202. Technik und Ergebnisse der totalen Gastrektomie

Summary

This paper gives details of indications for, and techniques and results of esophagojejunoplication. In 39 patients at the Goettingen clinic, there were 17 cases of benign diseases and 23 cases of stomach cancer. The essential feature of the operation is the use of a jejunum cuff around the anastomosis and the terminal esophagus. Particular advantages are the high degree of safety of the anastomosis-there were only two cases of insufficiency-and the prevention of postoperative pathologic jejunoesophageal reflux.

Dargestellt werden Indikationen, Technik und Ergebnisse der Oesophagojejunoplicatio. Bei 39 an der Göttinger Klinik operierten Patienten gab 17mal eine gutartige Grunderkrankung und 23mal ein Magencarcinom die Indikation zur totalen Gastrektomie ab. Wesen der Operation ist die Bildung einer Jejunummanschette, die um Anastomose und terminalen Oesophagus gelegt wird. Besonderer Vorteil ist die hohe Anastomosensicherheit—nur 2 blande verlaufende Insuffizienzen — und die Verhinderung eines postoperativen pathologischen jejunooesophagealen Refluxes.
