801 members of the Obstetric Anaesthetists Association was carried out in the United Kingdom and Ireland to investigate the use of surgical facemasks whilst performing spinal and epidural blocks. Two hundred and twenty three out of 539 respondents (41.3%) routinely wore masks for both spinals and epidurals; 22 (4.1%) wore masks only for epidurals; 21 (3.9%) wore masks only for spinals and 273 (50.6%) did not wear masks for either spinals or epidurals. Fifty out of 240 (21%) of those who routinely wore masks did not believe that wearing a mask reduced the risk of infection. Only 83 out of 259 (32%) mask wearers changed their masks between cases.

    Notizen: The trachea of an 81-year-old woman was intubated with an orotracheal tube for emergency abdominal surgery and the tube was left in place for postoperative management on the Intensive Care Unit. After 36 h, she developed apparent massive tongue swelling. On closer examination, a normal sized tongue was found to be displaced by oedema of the floor of the mouth and submandibular space, secondary to purulent sialadenitis from right submandibular duct obstruction by the tracheal tube.

    Notizen: Tracheal intubation may be hindered by difficulty in insertion of the laryngoscope blade into the patient's mouth because the handle impinges on the patient's chest or on the hand of an assistant applying cricoid pressure. An adaptor is described which modifies the standard Penlon laryngoscope to enable lateral swivelling of the handle, thus avoiding the obstruction.

    Notizen: Unfractionated heparin is widely used for prophylaxis against venous thromboembolism after Caesarean section. We performed a survey of thromboprophylactic methods after elective Caesarean section in 50 maternity units in the United Kingdom. We found that a variety of regimens were used. Thirteen (26%) used subcutaneous unfractionated heparin at standard (non-pregnant) doses. We then studied anti-Xa activity in women following elective Caesarean section under regional anaesthesia. Initially, eight women were given 5000 U unfractionated heparin subcutaneously after surgery and anti-Xa activity was measured 1, 2, 3, 4, 5, 6, 8 and 10 h after administration. There was no detectable anti-Xa activity in any of the samples so the dose was increased to 7500 U in a further five women and a single anti-Xa assay performed at 3 h when peak activity should occur. Again, no activity was detected so the dose was increased to 10 000 U heparin in a final group of 10 women and anti-Xa activity measured at 0.5, 1, 1.5, 2, 3, 4, 5 and 6 h. Although there was some activity after 10 000 U heparin, the level was below that accepted for prophylaxis. If anti-Xa activity is an appropriate monitor of prophylactic unfractionated heparin, doses up to 10 000 U are inadequate. Since there is evidence that enoxaparin is effective at producing adequate prophylactic anti-Xa activity following Caesarean section, we suggest abandoning the use of unfractionated heparin in favour of enoxaparin for this purpose.

    Notizen: We conducted a prospective randomised study to compare the intubating laryngeal mask tracheal tube with a standard tube, for ease of tracheal intubation over a fibrescope. Thirty-six patients were investigated, using a sequential analysis technique. Ease of intubation was improved when the intubating laryngeal mask tracheal tube was used compared with the standard tube (p = 0.0009). Median (interquartile range [range]) intubation times (time from the tube's tip being level with the patient's teeth to successful placement) were 1.5 (1–2.3 [1–3]) s when the intubating laryngeal mask tracheal tube was used and 5 (3–7.3 [3–13]) s when the standard tube was used (p 〈 0.0001).
