Zusammenfassung
1967 wurden im Zentrum für Innere Medizin Ulm 42 Infarktkranke, die innerhalb von 12 Std nach Auftreten des akuten Verschlußereignisses zur stationären Aufnahme kamen, während der ersten 3 Monate des Jahres mit Heparin, später regelmäßig mit Streptokinase behandelt. Die Wirksamkeit dieses therapeutischen Vorgehens wurde aufgrund der Letalitätsquote mit einer ohne gerinnungshemmende Substanzen behandelten, annähernd gleich großen Patientengruppe aus dem Vorjahr verglichen. Die statistische Auswertung der Ergebnisse zeigte einen gegenüber der Kontrollgruppe signifikanten Rückgang der Spätletalität bei den mit Streptokinase behandelten Patienten. Die Gesamtletalität konnte durch die Anwendung von Heparin bzw. Streptokinase von 41,7 auf 21,5% gesenkt werden. Eine vorläufige Auswertung der Elektrokardiogramme ließ bei einem Teil der Patienten eine ungewöhnlich rasche Rückbildungstendenz der akuten Infarktzeichen in das Folgestadium erkennen.
Summary
In 1967, forty-two patients with myocardial infarction were admitted within the first 12 hours following occlusion of the coronary artery. During the first three months of the year, they were treated with heparin and, thereafter, regularly with streptokinase therapy. The effectiveness of this therapy was compared to a similar group from the previous year treated without anticoagulants. The statistical evaluation of the results showed a significant reduction of death rate in the first month after therapy in those patients treated with streptokinase. The overall mortality rate could be reduced from 41.7 to 21.5%. A preliminary evaluation of the electrocardiogram indicates an unusually quick regression of the primary signs of the acute myocardial infarction.
