Beitrag zur Ätiologie der Hemiatrophia cerebri

Summary

A case of hemiatrophy of the brain with crossed atrophy of the cerebellum is reported. A 43 years old woman had been suffering from epileptic fits since her earliest childhood. Neurological examination indicated little spastic and athetotic signs on the right. At the age of 15 a carotisangiography was performed with thorotrast. 28 years after this she died because of a carcinoma of the liver. Histologically, one finds an excessive loss of neurons in the atrophic parts of the brain and a severe gliosis. We supposes, that an old cystic scar (Echinococcus cysticus) in the left Gyrus temporalis superior was the cause of epilepsy. We could not find any deposits of Thorotrast in the brain and in the scar by autoradiographic examinations, but there was much Thorotrast in the fibrotic changes of the liver, the spleen, the adrenals, the abdominal lymphonodes, and the left Arteria carotis communis. We think that the mentioned parasite was acquired at the end of the first year of life.

Zusammenfassung

Es wird über einen Fall von Hemisphärenatrophie links mit gekreuzter Kleinhirnatrophie berichtet. Die 43 jährige Patientin litt seit frühester Kindheit an epileptischen Anfällen. Neurologisch bestanden Hemispastik und Hemiathetose rechts. Tod 28 Jahre nach Carotisangiographie an einem Thorotrastcarcinom der Leber. Histologisch in der geschrumpften linken Großhirnhälfte schwere Krampfschäden bei relativ guter Erhaltung der primären Rindengebiete. Als Krampffocus findet sich in der 1. Schläfenwindung links eine Rindenschwiele (Echinococcus cysticus?). Autoradiographisch keine Thorotrastablagerungen in der Narbe und im ZNS bei hochgradiger Thorotrastfibrose von Leber, Milz, Nebennieren, Bauchlymphknoten und linker Arteria carotis communis. Die gute Kompensation der atrophischen, nicht fehlentwickelten linken Großhirnhemisphäre durch die gesunde rechte weist auf ein Erkankungsalter um das 1. Lebensjahr hin.
