Summary
Complications resulting from wrong incision lines in the breast area are reported. Radial incisions leave bad scars and cause unwanted tractions on the areola. With regional excisions, mamma reduction plastics, gynaecomasties and ablations the different advantages of the circular incisions, incision lines in the submammary fold, the transmamillary cut and the vertical incisions are discussed. Prior to every mamma ablation there should be consultation with the plastic surgeon regarding later breast reconstruction. Most important for the postoperative result is the intraoperative handling of the skin as well as the atraumatic closure of the wound.
Zusammenfassung
Es wird über die Komplikationen bei falschen Schnittführungen im Bereich der Brust berichtet. Radiäre Schnitte führen zu häßlichen Narben und auffälligen Verziehungen der Brustwarzen, vornehmlich bei Anheben der Arme. Bei Probeexcisionen, Reduktionsplastiken, Gynäkomastien und Mammaablatio werden die verschiedenen Vorteile der zirkulären Schnitte, der Schnittführungen im Bereich der Umschlagfalten, der transmamillären Schnitte und der schrägen Schnitte diskutiert. Vor der Mammaablatio wird ein Konsil mit dem Plastischen Chirurgen empfohlen, der die Brustrekonstruktion plant. Ausschlaggebend für das postoperative Ergebnis ist eine schonende Wundrandbehandlung sowie ein atraumatischer Wundverschluß.
