    Notes: Abstract An anatomic study of the lateral extraosseous and intraosseous arterial supply of the calcaneus and the lateral soft tissue was carried out on 10 fresh lower adult cadaver legs using different anatomic and radiologic procedures (plastination, modified Spalteholz clearing technique, digital subtraction and rotational angiography and computed tomographic angiography). Consistent patterns of extraosseous and intraosseous lateral calcaneal vascular anatomy were demonstrated. The lateral calcaneal artery is a branch of the anterior tibial artery which crosses over the calcaneal tuberosity and forms a large lateral arch with the lateral tarsal artery which is a branch of the dorsalis pedis artery. The intraosseous circulation is supplied laterally by the lateral calcanear artery, medially via the short branches of the lateral plantar artery. Comparing magnet resonance images after fresh calcaneal fractures the lateral calcanear artery may be interrupted by the impacted lateral bulge, by the conventional lateral surgical approach, or by applying a lateral osteosynthesis plate. This may cause avascular bone necrosis. Furthermore the lateral calcanear artery can clinically serve as a vascular pedicle for a local rotational skin flap to cover soft tissue defects of the heel.

    Description / Table of Contents: Résumé L'étude anatomique de la vascularisaiton artérielle extra et intraosseuse du calcanéus et des parties molles a été faite sur 10 jambes fraîches de cadavres d'adultes. Différentes techniques anatomiques et radiologiques ont été utilisées : plastination, diaphanisation (technique de Spaltholtz modifiée), angiographie numérisée par soustraction, angiographie rotationnelle et angiographie par tomographie computérisée. Les aspects habituels de l'anatomie vasculaire de la région latérale du calcanéus ont été décrits : l'ACL est un rameau de l'artère tibiale postérieure ; elle surcroise la tubérosité calcanéenne et constitue une large arcade latérale avec l'artère tarsienne latérale, branche de l'artère dorsale du pied. La moitié de la circulation intraosseuse passe par l'ACL, médialement par des rameaux courts de l'artère plantaire latérale. Lorsqu'on examine les images obtenues en IRM lors de fractures récentes du calcanéus, on constate que l'ACL risque d'être lésée par l'élargissement latéral du calcanéus écrasé. Elle peut l'être aussi lors de l'abord latéral classique de l'os ou lors de l'application d'une plaque d'ostéosynthèse. Le risque en est la nécrose ischémique de l'os. Enfin, l'ACL peut constituer un pédicule vasculaire intéressant pour un lambeau local de rotation, lors de la couverture des pertes de substances cutanées du talon.

    Notes: Reactions with a heavy projectile incident on a light target can be used for the efficient in-flight production of secondary radioactive beams. An overview of this technique is given using data on 17F beams produced via the p(17O, 17F)n and d(16O, 17F)n reactions. With primary 16,17O beam currents of 100 pnA, intensities of up to 2×106 17F/s on target were achieved. Using this beam, the p(17F,α)14O reaction was measured. © 2000 American Institute of Physics.

