Perforating granulomatous folliculitis in Becker's nevus

Summary

Acneform lesions strictly confined to a Becker's nevus were observed in a 18-year-old man. Histologically, a granulomatous folliculitis with partial destruction of the epithelial root sheath and follicular cysts with focal penetration of keratin into the surrounding dermis were present, ruling out acne vulgaris. The concept of Becker's nevus as a complex nevogenic process, affecting ectodermal, neuroectodermal, and mesenchymal structures, also in terms of dysfunction of the pilary complex, is suggested.

Zusammenfassung

Bei einem 18jährigen Patienten wurden acneforme Veränderungen innerhalb eines Becker-Naevus beobachtet. Das histologische Substrat bestand in einer granulomatösen Follikulitis mit teilweiser Destruktion der epithelialen Wurzelscheide und Follikelycysten mit fokaler penetration von Keratin in das Corium. Das Vorliegen einer Acne vulgaris konnte histologisch ausgeschlossen werden. Der Becker-Naevus repräsentiert einen komplexen ektodermalen, neuroektodermalen und mesenchymalen Prozeß auf naevogener Basis, bei dem auch eine funktionelle Störung im Bereich der pilären Komplexe vorliegt.
