Summary
The reported incidence of blood vessel deformities of the spinal cord varies among different authors. This discrepancy is to be partly attributed to erroneous diagnoses. In our own cases and those of the literature we found a significant predominance of males in their fifties, but all other age groups may be affected. Half of the patients showed an acute beginning with pain arising from nerve roots, followed by pareses of the lower limbs. Many cases began in terms of a “dysbasia intermittens medullae spinalis” (Foix-Alajouanine) or with disturbed sensibility. Rarely, a bladder disturbance was the first symptom. In 30% of all cases a gradually progressive course could be seen. In the fully developed cases motor and sensory as well as vegetative disturbances are present at about the same rate.
A special group is represented by the pediatric cases with 70% taking an acute course.
Another group develops symptoms during pregnancy with post partum remissions. As for diagnosis we relied especially on CSF-findings. Only 9.2% of the cases showed a total protein value of less than 40 mg%. The protein values are frequently variable and may even display a tendency to drop.
There is a dissociation between cisternal and lumbar CSF and a “dissociation albuminocytologique” as classically described by Foix and Alajouanine; furthermore, a slightly increased cell count with alterations in the differential cell count such as reported by Sayk; immature cell forms also appear. In 50% of all cases pathological colloidal reactions with a tendency to the left are present. In 10%, there is xanthochromia. There is also a dissociation between the findings on Queckenstedt's test and myelography. Reference is made to functional examinations during measurements of CSF-pressure, and to myelographic findings. The necessity for laminectomy and a combined surgical-medical treatment to be administered is explained on physiopathological grounds.
Zusammenfassung
Die Gefäßmißbildungen des Rückenmarks werden in ihrer Häufigkeit von den einzelnen Autoren oft unterschiedlich angegeben. Dieser Unterschied ist zum Teil auf Fehldiagnosen zurückzuführen. Sowohl bei unseren eigenen Fällen als auch den Fällen der Literatur fanden wir ein eindeutiges Überwiegen des männlichen Geschlechtes im 5. Lebensjahrzehnt, doch auch alle übrigen Jahrgänge können befallen werden.
Die Hälfte der Patienten zeigte einen akuten Beginn mit radikulären Schmerzen und nachfolgenden Paresen unterer Extremitäten. Viele Fälle begannen im Sinne einer Dysbasia intermittens medullae spinalis (Foix u. Alajouanine) oder Gefühlsstörungen. Selten war eine Blasenstörung Erstsymptom. In 30% aller Fälle konnte ein schubweiser Verlauf fest-gestellt werden. Bei voll ausgebildetem Krankheitsbild sind motorische, sensible und vegetative Störungen in etwa gleicher Häufigkeit ausgeprägt.
Eine besondere Art stellen die kindlichen Fälle dar, die zu 70% einen akuten Verlauf zeigen.
Als weitere Gruppe wurden die Erkrankungsfälle bei einer bestehenden Schwangerschaft sowie Remissionen post partum beschrieben. Diagnostisch wurde von uns der Liquorbefund besonders herausgestellt. Nur 9,2% der Fälle zeigten einen Eiweißwert unter 40 mg%. Die Eiweißwerte können häufig wechseln und auch eine sinkende Tendenz zeigen. Es besteht eine Dissoziation zwischen zisternalem und lumbalem Liquor, eine dissociation albuminocytologique, wie sie klassisch von Foix u. Alajouanine beschrieben wurde, eine leichte Zellzahlerhöhung mit Veränderungen des Differentialzellbildes nach Sayk und Auftreten von Reizformen. In 50% aller Fälle kommt es zu einer pathologischen Linkszacke in der Normomastixkurve und pathologischem Ausfall der Salzsäure-kollargolreaktion, in 10% liegt Xanthochromie vor. Es besteht eine Dissoziation zwischen Queckenstedtschem Versuch und myelographischem Befund. Auf Funktionsproben bei der Liquordruckmessung wurde eingegangen, ebenso wie auf die myelographischen Verfahren. Die Not-wendigkeit der Laminektomie und einer kombinierten chirurgisch-medizinischen Behandlung wurde begründet.
