Summary
We have described a technique of one stage intraabdominal heterotopic pancreaticduodenal autotransplantation which has resulted in long term survival of dogs in good health. Malnutrition related to pancreatic exocrine hypofunction occurs initially but gradual recovery by 3 months nutrition is normal. In contrast, endocrine function is well preserved throughout the postoperative period except for a few days immediately after transplantation. Tolbutamide testing showed a significant prolongation of hypoglycemia as compared to controls although glucose tolerance curves were normal.
There was no fibrosis after pancreas- autotransplantation. This experimental model is useful for long term evaluation of heterotopic transplantation of the pancreas. Denervation and lymphatic interruption of the pancreas may cause a transient but not definitive influence on pancreatic endocrine or exocrine function.
Zusammenfassung
In unserer Versuchsanordnung haben wir ein Modell einer in traabdominellen heterotopen Pankreas-Duodenum-Autotransplantation geschaffen. Zwei Hunde überlebten für lange Zeit in befriedigendem Zustand. Zu Anfang, unmit telbar nach Pankreas-Autotransplantation, konnte ein Gewichtsverlust infolge exokriner Pankreasunterfunktion beobachtet werden. 3 Monate später hatten die Tiere wieder ihr Ausgangsgewicht. Im Gegensatz zur exokrinen Funktion konnte die endokrine über den gesamten Beobachtungszeitraum, mit Ausnahme weniger Tage unmittelbar nach der Transplantation, erhalten werden. Im Gegensatz zu Kontrolltieren zeigte sich beim Tolbutamid-Test eine signifikante Verlängerung der Hypoglykämie. Der Glucosetoleranz Test verlief normal. Eine Fibrosierung nach Pankreas-Autotransplantation konnte nicht beobachtet werden. Dieses experimentelle Modell ist sehr hilfreich für die Langzeit Beobachtung einer heterotopen Pankreas-Autotransplantation. Denervierung und Unterbrechung der lymphatischen Bahnen scheinen zwar einen vorübergehenden, jedoch keinen dauerhaften Einfluß auf die endokrine oder exokrine Pankreasfunktion zu haben.
