    Description / Table of Contents: Summary A 38-year-old male patient presented with circumscribed scleroderma (10×12 cm) in the left pectoral area. Hyperaemia of the skin in the area of the lilac ring (morphoea) was quantified by means of Laser-Doppler flux (LDF). At the lilac border of the morphoea LDF was increased to 420% of the reference value recorded for the unaffected skin of the contralateral side. Immediately after 13 days of antibiotic therapy (3×10 Mega penicillin G per day, i.v.) LDF normalized, though clinical changes were hardly apparent at this stage. By 6 months later, LDF was still in the normal range, and clinical signs, such as induration and hyperaemic redness, had also significantly improved. LDF measurements allow assessment of the therapeutic response to antibiotic therapy at a very early stage while the clinical situation is still unchanged.
    Notes: Zusammenfassung Wir berichten über einen 38jährigen Patienten mit zirkumskripter Sklerodermie (CS) und einem linkspektoralen Morphaea-Herd. Der Rückgang einer entzündlich gesteigerten Mehrdurchblutung im Areal des Lilac-Rings wurde durch wiederholte Messungen des Laser-Doppler-Flux (LDF) unter Therapie objektiviert. Unmittelbar vor Therapiebeginn sowie am letzten Behandlungstag und nach weiteren 6 Monaten wurde der Laser-Doppler-Flux an denselben Hautarealen im entzündlichen Randbereich, im Zentrum des Morphaea-Herdes sowie im Gesunden rechtspektoral bestimmt. Vor Antibiose fand sich im Lilac-Ring eine auf 420% – im Vergleich zum gesunden rechtspektoralen Referenzareal – gesteigerte Fluxmotion. Nach 13tägiger Therapie mit Penicillin G (3×10 Mega i.v.d) kam es zu einer Normalisierung der pathologisch erhöhten Meßwerte, während klinische Veränderungen zu diesem Zeitpunkt kaum erkennbar waren. Erst 6 Monate später – die LDF-Werte waren immer noch im Bereich der Vergleichswerte gesunder Haut – war klinisch eine deutliche Rückläufigkeit von Induration und Erythem sichtbar. Mit Laser-Doppler-Flux-Messungen kann frühzeitig, bevor klinisch eine Verbesserung diagnostiziert werden kann, das therapeutische Ansprechen nachgewiesen werden.

    Description / Table of Contents: Summary We report about a new type of physical therapy which can be used in patients with joint immobility secondary to by chronic venous insufficiency. Biomechanical stimulation therapy (BMS) uses mechanical vibration of standardised frequencies from 18–35 Hz spectrum to expose the feet and legs to longitudinal mechanical stimuli. Therapeutic benefit and clinical improvement can be achieved after a short period of treatment. We describe a 76 year old female patient suffering from both impaired motion and recurrent venous ulceration due to chronic venous insufficiency. After 10 days treatment with BMS, mobility of upper ankle joints improved by 16 degrees and 19 degrees and was accompained by healing of venous ulcerations after skin flap transplantation. Biomechanical stimulation methods were developed in the former Sowiet Union where they were used in sports medicine to improve relaxation of strained muscle structures and to increase the stretching ability of capsules and tendons. We have successfuly treated 6 patients with impaired mobility and chronic venous insufficiency. We believe that BMS is lialy to become a valuable therapeutic tool in patients with this problem in the near future.
    Notes: Zusammenfassung Vorgestellt wird anhand einer Kasuistik ein neuartiges physikalisches Therapieverfahren, das bei Patienten mit arthrogenem Stauungssyndrom infolge einer chronischen Venenerkrankung zum Einsatz kommen kann. Die biomechanische Stimulationstherapie (BMS) stellt dabei eine technische Erweiterung physiotherapeutischer Möglichkeiten durch den Einsatz kontrollierter mechanischer Schwingungen dar und führt zu raschen und klinisch günstigen Therapieerfolgen. Vom BMS-Gerät werden im Rahmen der Behandlung definierte longitudinale Schwingungen im Frequenzbereich zwischen 18 und 36 Hz auf Fuß und Unterschenkel übertragen und damit eine Relaxation der beteiligten Muskulatur sowie eine verbesserte Dehnbarkeit des Kapselbandapparates ermöglicht. Bei der vorgestellten 76jährigen Patientin mit langjährig vorbestehenden Bewegungseinschränkungen im oberen Sprunggelenk, Ulcera cruris venosa bei postthrombotischem Syndrom, konnte der Bewegungsumfang im Sprunggelenk durch eine 10tägige Behandlungsphase mit der biomechanischen Stimulationstherapie (BMS) im Sprunggelenk um je 16 Grad bzw. 19 Grad an beiden Beinen verbessert und der Erfolg dermatochirurgischer Maßnahmen bei der Ulcussanierung gesichert werden. Das BMS-Verfahren stammt aus der ehemaligen Sowjetunion. Hier wurde es im Leistungssportbereich zur Muskelrelaxation und Dehnung eingesetzt. Beim arthrogenen Stauungssyndrom wurde es von uns erstmals klinisch erfolgreich zum Einsatz gebracht. Aufgrund vorliegender Ergebnisse bei bislang 6 Patienten mit arthrogenem Stauungssyndrom bewerten wir das BMS-Verfahren als wichtige neuartige physikalische Therapieform des arthrogenen Stauungssyndroms.

