164. Rekonstruktive Gefäß-Chirurgie ohne Antibiotiea?

Summary

43 disturbances of wound healing were seen in 207 reconstructive procedures of the arteries without prophylactic application of antibiotics (1/73–XII/77). Only 56% of these were caused by bacteria. In 6 patients a deep infection affecting the vessels was seen, two of these died. In 24 cases of infection in which the kind of germ was analyzed 19 patients were found with monoinfection, only 5 with mixed infection. In more than 70 % the infection was located in the groin. A postoperative temperature increase up to 38.5'C was seen in 50 % of the cases with thrombendarterectomy. Recovery of these patients was uneventful without application of antibiotics.

Zusammenfassung

Von 408 gefäß-chirurgischen Eingriffen im Zeitraum I/73 bis XII/77 werden bei 207 rekonstruktiven Operationen ohne Antibiotica-Prophylaxe 43 Wundheilungsstörungen beobachtet, von denen nur 56 % bakteriell bedingt sind. 6mal trat die tiefe Infektion mit Gefäßbeteiligung auf, davon sind 2 Patienten verstorben. Bei 24 Infektionen mit Erregernachweis handelte es sich bei 19 Patienten um Mono-Infektionen, 5mal um eine Mischinfektion. Die Leiste war von der Infektion in über 70 % der Fälle betroffen. Bei der Hälfte der thrombendarteriektomierten Fälle kam es postoperativ zum Temperaturanstieg bis 38,5°C; ohne Einsatz von Antibiotica erfolgte ein komplikationsloser Heilungsverlauf.
