Summary
Heavy lysozymuria (values up to 790 µg/ml) has been observed in 5 of 7 consecutive cases of chronic myelocytic leukemia. In 4 cases (secondary) lysozymuria occurred when induction of remission was achieved after X-ray irradiation directed against the enlarged spleen. In one case (primary) lysozymuria was found prior to X-ray therapy, but irradiation evoked an increasing lysozyme response. Following the first course of X-ray therapy, in doses of 4–5×100 r, a rapid drop in the number of peripheral leukocytes was observed and, simultaneously, an elevation of urinary lysozyme activity, which was still elevated after 45 days. With the development of prolonged remission return of the enzyme level to normal was noted.
High urinary lysozyme levels in these patients are due to an increased turnover of the granulocytic cell series. Heavy lysozymuria in patients with chronic myelocytic leukemia obviously reflects the increased lysosomal breakdown induced by the X-ray treatment of the granulocytic cells and their precursors. Thus the degree of lysozymuria mirrors the therapy response in patients with this disease. Lysozymuria is independent of the absence or presence of the Philadelphia chromosome.
Zusammenfassung
Bei 5 von 7 Fällen mit chronischer myeloischer Leukämie (1 Fall mit beginnendem Blastenschub) kam es unter Milzbestrahlung zu massiver Lysozymurie (mit Aktivitäten bis zu 790 µg/ml). Peripherer Zellabfall und Lysozymaktivitäts-Anstieg einerseits, Lysozym-Aktivitätsabfall und Beginn der klinisch-hämatologischen Besserung andercrseits sind zeitlich korreliert. Die Therapieansprechbarkeit hat ihren typischen Ausdruck im Lysozym-Verhalten. Die Lysozymaktivität ist ein Maß für den Zellumsatz der leukämischen Zellen. (Lysozym wird beim Zerfall lysozymhaltiger neutrophiler Granulozyten bis Promyelocyten frei). Die Bedeutung der Lysozymbestimmung für die Verlaufs-beurteilung der chronischen Myelosen ist damit erwiesen. Die Lysozym-Aktivitätsbestimmung ist bei gleichzeitiger Allopurinol-Behandlung anstelle der Harnsäurebestimmung einzusetzen. In Fällen mit primärer Lysozymurie sollte die Lysozymbestimmung eine Rezidiv-Früherkennung ermöglichen. Es besteht keine Beziehung zwischen cytogenetischem Typ (Philadelphia-Chromosom) und An-oder Abwesenheit der sekundären (durch Milzbestrahlung hervorgerufenen) Lysozymurie und deren Höhe. Die chronische Myelose dürfte wegen der Möglichkeit der leichten Gewinnung größerer Mengen individueller Lysozyme für das Verständnis der Genetik, Struktur und Funktion des Enzymes Lysozym Bedcutung erlangen. Die Lysozymbestimmung sollte in der klinischen Hämatologie einen vorangigen Platz einnehmen.
