Zusammenfassung
Im Serum einer gesunden Mutter wurden homologe, leukocytäre Isoantikörper festgestellt mit einem Agglutinationstiter von 1/32 – 1/256 und einem Immunadhärenztiter von 1/2. Absorptionsstudien ergaben ein Antikörpergemisch, das mit 97% unausgewählter Leukocytenspender reagierte und in der γ G-Fraktion lokalisiert war.
Die Antikörper wurden durch Plasmapherese als homologes Antileukocytenplasma (HALP) gewonnen und ihre Wirkung an 25 Freiwilligen, 3 Patienten mit lymphatischer und 1 Patient mit myeloischer Leukämie untersucht.
Während 5 ml HALP in zwei Drittel der Fälle pyrogene Reaktionen verursachten, wurden gleiche und größere Mengen HALP in den darauf folgenden Tagen von den gleichen Personen meist symptomlos vertragen. Die Bedeutung dieses Phänomens für die Bluttransfusion wird diskutiert.
5 ml HALP erzeugten bei 17 Freiwilligen vereinzelt geringgradige Leukopenien, in der 3. Stunde post injectionem jedoch Granulocytosen bis zu 300%. Unter wiederholter HALP-Gabe nahmen die Granulocytosen, an denen sich die eosinophilen Granulocyten nicht beteiligten, ab oder verschwanden ganz.
100–200 ml HALP führten zu einem Leukocytensturz auf rund 15% bei 7 Freiwilligen, worauf die Granulocyten nach der 1. Std, die Lymphocyten nach 6 Std den Ausgangswert wieder erreichten. Die gleiche Dosis HALP verursachte bei Patienten mit chronisch lymphatischer Leukämie eine wesentlich stärkere Lymphopenie als bei Gesunden.
Eine Kreuzreaktion mit51Cr markierten Erythrocyten und hohen Dosen von Leukocytenisoantikörpern konnte nicht nachgewiesen werden.
Summary
Plasma containing homologous leucocytic antibodies (HALP) was obtained from a healthy mother by plasmapheresis. The antibodies had an agglutination titre of 1/32–1/256 and an immuneadherance titre of 1/2. Absorption studies revealed a mixture of antibodies in the γ G fraction reacting with 97% of different leucocyte donors.
HALP was studied in 25 volunteers, 3 patients with chronic lymphatic, 1 patient with chronic myelogenic leukemia.
5 ml HALP caused pyrogenic reactions in 2/3 of the persons tested, wereas the same or higher doses of HALP were almost always well tolerated when HALP was continued the following days. HALP caused a granulocytosis 3 hrs after injection attaining 300%. The granulocytosis was reduced or disappeared entirely with the second injection of HALP.
100–200 ml HALP reduced the leucocytes of 7 volunteers to 15% of initial counts. Granulocytopenia was compensated after 1 hour, lymphopenia after 6 hrs. A 5 days' treatment with high doses of HALP lowered the lymphocytes of patients with chronic lymphatic leukemia relatively more than of healthy volunteers. Crossreactivity of HALP and Cr51-labelled red cells could not be demonstrated in vivo.
