72. Die Behandlung der Luxationstrümmerfrakturen des Humeruskopfes mit der Kopfkalottenrefixation

Summary

Because the fractures of the head of the humerus were irreducible in seven cases, refixation of the greatest fragment was performed. All patients were re-examined 1–4 years post-operatively. The subjective result and range of motion after refixation of the largest fragment and the rotator cuff were obviously better than after resection, although we always found partial or complete avascular necrosis of the fragment. It can be concluded that in all cases of comminuted and luxated fractures of the head of the humerus where a closed manipulation has failed, one should try refixation of the largest fragment of the head together with the rotator cuff to the shaft of the humerus.

Zusammenfassung

7 irreponible Luxationstrümmerfrakturen des Humeruskopfes wurden in den letzten 4 Jahren durch Refixation des größten Kopffragmentes behandelt. Das subjektive Ergebnis und das Bewegungsausmaß war nach Refixation des größten Kopffragmentes mit der Rotatorenmanschette deutlich besser als nach Humeruskopfresektion, obwohl immer eine partielle oder totale Kopffragmentnekrose eintrat. Es kann gefolgert werden, daß bei konservativ irreponiblen Luxationstrümmerfrakturen des Humeruskopfes immer die Refixation des größten Kopffragmentes zusammen mit der Rotatorenmanschette an den angeschrägten Humerusschaft versucht werden sollte.
