Untersuchungen zur circadianen Rhythmik der Glucosetoleranz

Diurnal variation of glucose tolerance and insulin secretion in man

Summary

The present investigation was designed to determine whether the circadian rhythm of glucose utilisation is based on a quantitatively different release of insulin and/or on changing secretory dynamics of the hormone. 22 healthy men received three oral glucose tolerance tests in the morning, afternoon and evening of the same day. The blood sugar levels in the afternoon and evening tests were significantly higher than those found in the morning tests.

Also the plasma insulin response was higher in the afternoon than in the evening, but it showed a delayed rise and late peak response.

In the evening the normal subjects responded as mild diabeties.

Zusammenfassung

Es wurde geprüft, ob die circadiane Rhythmik der Glucoseverwertung auf einer quantitativ unterschiedlichen Insulinfreisetzung und/oder einer veränderten Sekretionsdynamik des Hormons beruht. Die Untersuchungen erfolgten an insgesamt 22 gesunden männlichen Probanden im Alter zwischen 18 und 25 Jahren. Die Versuchsteilnehmer erhielten am gleichen Tage morgens um 7, mittags um 12 und abends um 17 Uhr Glucose in Form von Dextro-OGT®. 17 Personen wurden mit 100 g, 14 mit 50 g und 9 sowohl mit 100 als auch mit 50 g belastet. Venenblut zur enzymatischen Blutzucker- und zur radioimmunologischen Insulin(IMI)-Bestimmung wurde jeweils zu Beginn sowie 30, 60, 90 und 120 min nach Glucosegabe entnommen. Es zeigte sich, daß die Glucoseverwertung in den Abendstunden deutlich gestört war. Eine verminderte Bereitstellung von Insulin konnte als Ursache der Toleranzstörung ausgeschlossen werden. Jedoch war die Sekretionsdynamik in Form einer protrahiert einsetzenden Insulinfreisetzung in den Abendstunden erheblich gestört.
