Zusammenfassung
Es wird über 21 Fälle von Infektionen berichtet, die über einen Zeitraum von durchschnittlich fünf Tagen mit 3 × 3,0 g Cefamandol pro 24 Stunden behandelt wurden. Die behandelten Infektionen verteilen sich wie folgt auf Weichteilinfektionen an Extremitäten, Phlegmonen im Gesichts- und Kopfbereich, Peritonitis und auf intra- und retroperitoneale sowie auf subkutane und subfasziale Abszesse. Es fand sich dabei ein breites Cefamandolempfindliches Keimspektrum. Übereinstimmend kam es unter der Behandlung zu einem ungewöhnlich raschen Abklingen der Entzündungszeichen, meist schon in den ersten 24 Stunden. Zwei Patienten mit akuter Peritonitis verstarben trotz Therapie. Therapiebedingte Nebenwirkungen wurden nicht beobachtet. Aufgrund der gemachten Erfahrungen stellt sich die Frage, ob eine hochdosierte Kurztherapie nicht ebenso effektiv und andererseits rationeller ist als eine länger dauernde mit niedrigen Dosen.
Summary
This paper reports on 21 cases of infections treated with 3.0 gm. of cefamandole three times a day for an average of five days. Infections treated included soft tissue infections of the extremities, phlegmons of the face and head, peritonitis, intraperitoneal and retroperitoneal as well as subcutaneous and subfascial abscesses. There was a broad spectrum of cefamandol-sensitive organisms. The inflammatory signs consistently subsided very rapidly during therapy, in most cases within the first 24 hours. Two patients with acute peritonitis died despite therapy. No drug-related adverse reactions were noted. On the basis of this experience, the question arises whether a high-dosage short-term therapy might be just as effective as and, at the same time, more expedient than a longer-term therapy with low doses.
