    Notizen: Abstract Results of argon laser treatment of pigment epithelial detachments and of subretinal neovascular membranes in Junius-Kuhnt's senile disciform macular degeneration have been compared in a prospective, randomized investigation with the natural course of these affections. Of several hundred cases 26 eyes met our criteria for treatment. In control periods ranging from 1 to 3 years, six eyes with pigment epithelial detachments received laser treatment and seven served as controls; seven eyes with subretinal neovascular membranes received laser treatment and six served as controls. The results indicate that impairment of vision progresses more rapidly after argon laser treatment than when no such treatment is given.

    Notizen: Abstract The scanning ultrastructure of the remnants of the lens left in the eye after extracapsular lens extraction was investigated in the rabbit. Extracapsular lens extraction was performed in 25 eyes and the development of after-cataract followed by biomicroscopic examination. After survival times varying between 1 week and 12 months, the eyes were enucleated and the rings of Soemmerring treated for light microscopy and transmission and scanning electron microscopy. Soemmerring's ring consisted of the fused remnants of the dissected anterior and posterior lens capsule, enclosing the equatorial part of the former lens, left behind after the operation. The anterior capsule and, to a lesser extent, also the posterior capsule were multilayered and appeared to be thickened. While the remnant of the anterior capsule was lined by a monolayer of epithelial cells, the posterior part of the capsule was only partly lined by irregularly arranged epithelial cells. All epithelial cells were highly vacuolized. In transection the interior part of the ring consisted of normal fibers, irregularly oriented and irregularly shaped fibers, degenerated fibers, and globular amorphous masses. Many of the normal fibers contained cell nuclei. At the equator and at the posterior side of the fusing anterior and posterior capsule as well, the fiber organization resembled the lens bow region of normal lenses. Frequently, islands of epithelial cells were observed in the center of the ring. The vitreal face of the posterior capsule in the center of the ring (in the optic axis of the eye) seemed to be unchanged and on its pupillary surface, fibers of different size as well as fibroblastlike cells were found. However, clear-cut Elschnig's pearls were absent. Our results are compared with the observations summarized in the literature. It can be concluded that the epithelial cells in Soemmerring's ring retain their capacity for division and differentiation. The newly formed fibers seem to be pushed to the center of the ring and to degenerate.

    Notizen: Abstract In this paper the Rotterdam Elderly Study is presented. The aim of the study is to investigate determinants of disease occurrence and progression in the elderly. In addition to contributing to our understanding of the etiology of geriatric illnesses, the study is expected to lead to specific recommendations for intervention. The study focusses on causally related determinants of major diseases in the elderly. Fields of interest for the Rotterdam Elderly Study are- conditions which interfere the most with the quality of life for the elderly. The aims of the Rotterdam Elderly Study are: (1) To investigate, by means of epidemiologic, clinical and basic research, the determinants of diseases in order to assess their etiologic significance. (2) To investigate potentially modifiable determinants in order to be able to develop preventive strategies by providing specific recommendations for intervention studies. The Rotterdam Elderly Study focusses on four primary areas of research: neurogeriatric diseases, cardiovascular diseases, locomotor diseases and ophthalmologic diseases. It is a prospective follow-up study, in which determinants of disease and determintants of progression of disease will be investigated in the total population of 55 years or over of the district of Ommoord in Rotterdam. It is anticipated that about 1–0,000 people will participate in the study and they will be examined in the period of 1991 to 1995.

    Notizen: Abstract The clinical course of endophthalmitis, presumably due to Candida albicans, in nine eyes of heroin addicts is described. In six of the seven eyes in which pars plana vitrectomy was performed the diagnosis was confirmed by the cultivation of Candida albicans from the rinsing fluid. Both purely intraocular treatment and a combination of local and systemic antimycotic therapy caused the intraocular infection to disappear. In this small group of patients the results were most favourable when treatment was started early.

    Notizen: Abstract An analysis was made of 1309 patients with uveitis seen at University Hospitals participating in the Uveitis Centre of the Netherlands Ophthalmic Research Institute. In this series B27-associated anterior uveitis was the most frequent entity (18%) followed by Toxoplasma chorioretinitis (7%). Sarcoid uveitis, pars planitis and Fuchs' heterochromic cyclitis each accounted for approximately 4–5% of cases. In 44% of the patients no specific diagnosis could be made. Central diagnosis registration is of great importance when conducting clinical uveitis research.

    Notizen: Abstract We performed a prospective multicentre study to evaluate the efficacy of therapeutic strategies currently used for ocular toxoplasmosis in a large number of patients (n=106). Treatment was given for at least four weeks and consisted of three triple drug combinations: group 1, pyrimethamine, sulphadiazine and corticosteroids (n=29); group 2. clindamycin, sulphadiazine and corticosteroids (n=37); and group 3. cotrimoxazole (trimethoprim and sulphamethoxazole) and corticosteroids (n=8). Patients with peripheral retinal lesions remained without systemic therapy (group 4, n=32). Patients from group 1 received leucovorin 5 mg twice a week. No difference in the duration of inflammatory activity was observed between the treated and untreated patients or between the separate groups of patients. The most important factor predicting the duration of inflammatory activity was the size of the retinal focus itself, independently of the therapy given (P〈0.05). We showed a reduction in size of the retinal inflammatory focus in 52% of the pyrimethamine patients as compared to 25% of untreated cases. However the most frequent side effects were also associated with pyrimethamine medication and included hematologic complications as thrombocytopenia and leucopenia despite leucovorin medication.