    Description / Table of Contents: Zusammenfassung. In der Unfallchirurgischen Universitätsklinik Köln wurden von 1988 bis 1997 19 Patienten an einer Baker-Cyste operiert. Die subjektiven und objektiven Ergebnisse wurden mit Hilfe eines Fragebogens sowie einer klinischen und sonographischen Nachuntersuchung erfaßt (Rücklauf: 95 %). Der präoperative Status wurde durch klinische Untersuchung, Sonographie und Anfertigung von Röntgenaufnahmen des Kniegelenks ermittelt. Wir differenzierten zwischen der angeborenen primären Cyste (39 %) und der sekundären Form, die immer mit einer Kniebinnenpathologie assoziiert ist (61 %). Bei allen sekundären Cysten wurde eine Arthroskopie durchgeführt. Als postoperative Komplikationen traten in einem Fall ein revisionspflichtiges Hämatom und in einem anderen Fall ein punktionspflichtiger Erguß auf. Das Operationsergebnis wurde von 61 % der Patienten mit „sehr gut“ und von 39 % mit „gut“ bewertet. Alle nachuntersuchten Kniegelenke waren passiv und aktiv frei beweglich. In einem Fall kam es zu einem Rezidiv. Nach unserer klinischen Erfahrung sollten primäre Baker-Cysten immer entfernt werden. Bei der sekundären Cyste muß neben der Exstirpation die Bedeutung der arthroskopischen Diagnostik und Therapie sowie der Sanierung der Grundkrankheit diskutiert werden.
    Notes: Summary. Baker's cysts were treated operatively in 19 patients in the Department of Traumatology of the University of Cologne from 1988 to 1997. The subjective and objective results were evaluated with a questionnaire and a clinical examination and sonography (follow-up: 95 %). All patients were examinated before surgery, sonography and X-ray of the knee were performed. We differentiated between the congenital primary cyst (39 %), and the secondary form, which was always associated with an intraarticular lesion (61 %). Arthroscopy was performed in all secondary forms of Baker's cyst. Postoperative complications were two reinterventions due to one hematoma and one effusion. Patient's evaluation of operation result was “excellent” in 61 % and “good” in 39 % of cases. All knee joints had a full range of motion. There was only one case of a recurrent cyst. The primary form of Baker's cyst has always to be extirpated, according to our clinical experience. The extirpation of the secondary Baker's cyst and the relevance of arthroscopy and treatment of the basic disease have to be discussed.

    Description / Table of Contents: Zusammenfassung In zunehmendem Maße werden Candidaspezies Ursache für Infektionen des Menschen. Verschiedene Lokalisationen im Körper wurden in der Literatur beschrieben, aber nur wenige Fälle weisen eine Candidaspondylitis auf. Berichtet wird über eine Patientin mit einer Spondylitis der Lendenwirbelsäule und einer Candidakoxitis. Die Literaturübersicht umfaßt 31 Fälle von Candidaspondylitiden. Die Candidaspondylitis ist in den meisten Fällen eine Spätmanifestation einer Sepsis dieses Erregers. Die Spondylitis kann oft nicht durch die systemische Therapie mit Amphotericin B in der akuten Infektionsphase verhindert werden. Auch bei immunsupprimierten Patienten ist die Candidaspondylitis niemals Folge einer postoperativen Wundinfektion gewesen. Die lange Latenz zwischen Sepsis und Manifestation einer Candidaabsiedlung in der Wirbelsäule oder anderen großen Gelenken, wie der Hüfte, sowie der häufig falsch-negative Erregernachweis kann zu einer erheblichen Diagnose- und Therapieverzögerung führen. Die Diagnose kann durch eine offene Biopsie oder eine CT gesteuerte Punktion geschehen. Behandelt wird die Candidaspondylitis mit einer Kombination aus radikalem chirurgischen Debridement und einer medikamentösen antimykotisch Therapie (Amphothericin B oder Flukonazol). Chirurgisch wird eine ventrale und fakultativ dorsale Instumentation zur Stabilisierung der Wirbelsäule durchgeführt. Die Spondylitis wird in zunehmendem Maße Manifestation einer abgelaufenen Candidasepsis insbesondere bei immunsuppremierten Patienten sein.
    Notes: Summary Candida species have emerged as important pathogens in human infection. Although a variety of deep-seated candidal infections have been reported, Candida spondylitis has rarely been described. One patient with candida tropicalis spondylitis L I and L II in combination with candida coxitis is presented, and the 31 adult cases with vertebral involvement previously reported are reviewed. Candida spondylitis is noted as a simultaneous occurrence or late manifestation of hematogenously disseminated candidiasis. Spondylitis may not be prevented by a course of Amphotericin B adequate to control the acute episode of disseminated candidiasis, particularly in immune suppressed patients. Spondylitis does not present as a postoperative wound infection. The insidious progression of infection, the nonspecificity of laboratory data, and the failure to recognise Candida as a potential pathogen may lead to diagnostic delay. Diagnosis can be made by either open biopsy or CT controlled needle aspiration. Successful therapeutic regimes have employed combinations of antifungal therapy (Amphotericin B or fluconazole) with radical surgical debridement. Ventral and facultatively dorsal instumentation is required to stabilize the spine. It is anticipated that the spondylitis will become a more commonly recognised manifestation of hematogenously disseminated candidiasis. A increasing significance of candida species as etiologic agents of infection immune compromised humans has been recognised in the recent years. In those patients whom an antecedent Candida septicaemia was documented, a striking delay of 3.3 months was found between the septicaemia and the onset of symptoms as well as the time of diagnosis.

