Summary
Iron and protein contents from lungs in a case of pulmonary hemosiderosis combined with glomerulonephritis were examined by biochemical, electrophoretic, immunoelectrophoretic techniques and gel filtration. Total organ iron was found to be 80 times higher than normal. No abnormal avidity of the lungs for iron salts could be seen. The qualitative determination of total organ iron yielded 98% insoluble iron, 1% heme-iron and 10% ferritin-iron.
No other soluble protein fractions could be found. The behaviour of ferritin from lungs in the presence of Good-pasture syndrome in gel filtration and electrophoresis corresponded to normal ferritin. Because of the low ferritin content we searched for a mechanism to explain this concentration of ferritin. After addition of ferritin and after iron dextran gel filtration, it was found that iron-containing particles of lower molecular size than ferritin are produced. The formation of albumin polymers, probably with reversible iron binding, must also be considered.
The presence of the low ferritin content and of a high content of insoluble iron could be caused by a mechanism limiting ferritin concentration. The protein spectrum was investigated qualitatively and semi-quantitatively by means of immuno-electrophoresis.
Zusammenfassung
Es wird über chemische, säulenchromatographische, elektrophoretische und immunelektrophoretische Untersuchungen der Lunge bei einem Fall von Goodpasture-Syndrom und anderen Formen der Lungensiderose berichtet.
Dabei wurde ein 80fach erhöhter Gesamtorganeisengehalt festgestellt. Eine erhöhte Avidität des Lungengewebes für Eisensalze konnte nicht nachgewiesen werden. Die Differenzierung des Eisens ergab in 89% unlösliches Eisen (z. B. Hämosiderin), in 1% Hämeisen und in 10% Ferritineisen. Neben diesem Ferritin, das in seinem säulenchromatographischen und elektrophoretischen Verhalten keinen Unterschied zu Ferritin aus Leber und Milz zeigte, waren keine weiteren löslichen Eiseneiweißfraktionen nachweisbar. Im wäßrigen Lungenextrakt bei Goodpasture-Syndrome kann Ferritin eine gegenüber anderen Formen der Lungehämosiderose und normaler Lunge auffallend niedrige Konzentrationsgrenze nicht überschreiten. Oberhalb dieser Grenze wird Ferritin durch einen nicht näher bekannten Mechanismus offenbar in kleinere eisenhaltige Fraktionen gespalten, die nach Umformung in unlösliches Eisen irreversibel abgelagert werden. Aufgrund der immunelektrophoretischen Ergebnisse darf die Bildung eines polymeren Albumins mit reversibler Eisenbindung, womit eine zusätzliche Eiseneinschleusung in die Lungen möglich erscheint, angenommen werden. Der konzentrationsabhängige Mechanismus wird als Ursache des einerseits geringen Ferritingehaltes und andererseits hohen Gehaltes an nicht löslichem Eisen erwogen und mit den bisher bekannten Befunden diskutiert.
