124. Hüftpfannenbrüche

Summary

About 40% of all fractures of the pelvis concern the acetabulum. Judet and Letournel's classification of acetabular fractures is based on exact X-rays from three different views (AP, Obturator, and Ala). Treatment can be conservative with longitudinal and/or lateraltraction or operative with an anterior, posterior, or lateral approach. After exact reconstruction of the acetabular socket by means of screw and plate fixation, functional stability is obtained. Examples are given of conservatively and operatively treated acetabular fractures; we also report the long-term results of our own cases (n = 117) and a multicenter study of ten AO clinics (n = 190).

Zusammenfassung

Etwa 40 % aller Beckenfrakturen entfallen auf den Bereich der Hüftgelenkspfanne. Judet und Letournel haben aufgrund einer exakten röntgen-diagnostischen Zuordnung verschiedene Frakturtypen unterteilt. Die Behandlung kann konservativ durch Längs- und/oder Seitenzug oder operativ durch einen hinteren, vorderen oder seitlichen Zugang erfolgen. Nach exakter Rekonstruktion der Hüftgelenkspfanne wird durch Schrauben- oder Plattenosteosynthese funktionsstabil fixiert. Beispiele konservativer und operativer Behand lungsergebnisse sowie Langzeitkontrollen des eigenen Krankengutes und einer multizentri schen Sammelstudie werden mitgeteilt.