    Description / Table of Contents: Summary   Graph drawing is a new and growing area in Computer Science. It is concerned with the design, analysis, implementation and evaluation of new algorithms for aesthetically nice drawings of graphs. Through the use of some selected examples of applications, typical problems, and solutions, we would like to provide an introduction into this still relatively unknown field. And we survey activities and goals of a working group consisting of members of the universities of Halle, Köln and Passau and the Max-Planck-Institut für Informatik in Saarbrücken, that is funded by the German Science Foundation DFG under the program „Efficient Algorithms for Discrete Problems and their Applications“.
    Notes: Zusammenfassung   Das Zeichnen von Graphen ist ein junges aufblühendes Gebiet der Informatik. Es befaßt sich mit Entwurf, Analyse, Implementierung und Evaluierung von neuen Algorithmen für ästhetisch schöne Zeichnungen von Graphen. Anhand von selektierten Anwendungsbeispielen, Problemstellungen und Lösungsansätzen wollen wir in dieses noch relativ unbekannte Gebiet einführen und gleichzeitig einen Überblick über die Aktivitäten und Ziele einer von der DFG im Rahmen des Schwerpunktprogramms „Effiziente Algorithmen für Diskrete Probleme und ihre Anwendungen“ geförderten Arbeitsgruppe aus Mitgliedern der Universitäten Halle, Köln und Passau und des Max-Planck-Instituts für Informatik in Saarbrücken geben.

    Notes: Objective To study the influence of compression on the haemodynamics o the deep venous system in pregnancy.Design A prospective, observational studySetting The phlebological unit of the department of dermatology at a university hospital.Population Fifteen pregnant women with no previous signs of chronic venous insufficiency.Methods Parameters of the venous pump function were assessed by strain-gauge plethysmography. Blood flow velocity, flow volume and vessel diameter in the superficial femoral vein were measured by duplex sonography. All examinations were performed with and without applied compression stockings (25–32 mmHg) at two different stages of gestation and after delivery. In addition, subjective symptoms were graded.Results Venous pump function improved and refilling time lengthened significantly when compression was used during gestation and postnatally. Duplex sonography showed an increase in blood flow velocity and flow volume in the superficial femoral vein with applied compression; the vessel diameter increased slightly. Subjective symptoms of the leg, graded on an arbitrary scale, were reduced by regular compression therapy.Conclusion Our results indicate that compression improves the clinical symptoms of venous congestion and the venous haemodynamics of the legs during pregnancy and in the postpartum period. Thus, the regular use of compression during pregnancy and the puerperium may reduce the incidence of thromboembolic events.

    Description / Table of Contents: Résumé Des progrès récents ont augmenté les possibilités de diagnostic et les moyens d'investigation vasculaire non invasifs. Ceux-ci comprennent la mesure de la pression partielle en oxygène à travers la peau, la débitométrie par laser Doppler, et la microscopie intravitale avec et sans utilisaton de colorants fluorescents, analysant l'hémodynamique de la peau et la morphologie microvasculaire, la vitesse sanguine capillaire, et l'échange transcapillaire de solutés. De plus, la duplex-sonographie offre la possibilité de détecter des sténoses dans les artères auparavant inaccessibles comme les artères mésentériques et rénales. Cette méthode, associant l'échographie mode B et l'échographie par Doppler pulsé convient également à dessiner la cartographie et à quantifier les lésions occlusives de l'aorte jusqu'aux vaisseaux à mi-mollet avec une précision comparable à celle de l'artériographie. Les implications cliniques de ces nouvelles méthodes sont discutées dans cette revue.
    Abstract: Resumen Recientes avances tecnológicos han incrementado la capacidad diagnóstica del laboratorio vascular. Los nuevos avances incluyen la medici'on de la presión parcial transcutánea de oxígeno, la flujometría con el Doppler de laser (que utiliza luz laser en vez de ultrasonido) y la microscopía intravital con y sin el uso de colorantes fluorescentes, para el análisis de la hemodinamia de la piel y de la morfología microvascular, de la velocidad del flujo capilar, y del intercambio transcapilar de solutos. Además, la escanografía duplex abre una nueva posibilidad para detectar áreas de estenosis en arterias hasta ahora inaccesibles, tales como troncos mesentéricos y renales; la combinación de la ultrasonografía de modo B y con los datos del Doppler hace posible delinear y cuantificar lesiones oclusivas desde la aorta hasta las arterias proximales de la pantorrilla con una certeza similar a la de la arteriografía. En esta revisión se discuten las diferentes implicaciones clínicas de estas novedosas técnicas.
    Notes: Abstract Recent advances in technology have further increased the diagnostic potential of the noninvasive vascular laboratory. They include measurement of transcutaneous oxygen pressure, laser Doppler flowmetry and intravital microscopy with and without the use of fluorescent dyes for analyzing hemodynamics of the skin and microvascular morphology, capillary flow velocity, and transcapillary exchange of solutes. Moreover, duplex scanning opens a way to detect stenoses in previously inaccessible arteries such as the mesenteric and renal trunks. The combination of B-mode and pulsed Doppler ultrasound is also suited to map and quantify occlusive lesions from the aorta down to the proximal calf arteries with an accuracy similar to arteriography. The clinical implication of these new techniques is discussed in this review article.

