    Notizen: Abstract We report results of a prospective study of daily 5-ASA enema therapy in patients with proctocolitis that was unresponsive to conventional therapy. Forty-seven patients with active colonic inflammation distal to the splenic flexure have been followed for three months to three years. Rapid improvement was noted within three months in 41 of 47 (87%) patients, while six failed therapy. Of the 41 patients responding to therapy, 39 achieved complete remission. Nineteen patients have relapsed at least once following discontinuation of 5-ASA, but 23 of 25 (92%) relapse episodes responded to a course of 5-ASA therapy. Complications of 5-ASA therapy were limited to hemorrhoidal irritation and local perianal injury. We conclude that 5-ASA enema is effective in inducing remission in patients with proctocolitis unresponsive to conventional therapy.

    Notizen: Abstract A prospective multicenter trial was performed to evaluate the use of external shockwave lithotripsy (ESL) and adjuvant medical therapy for the treatment of gallstones. A Medstone STS lithotripter was used together with ursodiol. Two hundred twenty-three patients were treated under general anesthesia (75%) or with intravenous analgesia (25%). Initial treatments were on an inpatient basis, but as centers gained experience, outpatient treatments became more common. Stone fragmentation and clearance were greatest in patients with solitary gallstones 〈2 cm in diameter. In this group of patients, stone fragmentation occurred in 97% of patients, and the cumulative stone-free rates at three and six months were 54% and 90%, respectively. These results indicate that fragmentation of gallstones can be achieved by a dry shock-wave lithotripter and that stone clearance is induced more rapidly by external shock-wave lithotripsy and adjuvant ursodiol therapy than by ursodiol therapy alone.

    Notizen: Abstract A 23-year-old woman developed acute fatty liver of pregnancy in association with acute pancreatitis, renal hypoperfusion and protein-losing enteropathy. Even though hepatic function improved, the patient succumbed to extrahepatic manifestations of the condition. This case demonstrated the multisystem involvement of this entity and suggests the possibility that manifestations are related to the presence of a vasoconstrictor substance.

    Notizen: Abstract Recent studies have demonstrated that cholestyramine, an anionic exchange resin, binds inorganic and hemoglobin ironin vitro and inhibits the absorption of these compounds from the rat small intestine. However, these studies did not answer the question: Will prolonged cholestyramine administration cause depletion of iron stores and development of iron deficiency anemia? Accordingly, rats were fed diets of standard rat chow or standard rat chow containing either 2 or 5% cholestyramine for 6 months. In the 5% cholestyramine group the hematocrit and hemoglobin values were significantly lower as compared to controls (37.5 vs 43.6%;P〈0.001) and (11.4 vs 14.2 g%;P〈0.001). Serum iron values in the 5% cholestyramine groups were significantly less than the control group (158±23 vs 205±15 µg%;P〈0.001). In the control group nonheme iron in the liver, spleen and duodenum was 206±31, 1482±212 and 12.5±1.1 µg/g tissue, respectively. In the 2% cholestyramine-treated group, liver nonheme iron was reduced to 172±33 µg/g, spleen nonheme iron to 1266±447 µg/g and duodenal iron to 11.7±1.0 µg/g. In the 5% cholestyramine-treated group liver nonheme iron was significantly reduced to 143±35 µg/g, spleen nonheme iron to 1030±3.4 µg/g and duodenal nonheme iron to 10.2±1.4 µg/g respectively. In addition, a significant decrease in serum vitamin B12 levels was noted in cholestyramine-fed rats compared to controls (256±83 vs 404±118 µµg/ml). These data indicate that prolonged administration of cholestyramine to rats results in significantly decreased serum iron, tissue nonheme iron stores and serum vitamin B12 levels associated with decreased hematocrit and hemoglobin levels.

    Notizen: Abstract In the present investigation, studies were carried out to examine the effect of a vegetable-protein diet as compared to an animal-protein diet on standard clinical and laboratory parameters in 3 patients with chronic hepatic encephalopathy. To evaluate more objectively the effects of vegetable or animal protein diets, an hepatic encephalopathy index was developed, and this included the following parameters: fetor hepaticus, asterixis, stage of hepatic coma (Parson-Smith), arterial ammonia levels, EEG, Reitan trail test, and any reduction or improvement in dietary protein tolerance. In controlled, randomized, single-blind trials, it was found that a vegetable-protein diet resulted in overall clinical improvement, decreased hepatic encephalopathy index scores, decreased arterial ammonia levels, improved performance on intellectual tasks, and in one case, markedly improved protein tolerance. Further, the beneficial effects of a vegetable-protein diet were enhanced by the use of the synthetic disaccharide, lactulose. These studies do not provide a clear-cut definition of the mechanism for the observed beneficial effects of vegetable protein in chronic hepatic encephalopathy. However, the most plausible explanation is that vegetable-protein diets contain lower amounts of certain amino acids and other putative materials which may be important in the pathogenesis of hepatic encephalopathy.

    Notizen: Abstract The first patient with a well documented case of Whipple's disease, in whom an anaerobic Corynebacteria was successfully cultured both from the mucosa of the small intestine and from mesenteric lymph nodes is described. Electron microscopic studies demonstrated the organisms in the mucosa of the small intestine and in lymph nodes. In addition, vascular invasion was observed and the vascular endothelium contained a large number of bacilli. These observations support the concept that anaerobic Corynebacteria may be important in Whipple's disease, but the precise role of this organism in the pathogenesis of this disorder remains to be elucidated.
