302. Zur chirurgischen Therapie von Stenosen und Verschlußprozessen im Bereich des cervicalen Abschnittes der A. vertebralis

Summary

Of 52 reconstructive operations of the vertebral artery (until 31 March 1986), 6 were situated in the cervical vertebral foramina. In 73 % of cases we found stenosis, while 27 were caused by kinking. We improved the diagnostic reliability and the assessment of the indications for surgery by injecting a bolus of technetium, and examining the quantitative differences between both sides and the hemodynamic changes. In cases of occlusion of the cervical part of the vertebral artery, a subclavian-vertebral bypass was implanted without causing complications. The longterm postoperative examination of 48 patients revealed recurrent stenosis in 4 and complete occlusion in 3 patients. After 3 years we observed neurological deficits in 3 patients. The reconstruction of high vertebral artery stenosis was without complications.

Zusammenfassung

Von 52 Rekonstruktionen der A. vertebralis (bis 31. 3. 1986) waren 6 im cervicalen Knochenkanal lokalisiert. In 73 % lag eine Stenose und in 27 % eine Kink-king vor. Zur weiteren Sicherung der Diagnostik und der Operationsindikation wurde der Einsatz von Dilutionssonden nach Applikation eines Technetiumbolus überprüft. Patienten mit Verschlußprozessen im cervicalen Abschnitt wurde einsubclavio-vertebraler Venenbypass komplikationslos angelegt. Die Nachuntersuchung von 48 Patienten ergab 3 Verschlüsse und 4 Stenoserezidive.In 3 Fällen wurde nach 3 Jahren ein neurologisches Defizit beobachtet. Die Rekonstruktion der hohen Verschlußprozesse waren komplikationslos.
