117. Die cancerologisch korrekte Operation des Nephroblastoms

Summary

The following approach is suited to increase the radicality and the vascular control of the transperitoneal operation for nephroblastoma, reducing the need for postoperative irradiation:

• 1. Incision of the plica duodenojejunalis, which facilitates direct exposure of v. cava, aorta, and the renal vascular croB.

• 2. Systematic retroperitoneal enbloc lymph diBection from the superior mesenteric artery down to the aortic bifurcation, when neceBary, with removal of the adrenals and adnexae

• 3. Primary ligature of the renal artery prior to the renal vein, easy on both sides.

• 4. Thereafter laterocolic removal of the diseased kidney on the left, mediocolic removal of the right tumor-bearing kidney after mobilization of the colon ascendens.

ZusammenfaBung

Folgende Methodik ist geeignet, die Radikalität und GefäBsicherheit der transperitonealen Operation des Nephroblastoms mehrfach zu erhöhen und zugleich die Nachbestrahlung x zu erübrigen. a) Primäre Freilegung des renalen GefäBkreuzes, der V. cava und d. Aorta mittels Incision der Plica duodeno jejunalis samt Mobilisierung des Duodenums nach rechts, des Mesocolons nach links. b) Primäre Ligatur der A. renalis vor der V. renalis, beiderseits unproblematisch. c) Systematische retroperitoneale en bloc-LymphdiBektion von der A. mesenterica superior bis zur Aortengabelung, seitlich bis zum Zwerchfell, probatorisch an den Adnexen. d) Nachfolgende laterocolische Auslösung der linkBeitigen Tumorniere, mediocolisch der rechtBeitigen nach Mobilisierung des Coecums.
