Zusammenfassung Für die symptomlosen, symptomatischen, diasezierenden und rupturierten Aneurysmen der thorakalen und abdominalen Aorta werden Spontanprognose und Operationsletalitität gegenübergestellt. Die daraus abzuleitende Operationsindikation wird bei dem auf arteriosklerotischer Basis entstandenen Aneurysma eingeschränkt durch die häufigen Begleiterkrankungen vor allem vasculdrer Genese. Das therapeutische Vorgehen beim dissezierenden Aneurysma wird bestimmt durch seine Lokalisation, die damit verbundenen akut bedrohlichen Komplikationen und das hohe Risiko gefäßchirurgischer Korrektur in der akuten Dissektionsphase.

Summary The spontaneous prognosis and operative mortality of asymptomatic, symptomatic, dissecting and ruptured aneurysms of the thoracic and abdominal aorta are compared. The indications for surgery derived from this comparison must be modified due to the frequent concomitant vascular and other diseases, especially those of atherosclerotic origin. The treatment (medical or surgical) of dissecting aneurysm is determined by its localization, its acutely dangerous complications and the high risk attached to vascular surgery in the acute stage of dissection.