    Notes: Abstract The acyclic subgraph problem can be formulated as follows. Given a digraph with arc weights, find a set of arcs containing no directed cycle and having maximum total weight. We investigate this problem from a polyhedral point of view and determine several classes of facets for the associated acyclic subgraph polytope. We also show that the separation problem for the facet defining dicycle inequalities can be solved in polynomial time. This implies that the acyclic subgraph problem can be solved in polynomial time for weakly acyclic digraphs. This generalizes a result of Lucchesi for planar digraphs.

    Notes: Abstract LetD n be the complete digraph onn nodes, and letP LO n denote the convex hull of all incidence vectors of arc sets of linear orderings of the nodes ofD n (i.e. these are exactly the acyclic tournaments ofD n ). We show that various classes of inequalities define facets ofP LO n , e.g. the 3-dicycle inequalities, the simplek-fence inequalities and various Möbius ladder inequalities, and we discuss the use of these inequalities in cutting plane approaches to the triangulation problem of input-output matrices, i.e. the solution of permutation resp. linear ordering problems.

    Description / Table of Contents: Summary We report on a 70-year-old female patient with dermatomyositis. The connective tissue vascular bed was studied with capillaroscopy to evaluate morphological and functional changes of skin capillaries. Capillaroscopic findings showed morphological alterations such as rarefication of capillaries, avascular fields and tortuosity of nailfold capillaries. In addition, fluorescence-videomicroscopy showed pathologically increased transcapillary diffusion of dye in the apex region of nutritive skin capillaries. Increased capillary leakage probably contributed to the developement of oedema in our patient. Immunosupressive therapy combined with intensified manual lymphatic drainage was successful.
    Notes: Zusammenfassung Vorgestellt wird eine 70jährige Patientin mit Dermatomyositis. Zur weiteren Diagnostik krankheitsbedingter Veränderungen, die sich primär am gefäßhaltigen Bindegewebe abspielen, wurden kapillarmikroskopische Untersuchungstechniken eingesetzt, die eine Beurteilung sowohl funktioneller als auch morphologischer Veränderungen gestatten. Neben charakteristischen mikroangiopathischen Veränderungen in Form von Kapillarrarefizierungen, avaskulären Feldern und typischen knäuelartigen Verzweigungen fand sich in vivo ein umschrieben vermehrter, pathologischer Farbstoffaustritt für das i.v.-applizierte kleinmolekulare Natriumfluoreszein. Diese erhöhte Durchlässigkeit nutritiver Hautkapillaren weist auf eine pathogenetische Ursache der Ödementstehung hin. Unter konsequenter immunsuppressiver Therapie und manueller Lymphdrainage kam es zu einer raschen klinischen Besserung.

