Notes: Heisenberg spin exchange effects on the powder spectra of weakly perturbed triplet states have been studied by simulation techniques. From very small exchange frequencies on, the derivative powder spectrum is dominated by a sharp, essentially constant width, central line whose amplitude exhibits very close to a square-root dependence on the exchange frequency. The derivative powder edge lines are shown to behave according to the single crystal limiting exchange width equation and it is shown that the frequency dependence of the central dominating line provides yet another measure of exchange parameters in physical systems. Application of these methods to the (φ3AsCH+3) (TCNQ)−2 ion radical salt yields results in good agreement with those obtained by other methods.

Notes: Spontaneous movement following injection of propofol at induction was studied in 303 patients. Two hundred patients were Caucasians and 103 were Asians. In a pilot study carried out prior to the main study, 26% of the Asians moved at induction as compared with 6% of the Caucasians. The patients were studied in relation to a number of variables; age, sex, weight, height, race, smoker/non smoker, vegetarian/non vegetarian, alcohol consumption, premedication, use of fentanyl at induction and dose of propofol. When race was included as the sole variable there was a marginal but not significant difference between the two groups in terms of movement (p = 0.06). However, when the other recorded variables were taken into account, race was not included as a predictor of whether or not the patient moved. The best model for predicting whether the patients moved or not combines the variables weight and dose of propofol. Patients were more likely to move if they were lighter and the dose of propofol used at induction was higher.

Notes: One hundred patients undergoing breast lump excision using a standard anaesthetic technique in the Day Care Unit were randomly divided into five groups. Groups A and B received either saline or diclofenac in a double blinded, fashion before and at the end of the procedure, with both groups receiving 10 ml of 0.5% bupivacaine infiltration at the end. Groups C and D also received saline or diclofenac before and after surgery but had no bupivacaine infiltration at the end. Group E did not receive any injections but had bupivacaine infiltration at the end of surgery. In the postoperative period, pain was assessed by a visual analogue scale at 30 min intervals until discharge. All patients were requested to complete a pain relief questionnaire over the 48 h following surgery. There were highly significant differences between those who received bupivcaine and those who did not in the visual analogue scale scores at 30min (p 〈 0.001), 60 min (p 〈 0.001), 120 min postoperatively (p = 0.02) and at discharge (p = 0.03). Pain scores were lower in those who received bupivacaine and they were less likely to request rescue medication, although this did not reach significance (p = 0.07). There were significant differences between the groups who received bupivacaine and diclofenac injection and those who received bupivacaine alone, for visual analogue scale scores at 60 min following surgery (p = 0.05) and at 48 h (p = 0.002). Pain relief was better in those patients who received both bupivacaine and diclofenac injection. Although not significant (p = 0.22). fewer patients required rescue medication when diclofenac was given before surgery (10%) rather than after surgery (22.5%). Fewer patients had a fair amount or a great deal of pain in the 48 h following surgery when diclofenac was injected before (7.5%) rather than after surgery (12.5%). The mean number of oral analgesics taken in the 48 h after surgery was also lower in those patients who had the diclofenac before the surgery rather than after.

Notes: Changes in cardiac output were measured by transcutaneous aortovelography in 30 pregnant patients and in 30 control subjects with change of position from the supine. When compared to the supine position, the left and right lateral and left 15 till positions caused statistically significant increases in cardiac output, whereas the right 15 tilt position did not. Neither fetal head engagement nor the lime spent in each position had significant effects on the changes in cardiac output. It was not possible to identify a subgroup of pregnant patients who were particularly sensitive to changes in posture.

Notes: An 8-week survey was conducted to determine whether the introduction of low-flow anaesthesia (a fresh gas flow of 4 litres/minute or less) into routine use would be acceptable to members of a representative anaesthetic department and if the consequent reduction in use of volatile anaesthetics would result in financial savings. The hourly consumption of the volatile agents was measured during anaesthesia conducted using either conventional or low fresh gas flows. Anaesthetists' acceptance of low-flow anaesthesia was assessed using a questionnaire. Data were gathered on 286 patients undergoing inhalational anaesthesia for routine operative procedures. A 54.7% reduction in the consumption of isoflurane and a 55.9% reduction in that of enflurane was found. Of the 28 anaesthetists at the hospital, 21 would use low-flow anaesthesia routinely. The routine use of low-flow anaesthesia would therefore be acceptable and could result in annual savings of £26870 at Northwick Park Hospital.

Notes: A retrospective analysis has been undertaken of 53 operations in 42 patients with severe chronic obstructive airway disease. All patients had a forced expiratory volume in 1 second between 0.3 and 1 litre, but the outcome of surgery was successful after their first operations, all of which were elective; 38 of the 42 had uneventful anaesthesia and surgery together with a normal postoperative period, while four had artificial ventilation of the lungs. The best predictors of the use of postoperative ventilation were the arterial Po2 and whether the patient was dyspnoeic at rest.

Notes: The possibility of producing gynogenetic plants with pollen inactivated by irradiation was explored for the onion (Allium cepa L.). Two successive experiments included the use of cytoplasmic male sterility, recessive nuclear markers in the female plants and pollen irradiation of 150 Gy. Pollinations were achieved with the aid of flies and all seeds were harvested without the use of embryo rescue. Plantlets with recessive characters could be detected and they contained haploid and diploid cells. The development of these plantlets until flowering was observed under agronomic conditions. The use of doubled haploid (DH) plants in onion breeding is discussed.
