    Notizen: Lichen sclerosus is among the most frequently seen paediatric vulval disorders. In adults a strong association between lichen sclerosus and autoimmune diseases, and also with HLA class II locus DQ7, has been well demonstrated in women and a weaker association in men. These associations have not previously been studied in children, although in other autoimmune diseases, the HLA associations have been strongest in children. We performed HLA tissue typing and looked for autoimmune associations in a group of 30 children with vulval lichen sclerosus. HLA DQ7 was present in 66% of female children with lichen sclerosus compared with 31% in controls. Previous studies reported DQ7 in 51% of adult female patients and 45% of male patients. Sixteen per cent of the children were homozygous for DQ7 as opposed to 5% of controls. In the childhood group, only 4% had another autoimmune disease, but 56% of their parents or grandparents did. Age differences make comparison difficult, but the family history of autoimmunity appears to be strong in the early-onset group, in addition to the stronger association with DQ7.

    Notizen: Compositae dermatitis is rare in childhood, with few cases documented in the literature. We report a 7-year-old boy who presented with a dermatitis mainly affecting his dominant hand. Patch testing revealed contact sensitivity to sesquiterpene lactone (SL) mix, and subsequent testing with Compositae oleoresins showed sensitivity to daisy, dandelion and chrysanthemum. The dermatitis improved on avoidance of handling plants. We have not documented any other cases of Compositae sensitivity in 187 other children, aged under 16 years, who have undergone patch testing with SL mix between 1992 and 1996 in our department. This case illustrates that Compositae sensitivity may present with a localized dermatitis and that, although uncommon, sensitization may occur in early childhood.

    Notizen: A wide range of cutaneous reactions have been described in association with lithium therapy. Follicular eruptions have been noted briefly, but are not well documented in the dermatological literature. We report a 68-year-old man who developed a florid erythematous follicular eruption on the extensor surfaces of the limbs, with macular truncal erythema 6 months after commencing lithium therapy. His blood lithium levels were within the therapeutic range and there was no clinical evidence of lithium toxicity. Histology of lesional skin showed perifollicular inflammation and follicular plugging. The eruption improved on drug withdrawal. The cutaneous reactions to lithium and possible pathogenic mechanisms are discussed.

    Notizen: Two patients with clinical and laboratory evidence of co-existing lichen sclerosus and eicatricial pemphigoid are reported. Autoimmune bullous diseases affecting the vulva may mimic lichen sclerosus but in these two patients both diseases were present. These two diseases have not previously been reported simultaneously to date.

    Notizen: Background Histological criteria in lichen sclerosus have been correlated with disease duration. A progressive tendency for the inflammatory infiltrate to become more deeply placed and more sparse with long-standing disease has been described together with a tendency for collagen homogenisation in the upper dermis to become more prominent with time.Materials and methods We performed a retrospective Wind clinico-pathological study on 20 untreated patients with lichen sclerosus. We looked at sections from five additional patients with lichen sclerosus who required serial biopsies in the course of their disease. The purpose was to observe the histological patterns at different stages of the clinical course.Results We found a poor correlation between estimated disease duration and histological criteria for early and long-standing disease.Conclusion We conclude that the pathological process in lichen sclerosus is a continuing process and the inflammatory component can be a persistent or recurring phenomenon which may be site-determined. The estimation of disease duration in vulval lichen sclerosus using histological criteria is unsatisfactory.

    Notizen: The characteristic expression of keratins by keratinocytes is well documented. A typical ‘hyperproliferative’ profile of epidermal keratin expression occurs in psoriasis, wound healing and warts. This study analyses keratin expression in cutaneous lichen planus to determine abnormalities of differentiation occuring in this inflammatory disorder. Using a panel of monoclonal antibodies 28 samples (20 patients) were studied. The results showed that squamous differentiation was unaffected, with keratins K1 and K10 being expressed normally for the site sampled. The main abnormalities included extension of reactivity of the basal cell marker, LH8, into the suprabasal compartment. Keratin K17, usually restricted to adnexal structures, was variably expressed in the basal and suprabasal layers of the interfollicular epithelium of affected epidermis. Keratins K6 and K16, found suprabasally in hyperproliferative states, were detected both basally and suprabasally in all diseased samples. The keratin profile in lichen planus is analogous to the wound healing response. Suprabasal keratin K17 is found in psoriasis, wound healing and viral warts so the changes in keratin K17 may reflect hyperproliferative changes. It is likely that the changes in epidermal keratin expression are due to up-regulation of specific keratin genes by the production of cytokines and inflammatory mediators from the lymphocytic infiltrate typical of lichen planus.
