Summary

The great demand for vascular surgery of the supra-aortic branches with reasonable low risk (mortality below 1.5%) points to the question of adequate diagnostic procedures. Ultrasound Doppler examination has proven to be an excellent method of control before, during and after surgery. Thus, in conjunction with CT, in almost 50% of all operable cases, preoperative angiography is not required. For postoperative follow-up, Doppler studies are always sufficient. The application of more specialized diagnostic tools should be reserved for exceptional cases. The rate of restenosis is remarkably high (15%), as is the rate of complications in the primary disease (18% mortality within 3 years).

Zusammenfassung

Der große Bedarf von Gefäßoperationen an den supraaortischen ästen mit vertretbar geringem Risiko (Letalität unter 1,5%) wirft die Frage nach der adäquaten Diagnostik auf. Zur prä-, intra- und postoperativen Kontrolle hat sich das Ultraschall-Doppler-Verfahren hervorragend bewährt, so daß in nahezu 50% aller operablen Fälle in Verbindung mit einem CT auf eine angiographische Abklärung präoperativ verzichtet werden kann, postoperativ ist für die Erfolgsbeurteilung stets die Doppler-Untersuchung ausreichend. Spezielle Untersuchungen sollen Ausnahmefällen vorbehalten sein. Auffallend hoch ist die Rate an Re-Stenosen (15%) und Komplikationen der Grundkrankheit (18% Letalität innerhalb von 3 Jahren).