    Description / Table of Contents: Zusammenfassung Biodegradable Implantate bieten gegenüber metallischen Implantaten Vor- und Nachteile. Zahlreiche Anwendungsideen stellen sich dar. Darunter ist der Fixationsstift für gering belastete osteochondrale Fragmente am gründlichsten untersucht. Die primäre Festigkeit für Anwendungen am distalen Radius wurde vergleichbar zu herkömmlichen Implantaten wie der 1,8-mm-Kirschner-Draht gemessen. Eine prospektiv randomisierte Studie mit 183 Radiusköpfchenfrakturen zeigte gleichwertige Ergebnisse bei Metall und Polymer und insbesondre keine erhöhte Komplikationsrate für die Stifte aus Polylactid (Polypin®). Für diese Indikation und vergleichbare Lokalisationen ist eine Anwendung zu empfehlen. Die Gesamtbehandlungskosten können damit gesenkt werden, auch wenn das einzelne Implantat teurer ist. Hochfeste Polylactidschrauben haben in einer klinischen Langzeitstudie extrem lange Resorptionszeiten von 5–7 Jahren erbracht.
    Notes: Summary The application of biodegradable implants is combined with advantages and disadvan- tages. They offer a great deal of innovative ideas. Best evaluation has been done with pins for the fixation of low loaded osteochondral fractures. When using 2,7 mm Polypin® primary mechanical stabilility of distal radius fractures is comparable with the fixation by 1,8 mm K-wires. A prospective and randomised study with two years follow up, including 183 radial head fractures, showed no significant difference in results and complications in metal and polyactide implants. For this localisation and other comparable indications the implant can be recommended. Despite the more extensive degradable device overall costs can be reduced with the polymer implant. Orien- truded polyactide screws showed good clinical results, newetheless resorption was extremely late within 5 to 7 years.

    Description / Table of Contents: Zusammenfassung Der Begriff „okkulte Fraktur“ kennzeichnet das Fehlen sichtbarer Frakturzeichen in der zur Erstdiagnose durchgeführten konventionellen Radiographie bei dennoch vorhandener Fraktur. Die Diagnosestellung gelingt erst im Verlauf oder mittels weiterführender Diagnostik. Die hierzu durchgeführten Szintigraphie- und CT-(Computertomographie)Untersuchungen verfügen jedoch nur über eine mäßige Sensitivität und Spezifität. Mit der Magnetresonanztomographie (MRT) steht ein Verfahren zur Verfügung, das sowohl die Frühdiagnose der „okkulten Fraktur“ als auch die Diagnose etwaiger chondraler und ligamentärer Begleitverletzungen erlaubt. In der vorliegenden Studie berichten wir über 23 Patienten mit einer traumatischen Fraktur in 5 verschiedenen anatomischen Regionen, bei denen die Diagnose bei negativem Röntgenbefund mit der MRT gestellt werden konnte.
    Notes: Summary The term “occult fracture” defines the lack of visible fracture signs in conventional radiography although a fracture exists. The diagnosis is made in the sequel or through further diagnostic procedures. The use of scintigraphy and CT examinations is limited by their moderate sensitivity and specificity. In contrast, MRI is a diagnostic tool that allows early diagnosis of a fracture and of accompanying chondral or ligament damage. We report on 23 patients with traumatic fractures at 5 different anatomical sites, all of which were negative by x-ray, but the diagnosis could subsequently be made with MRI.
