Summary
In 70–75% of cases, bleeding from ruptured esophageal varices can be controlled by means of Sengstaken-Blakemore tubes, while shock is treated at the same time. Preventive measures to avoid hepatic-failure coma are initiated at the same time. After successful primary treatment, an emergency porto-systemic shunt operation should be performed. Many palliative operations have been proposed in the past, so that it is quite difficult to select the one most suitable for the individual patient. For best results we suggest a programmed, mainly standardized, but also flexible procedure. In cases where conservative management has not arrested the massive bleeding we have found transthoracic esophagus dissection to be the method of choice.
