    Notes: This randomised, single-blind study investigated the incidence of retching during emergence from general anaesthesia with a laryngeal mask airway in place. Eighty four patients, ASA grade 1 and 2, aged 15 to 60 years, were randomly divided into two groups. Each patient received fentanyl 1 μg.kg-1 and propofol 2mg.kg-1for induction. A laryngeal mask airway was then inserted and the patient breathed spontaneously on a nitrous oxide-oxygen mixture with isoflurane. Lubafax gel was the lubricant in the control group and 2% lignocaine gel was used in the test group. The patients were allowed to wake up at the end of surgery with the laryngeal mark airway in place while a blinded observer observed for retching. The age, sex, weight and duration of surgery were similar in both groups. The test group had a significantly lower incidence of retching on emergence from general anaesthesia with the laryngeal mask airway in place (p 〈 0.005, Chi-squared test).

    Notes: Summary The ultrastructural and biosynthetic characteristics of the steroid cells in the gonad of Monopterus albus have been studied. Ultrastructural features related to steroidogenesis have been identified in the interstitial Leydig cells, Sertoli cells, granulosa cells and thecal cells, and are especially abundant in the Leydig cells during the mid-intersexual phase. Steroidogenic ultrastructures in the Sertoli cells develop only during the maturation of the spermatogenic cysts, whereas in the granulosa and thecal cells, these features become obvious only during the maturation of the large oocytes. EM evidence also suggests a nutritive function for the Sertoli cells and the granulosa cells. Results of in vitro steroidogenic studies, using either testosterone or progesterone as a precursor, show a predominant conversion to androstenedione and 5α-reduced compounds, and suggest a change in biosynthesis from 5α-reduced products to androstenedione during sex reversal. 11-Ketotestosterone (11KT) has been identified, but not 11 β-hydroxytestosterone. Production of 11 KT is high in the late intersexual and the male phases, but a lack of a marked variation in 11KT production between the early and the mid-intersexual phase suggests that this steroid is not a trigger for natural sex reversal in Monopterus.

    Notes: Abstract Studies of thein vitro gonadal steroidogenesis in intersexual fishes, using labelled testosterone as precursor, showed large species variation. The protogynousMonopterus albus produced predominantly 5α-reduced metabolites while the protandrousRhabdosargus sarba produced mainly 5β-reduced products. Both fishes synthesized 11-oxotestosterone; the synthesis of which appeared to mediate mainly through adrenosterone inM. albus butvia 11β-hydroxytestosterone inR. sarba. When the plasma levels of androstenedione, testosterone, 11-oxotestosterone, 11β-hydroxytestosterone, estrone and 17β-estradiol among the male, intersexual and female phase of the same species were compared, available data showed that either there was no obvious difference among the different sexual phases or the differences could be accounted for by the seasonal reproductive activities of the animal. Except for androstenedione, there are no marked changes in plasma testosterone, 11-oxotestosterone, 11β-hydroxytestosterone, estrone and 17β-estradiol levels in the intersexual phase compared with the female and male, it is unlikely that these classical sex steroids act as a primary trigger of natural sex reversal in these fishes; the role of androstenedione awaits further elucidation.

    Notes: Abstract One hundred fourteen semen samples from Chinese males were analyzed for routine semen parameters including the semen volume, sperm count, percentage motility, and percentage normal morphology. Of these 114 samples, 54 also had movement characteristics of seminal and swim-up sperm evaluated by the computer image analyzer system (Cellsoft; Cryo Resources Co., New York). All semen samples were subjected to the swim-up procedure to harvest the motile sperm before inseminations of human oocytes. Fertilization was considered to have occurred when at least one oocyte was observed with two or more pronuclei. Semen samples were classified as infertile (0% fertilization rate;N=32) or fertile (〉0% fertilization rate;N=82) before statistical analyses. There was a significant difference (P〈0.005) in percentage normal morphology of seminal sperm between the fertile (mean±SE; 67.3±1.2%) and the infertile (59.3±2.2%) samples. The percentage normal morphology of seminal sperm correlated (r=0.3049;P〈0.002) with the fertilization rate and this parameter was selected by the multivariate stepwise discriminant analysis as the discriminator capable of predicting the fertilization rate with 57.9% accuracy. Statistical analyses of samples where sperm movement was also evaluated demonstrated that there was significant differences (P〈0.01) between the fertile (N=38) and the infertile (N=16) samples in percentage normal morphology of seminal sperm (67.8±1.8% vs 56.2±2.6%) and curvilinear velocity of swim-up sperm (89.2±3.5 vs 68.2±7.2 μm/sec). The fertilization rates correlated with the percentage normal morphology of seminal sperm (r=0.3868;P〈0.005) and velocity of swim-up sperm (r=0.3842;P〈0.005). Multivariate stepwise discriminant analysis demonstrated that these two sperm parameters in combination were capable of predicting the fertilization rate with 74.1% accuracy. Our results indicate that seminal sperm morphology, coupled with computerassisted image analysis of movement characteristics of swim-up sperm, can help to predict the outcome of in vitro fertilization of human oocytes.

    Notes: Abstract Fifty-eight treatment cycles in an in vitro fertilization/gamete intrafallopian transfer (IVF/GIFT) program were studied to compare the efficacy of two urinary methods, hemagglutination test (Higonavis) and enzyme immunoassay (Ovustick), in detection of spontaneous luteinizing hormone (LH) surge. If an isolated rise in urinary LH level was taken as indicative of LH surge, the false-positive rate was 36.7% for Higonavis and 10.2% for Ovustick. The difference was statistically significant (P〈0.001). If only a sustained rise in urinary LH was taken to indicate LH surge, the false-positive rate was 6.1% for Higonavis and 0% for Ovustick. In the seven cycles with a spontaneous plasma LH surge, there was a positive correlation between the plasma LH levels and the two urinary assay methods in six cycles (85.7%). Compared to plasma LH, there was a mean delay of 17.4 hr by the Higonavis test and 15.6 hr by the Ovustick test. If a sustained rise in urinary LH levels was taken as indicative of LH surge, both methods are quite accurate but the Ovustick appeared to be more specific.

    Notes: Enlargement of the ulnar nerve behind the medial epicondyle was noticed in museum and demonstration specimens and also in the dissection room cadavers. An investigation was carried out to study the incidence and character of the enlargement entirely from fresh autopsy cases, which included 400 ulnar nerves from 200 autopsy cadavers of Chinese subjects of all ages. The enlargement was graded into three categories  -  slight, moderate, and marked. Histological sections, 40 μthick and stained with H. & E., were examined. Twenty nerves from ten adult bodies were tested under stress for extensibility along the length of the nerve. Fortynine per cent of the ulnar nerves and 60.5% of cadavers of the total sample showed this enlargement, the incidence being greater in males than in females and slightly greater in the right limb than the left limb. Slight and moderate enlargement occurred in all ages; marked enlargement occurred only in ages over 35. The enlargement presented a normal histological picture; the enlargement was due to an increase of connective tissue present. The experimental results of extensibility along the length of the nerve showed the retro-epicondylar region, whether enlarged or not, to be more extensible than the rest of the nerve. While there seemed to be a tendency of a decrease of extensibility with enlargement, the correlation between extensibility and enlargement was insignificant statistically.