    Description / Table of Contents: Summary A 25-year-old carpenter with severe localised Raynaud syndrome of the lateral three fingers of his right hand is described. The patient reported digital ischaemia caused by cold or vibration, and especially when both were present simultaneously. He was initially diagnosed with thrombangitis obliterans, but failed to respond to intravenous prostaglandin therapy, pentoxifyllin or aspirin. Diagnostic procedures performed in our hospital, especially angiography, confirmed our suspicion of hypothenar hammer syndrome. Radiological examination revealed severe anatomical and functional changes of the distal ulnar artery and, digital arteries of the affected digits. The differential diagnosis of Raynaud syndrome should include the hypothenar hammer syndrome as a possible cause. Early diagnosis and prevention of hand trauma is the only possible waý to stop progression of this disease, which leads to invalidity.
    Notes: Zusammenfassung Vorgestellt wird ein 25jähriger Zimmermann mit schweren Raynaud-Attacken an den drei lateralen Fingern der rechten Hand unter Kälte und Vibrationen, bevorzugt auftretend unter einer Kombinationsbelastung aus Schwingungen und Kälte, wie z.B. bei der Arbeit an der Kreissäge im Winter. Die von uns durchgeführten Spezialuntersuchungen bestätigten den Verdacht auf ein Hypothenar-Hammer-Syndrom, nachdem zuvor eine Behandlung mit Prostavasininfusionen, Acetylsalicylsäure und Pentoxifyllin im Heimatkrankenhaus unter der Verdachtsdiagnose einer Thrombangiitis obliterans erfolglos verlaufen war. Angiographisch waren bereits fortgeschrittene Gefäßveränderungen die Digitalarterien und der A. ulnaris nachweisbar. Die sorgfältige differentialdiagnostische Abgrenzung gegenüber der Thrombangiitis obliterans und anderen, klinisch ähnlich imponierenden Krankheitsbildern mit lokalem Raynaud-Syndrom ist von entscheidender prognostischer Bedeutung. Angesichts der potentiell invalidisierenden Gefäßveränderungen muß auf eine Nichtverwendung der Hand als Hammer sowie eine Vermeidung von vibrierendem, handgeführtem Arbeitsgerät geachtet werden.

    Description / Table of Contents: Summary Across the boundaries of the medical specialties we have realized how important the concepts of quality of life and disease-related coping behavior are to understand the patients’ subjective perception of the medical condition and its treatment. Although standardized instruments are already available for different medical indications and even in the related fields of peripheral vascular and cardiac diseases, phlebology still lacks standardized concepts for evaluating quality of life and/or disease-related coping behavior in patients with chronic venous insufficiency (CVI). We report on a newly developed instrument specifically designed for recording quality of life in patients with CVI. It not only meets the requirements of psychometric standards, but has also proven its applicability in clinical use. The „Tübingen Questionnaire for measuring Quality of Life in patients with CVI (TLQ-CVI)” and the results of a study on quality of life in 142 patients with various stages of chronic venous insufficiency are presented. It was possible to distinguish between Stage I/II and Stage III CVI patients with respect to parameters such as „leg complaints” and „day-to-day fears and worries”. These convey clinically relevant insights into the patient’s subjective perception of the disease and how they cope. The information gathered provides a set of reasonable target scores for clinical studies presently being carried out at various study centers in Germany incorporating the TLQ-CVI.
    Notes: Zusammenfassung Dem Konzept der Lebensqualität und Krankheitsbewältigung wird in der medizinischen Disziplin fächerübergreifend eine zentrale Bedeutung insbesondere im Hinblick auf das subjektive Erleben der Erkrankung und eingesetzter Therapien durch die Patienten beigemessen. Während in benachbarten angiologischen und kardiologischen Fachdisziplinen bereits Erhebungsverfahren zur Lebensqualitätsforschung verfügbar sind, mangelt es in der Phlebologie derzeit noch an standardisierten Konzepten zur Evaluation der Lebensqualität und Krankheitsbewältigung bei Patienten mit einer chronischen Veneninsuffizienz (CVI). Die vorliegende Arbeit berichtet über ein neuentwickeltes krankheitsspezifisches Instrument zur Erfassung der Lebensqualität bei Patienten mit chronisch venöser Insuffizienz, das sowohl die Anforderungen der psychometrischen Testgütekriterien erfüllt als auch sehr praktikabel im klinischen Einsatz erscheint. Der „Tübinger Fragebogen zur Messung der Lebensqualität bei CVI-Patienten” (TLQ-CVI) sowie die Ergebnisse einer Untersuchung zur Lebensqualität von 142 Patienten mit chronisch venöser Insuffizienz unterschiedlicher Schweregrade wird erstmalig vorgestellt. Eine Unterscheidung von CVI-Patienten in den Stadien I/II vs. III war u.a. im Hinblick auf „Beinbeschwerden” und „Ängste und Sorgen im Alltag” möglich und vermittelt klinisch relevante Einsichten in die subjektive Patientensicht der Erkrankung und ihre Verarbeitung. Die Informationen aus dem TLQ-CVI stellen bereits jetzt sinnvolle Zielkriterien klinischer Prüfungen dar, die derzeit unter Einsatz des TLQ-CVI an verschiedenen Studienzentren in Deutschland durchgeführt werden.
